AMA Calls for Meaningful Use Reforms if Stage 3 is to Succeed
The nation’s largest physician organization is calling on the Centers for Medicare and Medicaid Services to fix the problems with what it calls the “troubled” Meaningful Use program, before moving on to Stage 3. In a Dec. 15 letter to Acting CMS Administrator Andrew Slavitt and National Coordinator for Health IT Karen DeSalvo, M.D., the American Medical Association submitted a set of recommendations with proposed revisions to the final Stage 3 rule released in October.
“The AMA has raised substantial concerns about the effect of the MU program on the practice of medicine and the innovation of technology. Yet, Stage 3, as currently drafted, continues to restrict innovations in technology for patients and physicians and creates barriers in moving to the new Merit-Based Incentive Payment System (MIPS) and alternative payment models (APMs),” states the letter. AMA’s vision for a “revised” Stage 3 is to address the current challenges with electronic health records and to provide a “glide path” towards MIPS and APMs.

PRACTICE MANAGEMENT TIP OF OF THE DAY |
Four Steps to Managing Unrealistic Patients
Manage these folks. Empathize with their situation, but maintain appropriate boundaries. Respect them as both customers and human beings needing attention. Respect the needs of yourself and your staff. Defuse the emotion on both sides of the interaction.
Try to remember that the real problem is probably more emotional than medical. It is scary to be sick and not know whether or not it is serious, or to be in pain and not know how long it will last. Lack of control increases anxiety and viable options relieve it. Some patients are outrageously demanding. They need to be asked to find care elsewhere.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1 |
From: Robert Kornfeld, DPM
PM News (12/16/15) had two posts relative to the battle podiatrists face with insurance companies and Medicare, not to mention the ICD-10 debacle.
For years, I have been reading these posts where DPMs look for ways to get paid or fight back against insurance company rejections. All I can ask is why? After all the years of commitment to education and the obtaining of medical and surgical expertise, how (and why) would any doctor allow themselves to be at the mercy of insurance companies in order to make a living? Why should any of you have anxiety while coding your claims? Why should any of you fear audits and...
Editor's note: Dr. Kornfeld's extended-length letter can be read here.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 |
From: Robert Scott Steinberg, DPM
Dr. Deutsch is mistaken to think, or even suggest, the PPACA was meant to be the end all of healthcare fixes. His comment is an unfair attack on the PPACA. Children can now stay on a parent's plan until 26. Insurance companies can no longer discriminate against those with pre-existing conditions. Millions more American now have a health insurance plan. Could the PPACA be better? Absolutely, but at every opportunity, the Republicans tired to kill the PPACA, instead of helping to make it better. Outside of the PPACA, employer sponsored health insurance and individual plan rates continue to go up, just like they did before the PPACA. And reimbursements continue to go down, just like they did before the PPACA.

RESPONSES/COMMENTS (CODINGLINE CORNER) |
From: Matthew B. Richins, DPM
We are having the same problem and are also being told it is an excluded diagnosis. During the appeals process, I was told, "bunions and calluses are excluded." I found that an odd choice in wording and wonder if there is an internist on their board who does not know the difference.

RESPONSES/COMMENTS (NEWS STORIES) |
From: George Jacobson, DPM
Here are six possible reasons why there were 87,000 fewer deaths in hospitals and a 17 percent decline in hospital-acquired conditions that occurred over the period from 2010 to 2014, due to Obamacare. I'm sure this list isn't complete so go ahead and add you own reasons.
1. Your network is so narrow it doesn't include your local hospital, so you die at home.
2. You can't get a pre-authorization, and die while waiting.
3. After your hospital stay, you learned what deductible meant and when you get the bill, you die from "sudden death syndrome."
4. Your doctor isn't on staff at the Obamacare hospital, so you don't go.
5. Fewer patients qualify for admission, so you can't get hospital-acquired conditions.
6. Your deductible and co-insurance are so high that you avoid the hospital and die just thinking about it.

RESPONSES/COMMENTS (YOU CAN'T MAKE THESE THINGS UP) - PART 1 |
From: Keith L. Gurnick, DPM

If shoes could talk! |
These shoes do talk plenty. They speak much about the biomechanics of the person who wears them. The wear pattern is symmetrical. There is even increased friction wear pattern across all five metatarsal heads and under the hallux and to the lesser toes (more so on the left shoe than the right shoe). The heels do not appear to show signs of lateral heel wear, which to me indicates a normal angle or gait, or a slow short step gait pattern, and possibly even a vertical calcaneal heel position in stance. Here is what the shoes tell me: "Time for new shoes."

RE: Outrageous ICD-10 Code of the Day
From: H. David Gottlieb, DPM
I agree that the "Outrageous code of the day" can be amusing as well as appearing to be improbable.
One should remember that these codes exist because they are used world wide. These amusing codes are not so amusing to the people that they happen to.
