Podiatrists Volunteer at Boston Marathon
 
The 125th Boston Marathon was run October 11 after a year and a half delay due to the COVID-19 pandemic. The podiatry team was led by Jonathan Kaplan, DPM and consisted of podiatrists Drs. Paula Marella, John Pizzuto, Paul Heffernan, Kurt Neustrom, Joel Siegelman, Neal Zomback, Scott Aronson, Richard Cullen, Sarah Cullen, Dominic Garibaldi, Carl Conui, Quinn Charbonneau, and Satwinder Gosal.

Residents from local hospitals and podiatric students from the Temple University also participated in treating runners at the finish line medical tents of this event. Next year’s Boston marathon will return to its usual April date.

Kevin Kirby, DPM was recently interviewed on The Foot Health Show by podiatrist, Dr. Peter Allton, for a broadcast on UK Health Radio. UK Health Radio is billed as the “World’s #1 Talk-Health Radio” show with 1.2M+ dedicated listeners in 54 countries around the world.

Included in the show were discussions about the unique mechanical importance of the human foot and common problems seen and treated in podiatrists’ offices. In addition, Dr. Kirby explained how the concept of biomechanics can be used by the podiatrist to better evaluate and treat patients and how foot orthoses may be used as biomechanical treatments of many ailments of the foot and lower extremity.

The American College of Foot & Ankle Pediatrics (ACFAP) hosted its 6th Annual Conference at the Oasis Resort in Death Valley, CA on October 7-9, 2021. The in-person conference featured a National Park outing combined with exceptional CME lectures and hands-on pediatric workshops.

Marion Zuckerman Cohen and Gerald William Cohen, DPM of University City recently celebrated their 70th anniversary. The couple has three children, Ellen Cohen Mirowitz, Joyce Cohen Olshan and Howard Cohen, as well as eight grandchildren and four great-grandchildren.

This great couple met many years ago through Marion’s brother, Israel, while Jerry, from New Jersey, was at Washington University. He went on to podiatry school in Chicago after a steamy (very hot outside!) wedding on August 19, 1951. They returned here for him to practice podiatry and Marion to use her degree from Harris Teachers College.

Microlyte® SURGICAL Shows Positive Outcomes at Risk for Surgical Site Infections
 
Imbed Biosciences, Inc. has announced the results of a prospective clinical evaluation of its next-generation wound matrix, Microlyte® SURGICAL, in diabetic patients undergoing foot and ankle surgery who would ordinarily be at risk of a surgical site infection. Results will be published in the October issue of WOUNDS, a widely read, peer-reviewed journal in wound care and research.

In the study conducted by Ryan Chatelain, DPM at East Tennessee State University, 22 diabetic patients with neuropathy, infection, open wounds, history of recurrent infection, non-healing wounds, and/or peripheral vascular disease, were included. All patients were at-risk for surgical site infection (SSI) due to the sequelae of diabetes. The overall SSI rate for the diabetic patient is 13%, and the study’s primary objective was to decrease this rate to 6.5% by prophylactic treatment with Microlyte® SURGICAL wound matrix. Patients were assessed on day 3 or 5 for SSI incidence. The per-protocol patient population (20/22) had a 0% SSI rate with healing times typical to each procedure (between 10 and 34 days).

PRACTICE MANAGEMENT TIP OF THE DAY
Five Financial Reports You Need to See Monthly
 
There are five financial reports that physicians should review no later than the tenth day of every month, for the previous month's activity.
 
3. Adjustment. Also known as "write-offs," adjustments are the detailed categories to which staff post the amount between an actual payment and your standard fee. These include contractual adjustments and multiple procedure discounts, of course, but also adjustments for denials (such as patient ineligible or a coding error), and A/R clean-up actions (such as bad debt and small balance adjustments).
 
The report should be several pages long; adjustments are supposed to be granular. If your report contains broad categories such as "miscellaneous" or "insurance write-off," that's a problem. Ask your manager to explain vague sounding categories, as well as large dollar amounts, and request the addition of more detailed adjustment categories if necessary.

Query: Denial of Ketorolac IM Injection
 
We had a patient present to our office for a painful onychocryptosis. She stated that she was having severe pain even after taking her prescribed opioid drugs from a non-related surgery she had recently. A partial nail avulsion and matrixectomy was performed. We felt that the best option to manage this patient's pain that evening (insurance companies and family medicine offices were closed) and to prevent her from going to the hospital for post-operative pain was to give her a ketorolac IM injection. We utilize these injections at the hospital on a regular basis for post-operative patients. We billed for this injection and Highmark denied it stating that “No payment can be made because we do not cover this service when performed by this type of provider.”
 
We called for clarification and we were told that doctors of medicine, doctors of osteopathic medicine, dentists, licensed nurse midwives, pharmacists, and ambulatory infusion centers are the only ones that can give this injection. We appealed once with the arguments that (1) we order this in the hospital post-op all the time, (2) that we had no other options to manage this patient's pain but to send the patient to the emergency room and (3) that the list of people allowed to give such an injection sounds like those allowed to vaccinate and this is entirely different than a vaccination. We sent along medical records and showing proof of relief of pain from this injection. It was denied. We plan to appeal again but are looking for any insight or assistance with what may help us win this appeal?

Response: This sounds similar if not exactly the same as denial of payment for prescribing DME, performing a skin graft, etc. It’s because 3 letters after our name are not as good as 2. My advice is simple, yet complicated in the time frame it may take to get this issue resolved. Get your state component association to write a letter to the state board of podiatry in your state. That letter should request the state board to provide a letter that within your state scope of practice, podiatrists are licensed to not only order all medications for treatment within the scope of practice, but to perform the therapeutic injection for its use. They can elaborate on training, etc. but essentially that’s it. It will take time, but you should not be writing the state board yourself. This should come from your state component.

I just had a new patient who was on a ventilator for an extended period of time for COVID-19. She developed bilateral drop foot. I had not heard of this but found the article titled above. Peroneal nerve compression neuropathy makes sense. She is now able to actively dorsiflex one foot to 90 degrees but can't actively dorsiflex the other foot at all.

Dr. Markinson, with all due respect, any of my colleagues and friends, from this city, state, country, and several other countries around the world, who have known me for years would corroborate the numbers. Some people give back to others less fortunate in foreign settings in a big way, but don't need or want the “press.” I don't do ankle replacements or gastrocnemius lengthenings, but I have done/do significant forefoot work. I stand by my post and by my numbers.
 
Please don't be concerned about what you are missing, just do what you do best, enjoy your work and the rest will fall in place. I won't spend any more time defending my colleagues or myself. It's time to get back to work and fix some ingrown toenails!
