MI Podiatrist Discusses the Mystery of Neuropathy
According to Conway McLean, DPM, "Neuropathy is a very general term which means simply “nerve problem.” This label can be applied to numerous conditions. Many are afflicted with idiopathic neuropathy, meaning we just don’t know why they have it. But the term is most often associated with the nerve changes of diabetes. This all-too-common disease of rising blood sugar levels leads to a dying-back phenomenon of the nerves farthest from your core. This equates to a loss of function of the tiny skin nerves of the toes. Sadly, this is usually a progressive process meaning it spreads insidiously, gradually, upwards. With time, nerve changes in the hands may be seen as this dying-back phenomenon spreads inward."

"Most nerve pathologies produce certain distinctive types of pain. Burning, tingling, or shooting sensations can be the result of both neuritis and a polyneuropathy. Numbness will be noted in the area of skin supplied by the injured nerve. This can lead to difficulties with balance and stability; consequently more falls occur in the presence of neuropathy. The research on neuropathy is on-going but many questions remain. Such is the case with diabetic neuropathy, which is an incredibly common problem but still not understood," says Dr. McLean.

CA Podiatrist Finishes 5th on Day 3 of Multi-Half Marathon Event
Vacation Races recently hosted the 2025 Coastal Redwoods Trailfest, a three-day trail running adventure through the breathtaking Redwoods of Northern California. Participants ran a half marathon each day, navigating stunning, yet challenging terrain surrounded by ancient forests and coastal beauty. Among the runners was Holly Spohn-Gross, DPM, a member of the APMA Board of Trustees.

When one participant suffered a Grade 2 ankle sprain, Dr. Spohn-Gross provided on-site care and secured a brace to support his recovery, embodying the supportive spirit of the trail running community. Holly, a lifelong runner with a background in competitive high school and collegiate athletics, joined Trailfest for the love of the sport and its camaraderie. She surprised even herself by finishing 5th overall on Day 3 among female runners out of a field of 305.

VA Podiatrist Discusses Surgical Complications with PA Podiatrist
Ben Pearl, DPM reviewed surgical complications with recently retired author and practitioner, Michael Downey, DPM on Fit Foot U, Dr. Pearl’s podcast. Dr. Downey stressed the importance of dealing with surgical complications head on with your patients. He mentioned that at one point in his career, it was not unusual for him to bring a patient back to the OR a couple of times a year while they were still in the recovery area or if they were hospitalized, and he was unhappy with the placement of an osteotomy or some other aspect of the surgery.

When there was a complication that occurred later in the post-op period, Downey was careful not to place blame on the patient, but rather he emphasized the optimum result he wanted to achieve for the patient. He also mentioned that his greatest of all time procedures were the tried and true, and he was slow to adopt less tested procedures. He also did not quickly adopt the latest surgical equipment as he found many of the options superfluous.

Podiatry Scholarship Aims to Attract Bermudians to Profession
A medical scholarship offered by a growing business is investing heavily in attracting Bermudians into the field. Atlantic Podiatry marked its first anniversary in April 2024 by offering a $47,000 industry scholarship to promote the next generation of healthcare providers. Matthew Francis, head of the practice, does a Talking Toes radio show on the first Tuesday of each month educating the public on the importance of podiatry.

“Two scholarships at close to $50,000 each for a business that’s still just opened is a lot,” Dr. Francis said. “But at some point, I’m going to need another podiatrist and it would be great to have a Bermudian. When we opened in 2023, it was just me. I added Megan Kelly, a senior podiatrist, last December. But I need to start thinking about our longevity.”

RE: The Passing of David Leshner, DPM
Dr. David Leshner, age 91, died peacefully at home after a long illness on May 14, 2025. David graduated from the Ohio College of Podiatric Medicine in 1956. He practiced podiatry at the St. Luke's Hospital Diabetic Clinic and at his office in Shaker Heights for 40 years.

Always interested in continuing education, David went back to college in his 60s, earning a Bachelor's Degree in Accounting (Summa Cum Laude) from the University of Akron in 1997, followed by a Master of Taxation degree in 1999. A lifelong athlete, David excelled at both racquetball and golf. In 1979, he was a member of the Ohio Racquetball Association Men's Team that won the state championship.

I started out as an electrical engineer, attending Milwaukee School of Engineering. My roommate and I ended up becoming best friends and it is funny how our lives progressed. I moved to South Texas and worked as a telecommunications engineer. Kevin went on to build Formula I engines for A.J. Foyt Racing. We both eventually chose a path in medicine. Because of my experience of tearing up my right ankle playing basketball at The University of Dallas, I wanted to do foot and ankle as a profession and attended the Temple School (PCPM at the time.) Kevin attended a DO school and is an ER doctor in Las Vegas, after serving some time in the Navy.
When we reconnected some years back, he was interested in my choice and I was interested in his choice. I never considered applying to a DO school for a good reason: what I wanted to treat. When I moved to South Texas, I ended up entering the seminary to study for the Diocese of Corpus Christi. I became friends with three individuals, all of whom left the seminary—as I did—and became doctors. All of them wanted to be surgeons. None of them ended up in the top 10% of...

Dr. Tomczak wrote, “As of this week, AACPM will not answer or even acknowledge my calls.”
I want to chime in here. I have also called AACPM several times over 3 weeks and sent emails as well with no response until I literally left a rather pissed off voicemail and email to them explaining that if I did not get a response, heads would roll. I finally got a response email that was not signed and claimed, “Due to the small size of our staff, we always recommend people reach out via email with any questions or issues.”
The problem with this response is that I had written several previous emails and called and heard nothing. Finally, after threatening heads would roll, I got a phone call back. The person that called could NOT explain what...

I want to acknowledge and thank two doctors who recently contributed to PM News, Dr. Gottlieb for his cogent argument for the 4-4-3 model and Dr. Jack Reingold for his affirmation that we are doctors. Both were clear on the need for a full educational experience, and it is best to have a rich broad educational experience and done, then focus on what you want to do or not do once you are in practice. This is the standard medical education/practice model. These discussions made me feel better after reading the tirades about the death of the profession and the need to have a degree change. I would like to address both of these areas of concern.
Let me first address Dr. Tomczak’s DO degree argument. If you are unhappy with the DPM degree, do you really think that a DO degree is going to improve your status with the public? I trained in a DO hospital and they suffered the same insecurities about their degree that podiatrists did then...

Sometime in 1981, shortly after starting my post-graduate training through NYCPM, I was told I had to join NYSPMA. I found that the local meeting was at a nearby now defunct hospital and ventured into what I thought was going to be a hornet’s nest. After all, I was misinformed that podiatry ate its young and you needed a sponsor to join the association. With many trepidations, I arrived at the meeting knowing that I would not know a single individual who could sponsor me. When I arrived at the room where the meeting was held, there was a kind man greeting others. He offered his hand to me and asked if he could help me. I informed him that I was a new graduate from ICPM and that I was back in NY to do my post-graduate work.
He firmly shook my hand and told me that he would not only sponsor me, but he gave me both his home and office phone numbers and told me that he would help me in any way he could. That man was Arthur Gudeon. He has been more than a friend and confidant to a generation of podiatrists not only in the NY area but to anyone...
