NJ Podiatrist Discusses Bunions
Bunions have sent many people, primarily women, to the podiatrist’s office, as Glenn Davison, DPM, owner of Advanced Podiatry in New York City and New Jersey and a Fellow in the American College of Foot and Ankle Surgeons, can attest. The good news is that, according to Dr. Davison, with early intervention it is possible to avoid bunions.

There are a lot of misconceptions about bunions and what causes them, with many people citing genetics, or even high heels as the inciting factor. However, it’s a little more complicated than that. “The bottom line is you get a bunion because of your biomechanics,” says Davison. When the foot’s arch drops, or pronates, this causes the muscles on the bottom and side of the foot to stretch. Pronation increases pressure on the foot’s inner area. Bunions occur when the muscles on the top of the foot try to compensate for this, pulling back, but causing bone to jut out.

KS Podiatry Practice Holds Sock Drive
The Foot Pain Center of Kansas City-Ottawa is holding its first "Socktober" sock drive to help those in need ahead of the upcoming winter. "We were looking to give back to the community, so we investigated opportunities that would relate to the practice and be good to the community," explained Jacob Goldstein, DPM a podiatrist at the Foot Pain Center.

Businesses around town Ottawa have received donation boxes and are accepting new socks of all sizes, colors, and patterns through the end of the month. The socks will then be collected by Dr. Goldstein and donated to Hope House. Having proper foot protection is crucial during the coldest months of the year, Dr. Goldstein said.

Five Financial Reports You Need to See Monthly
There are five financial reports that physicians should review no later than the tenth day of every month, for the previous month's activity.
1. Profit and Loss (P&L) Statement is a tally of revenue and expenses for a given period, and includes key financial indicators such as gross revenue, net revenue, and overhead. A well-developed P&L displays dollars and percentages for each revenue and expense line item, and compares them against previous periods and budget amounts. When reviewing the P&L, start with revenue. How does revenue this month compare to the same month last year? Is the practice on track to meet projections?
Then scrutinize expenses. Are they within budget? Comparable to the same period last year? In line with industry data for your specialty? If expenses seem "high," investigate instead of hastily slashing them. We've watched many physicians cut staff to save money, only to see overtime rise and collections drop because there weren't enough people to cover key tasks.

Query: Closed Reduction of a Dislocation
I’m looking for coding recommendations for a closed reduction of a distal interphalangeal joint dislocation, lesser digit, under local anesthesia, office setting.

Response: When a patient has a dislocation of the distal interphalangeal joint of a lesser toe and closed reduction or treatment is going to be performed, there are two CPT codes of note.
CPT 28660: Closed treatment of interphalangeal joint dislocation; without anesthesia
CPT 28665: Closed treatment of interphalangeal joint dislocation; requiring anesthesia
The dislocation was treated in an office setting under local anesthesia. Which of the above CPT codes would be the correct CPT code to bill? It all comes down to “anesthesia.” When you look at the Global Policy for Minor or Major Surgeries, the following is stated regarding “intra-operative services: All intra-operative services such as local anesthesia, injections, dressings, casts, splints, post-op shoes, wires, pins, supplies, etc. that are a necessary part of a surgical procedure are included in the surgery fee. Based upon the above, the correct CPT code to bill is 28660.

Several of the comments about the article on the break-in at Dr. Morris Stribling's office in Texas seemed to imply he was at fault and had done something wrong. Dr. Silver wrote, “why does he carry opioids in his office and how much is he prescribing to his patients?” I know Dr. Stribling and he is a fine man, businessman, and DPM. As well as having a podiatry office, he owns a licensed pharmacy in the same building as his office. If you look up the original article at KSAT.com, you will see the surveillance video of the criminal stealing from the pharmacy, not from Dr. Morris’ podiatry office.
Dr. Finer questioned the handling of the medications. A quote from Dr. Stribling in the original KSAT article states, “Some of them were controlled substances, but none of them were level one or two because we keep those locked away.” I have no doubt Dr. Stribling and his pharmacy follow all rules and regulations for storage and handling of their inventory. Podiatrists once again want to eat their young. Don’t attack a colleague because he was the victim of a crime by a criminal.
These colleagues ought to reach out and apologize for questioning the integrity of Dr. Stribling because of their lack of knowledge. We all ought to search out the whole story before we pen outrageous statements about one another.

I believe Dr. Tim Shea made a pertinent point about answering the inquiry as to whether a podiatrist used minimal incisional surgery when performing foot surgery. This is why I believe any podiatrist appearing for a deposition regarding any aspect of the practice of podiatric medicine should have a competent attorney at his side. Of course, if you are a defendant or an expert in a case, there would almost always be an attorney with you.
I suggest that you have an attorney with you, even if you are being deposed as a treating doctor in a simple personal injury case. The question as posed to us about MIS rightfully would have been objected to as lacking foundation. The question should have included a brief description of what is meant by MIS. Even with that, it could be objected to by some attorneys.
