AT THE COLLEGES
NYCPM 2016 Residency Fair the Biggest Yet
  
The New York College of Podiatric Medicine's (NYCPM) Residency Fair coordinators, Traci Bologna, Rebecca Calder, and Vrunda Dalal (all Class of 2018), went all out and produced the largest Fair to date on September 9th. Sixty-six hospitals (plus the U.S. Army) from 16 states plus the District of Columbia were represented; their tables occupied five large rooms as well as the second floor hallway.

Lisa K. Lee, Dean for Student Services and Enrollment Management, said that the Fair was a success due to the hard work of the coordinators and all of the NYCPM staff and student volunteers who also contributed their efforts. An alumni reception followed the residency fair; the alumni were all "very impressed with our current students," according to Dean Lee.

Keeping Feet Dry is Key to Preventing Bromhidrosis: NY Podiatrist
 
Why can some guys go sans shoes without clearing a room? “Some people are just more prone to excessive sweating,” says Michael J. Trepal, DPM, professor of podiatric surgery at the New York College of Podiatric Medicine. "Cotton is breathable and allows air to circulate, but can stay soggy," Trepal explains. Instead of switching your socks out every two hours, opt for a pair that wicks moisture away from skin but doesn't stay wet, like wool and certain synthetic blends."

How many times have you been running late, jumped out of the shower and dried off just enough to slide on jeans and shove your feet into socks and shoes? You're going to regret the results of the rush in a few hours. "You have to ensure that your feet are thoroughly dry before putting them into shoes to minimize moisture," Trepal says.

Jeffrey S. Gerland, DPM of Narberth, PA recently spoke about foot care, neuropathy, and podiatric issues at the 14th Annual Guillian-Barre Syndrome (GBS) & Chronic Inflammatory Demyelinating Polyneuropathy (CIDP) International Symposium that was held in San Antonio, Texas.

Why CMS Wants Surgeons to Code Every 10 Minutes
 
The comments flooded in after the CMS proposed in July that surgeons collect data on every 10 minutes of perioperative activity. “Undue burden,” the American Medical Association called the suggested requirement. The American Association of Neurological Surgeons and Congress of Neurological Surgeons warned that the approach “is onerous and will result in under-reporting of data.” To stop and code how they spend every 10 minutes of their time would be a tall order to ask of anyone, much less busy doctors and surgeons. So why did the CMS do it?
 
As the agency explains in its 2017 Medicare Physician Fee Schedule, the CMS pays for certain services, like surgery, as global packages, issuing a single established payment “for particular services that we assume to be typically furnished during the established global period,” which can be zero, 10 or 90 days. The problem with this is that the CMS was having trouble with the valuation of its 10 and 90-day global packages, which meant it was paying for it-didn't-exactly-know-what services. To determine precisely what services physicians were providing, “all codes are intended to be reported in 10-minute increments,” as either typical, complex, or critical inpatient or outpatient visits, the CMS said.

Oct. 1, 2016. This date has likely been highlighted on many a calendar at physician practices around the country. That's because this date marks the end of the one-year grace period from CMS for practices' use of these new diagnostic codes for Medicare fee-for-service claims. In other words, the flexibility on not having to be specific in ICD-10 coding will end on this day:

"The question we keep dodging is how to get the providers on board with all this logic [associated with ICD-10]. The road block I keep coming up against when I'm doing my best to 'sell' this concept of better documentation [and] more awareness of diagnosis coding is, 'Does this affect my payment today?' The answer is: 'Well, not directly, not today.'
 
"There are more denials for unspecified codes. For providers who have an EHR that requires that provider to select the diagnoses code prior to the claim being filed, this is an excruciating process that takes a lot of time. How can we convince them of the urgency and the importance for the future? As a coder, I can easily see the significance. On September 27, 2016, in a Diabetes Care report by Kamel Mohammedi, MD, PhD, et al. from the University of Sydney, using data from 11,120 patients with type 2 diabetes, it was found that the absence of dorsalis pedis and posterior tibial pulses were predictors of major macrovascular and microvascular events, death, and cognitive decline. The role of podiatric physicians as early detectors of this phenomenon can be crucial to quality of life, and even life-saving.

This is my first competition and I learned a lot! I encourage all my peers who have a little outside room to try their hands at apiary. Recently, I read that bees are considered an endangered species.  Raising bees can be tax deductible and, in some places, may qualify you for an agriculture break on you homestead taxes. You would not believe how my blueberries harvest has grown since I started raising bees.

In the aforementioned article, podiatrists are cited as "not trained at (allopathic) medical school, but rather complete four-year training at podiatric medical school." Although we are specialty trained at colleges of podiatric medicine, we are, in fact, trained as "allopaths". We DO practice allopathic medicine. Why our income is less is probably a reflection of our full scope of practice on a limited portion of the human body, among other factors. 
 
So, I never believed we were anything other than allopathic...
