IL Podiatrists and Scholl Students Volunteer at the 2021 Chicago Marathon
 
After a year hiatus due to the COVID-19 pandemic, the 2021 Chicago Marathon took place on Sunday October 10th. Podiatry was again well represented as an integral part of the marathon medical team. AAPSM Fellows Jeffrey Baker, DPM of the Weil Foot & Ankle Institute and Michael Chin, DPM of the Running Institute were podiatric co-coordinators for the event .

Other attending Volunteers were Katerina Grigoropoulous, DPM, Larry Kosova, DPM, Katie LaViolette, DPM, Paras Parekh, Amit Raj, DPM and Alison Young, DPM. Volunteering as well were podiatric medical students from the AAPSM Scholl Chapter. The podiatry volunteers were stationed at two medical tents at the finish line area and provided medical care to over 100 runners throughout the day.

The General Election for two seats on the North Brunswick Township Council is uncontested. On the Democratic ticket, incumbents Cologero “Carlo” Socio and Will Lopez, DPM are running for re-election. Dr. Lopez is a 15-year North Brunswick resident. A podiatrist, he has also served on the council for four years.

Type I Interferon Response and Vascular Alteration in Chilblain-like Lesions During COVID-19 
 
The outbreak of chilblain-like lesions (CLL) during the COVID-19 pandemic has been reported extensively, potentially related to SARS-CoV-2 infection, yet its underlying pathophysiology is unclear. This observational study was conducted during 9–16 April 2020 at Saint-Louis Hospital, Paris, France. All patients referred with CLL seen during this period of the COVID-19 pandemic were included in this study. We excluded patients with a history of chilblains or chilblain lupus. Fifty patients were included.
 
Conclusions: Our findings support an activation loop in the skin in CLL associated with endothelial alteration and immune infiltration of cytotoxic and type I IFN-polarized cells leading to clinical manifestations.

PRACTICE MANAGEMENT TIP OF THE DAY
Five Financial Reports You Need to See Monthly
 
There are five financial reports that physicians should review no later than the tenth day of every month, for the previous month's activity.
 
2. Aged Accounts Receivable (A/R) - The A/R report summarizes your practice's lifeblood: the potential revenue from payers and patients for services you've rendered. That's right: potential - because any number of things can hinder (or halt) payment to your bank account. And that's precisely why physicians must review their A/R on a monthly basis. Start with "Days in A/R," which indicates the average number of days it takes to collect on an account. [Total A/R ÷ (annual gross charges ÷ 365)]. "Days" of 30 or less is good. "Days" of 50 or more is not so good. And look at the percentage of total A/R in each aging category. More than 10 percent to15 percent in the 120+ days indicates a problem.
 
If the indicators signal trouble, analyze further. For instance, review A/R by individual payer to determine if specific companies are slow to pay, and identify why by analyzing explanation of benefits (EOB) for denial patterns such as noncovered service, applied to deductible, patient ineligible, or wrong modifier. If necessary, modify your operations or train staff accordingly to minimize denials.

Peter Ternus' response to the belittling post from Estelle Albright concerning the "use of woodworking tools" in the treatment of fungus nails was excellent. I would like to concentrate on the "be the physician that you are" aspect of that post. I prescribe terbenafine for onychomycosis in the following manner: I ask the patient if 1) he/she has ever had any form of hepatitis, or jaundice, 2) takes more than two pharmaceuticals every day for any condition, 3) has ever been told by a physician that there were unusual liver enzyme tests on a routine blood work-up. If they pass this test, I prescribe the drug. I also like to prescribe it if the patient is 35 years old or younger. I have never had a problem with it, not to say that it could not happen.
 
What is strange is that much of the public somehow knows that...

Sometimes when one wants to catalogue their experience to make a point, exaggeration of such, (usually unintentional), ultimately impugns their credibility. Dr. Abbott (in a thread about the complications of ingrown toenail surgery), claims to have under his belt "tens of thousands of these procedures over the span of a 30 plus years career" when discussing his experience with risks and complications of surgical intervention of ingrown toenails. 
 
At a conservative interpretation of these numbers, "tens of thousands" is a minimum of 20,000 and he would have us believe that it could be 30,000. So just in that range, over 30 years, assuming working 52 weeks a year, 7 days a week, he has done between 13 and 20 permanent ingrown toenail surgeries every week, or 675 to 1,040 procedures annually. His main point is that risks and complications are minimal, which I think most podiatrists would agree with. But I wonder what I might be missing if my very busy practice leads me to the performance of a permanent ingrown toenail correction by any method only 5 to 10 times a month, instead of 50 to 80.
