MO Podiatrist Works Around Pandemic Challenges
For Franklin Harry, DPM, podiatrist at Best Foot Forward in Festus, treating patients in the era of COVID-19 has proved challenging, but doable. Take face masks. While it is important for physicians and patients to wear a mask to help in stopping the spread of the virus, masks make communication more difficult. “I work with a mostly older population, and it can be really frustrating,” Harry said. “In people who have trouble hearing to start with, the muffling effect of the masks just adds to the problem.”

But there’s a solution. “You just have to slow down, speak more clearly, articulate, and enunciate,” he said. “And it works both ways. I’m not afraid to say, ‘Excuse me?’ to a patient if I didn’t understand them. It’s just a matter of all of us being patient and understanding.”

PA Podiatrist With First-Hand Knowledge Treats Running Patients
When an athlete goes to a doctor with a medical issue, he wants to consult with someone who speaks the same sports language. Ideally, the expert has already run “in the patient’s shoes,’’ i.e., he can identify and relate to the problem at hand. This is why so many elite runners visit Ira Meyers, DPM, a podiatrist from Horsham who just happens to have 90,000 miles on his personal odometer.

His approach is as much about treatment as it is for providing preventive measures to avoid injuries. “You look at elite runners — about a third or half of their time is spent running,’’ says Meyers, who just celebrated his 60th birthday with a 100-mile bike ride. “The other two-thirds to half are doing stuff so they don’t get injured. “I know what to do and I tell patients what to do.’’ After years of elite running and racing, Dr. Meyers has branched out into triathlons. Having run a 2:22 in the San Francisco Marathon back in the day, Meyers knows of what he speaks.

CA Podiatrist Opens Practice with Orthopedists
Four surgeons opened an orthopedic practice in Pasadena, CA. Pasadena Orthopedics is a new office run by orthopedic surgeons Annette Billings, MD, Lacey Zack, MD, and Robert Klenck, MD, and podiatric surgeon Sara Shirazi, DPM. They have practiced in the Pasadena area for 15-plus years.

Foot Specialists of Tri-County founder Lanhuong Pham, DPM has announced the addition of a second provider, Joel Robinson, DPM, MS Med.Ed. Dr. Robinson completed his Doctorate in Podiatric Medicine at Kent State University, College of Podiatric Medicine in Independence, OH.

Considerations for Integrating Medical Transcriptions into Your Practice - Part 6
If you too are looking at leveraging transcription to streamline the operations of your practice, here are a few crucial considerations that will help you make an informed decision:
6) Conduct thorough Research of the Provider - You need to get in the habit of asking for a complete list of the company's references within your specialty, not just a fixed list of positive reviewers. Find out from other physicians in your field what their transcription service plans cost them, how they use them, and what the quality of their service is.
You can also find ample reviews online and dig up information related to the company on various public forums. When outsourcing, make sure you have looked at all the services that the company or transcriptionist is prepared to offer and if they actually do what they say they can.

Query: Routine Foot Care and ABNs
Does the patient need to sign an ABN if foot care is not covered? An example would be a patient with adequate pedal pulses and no class findings. In the past, I have read that ABNs are not necessary because the service is never covered. I know if they have class findings, they are usually covered, but if the patient chooses to come early, do we need to have them sign an ABN?

Response: Technically, no they don’t. I made it a habit in my office to have at least one per year on file for those patients as a back-up, but for statutorily non-covered services, it is not necessary. It is vital to have your documentation verify the lack of coverage findings too. As you know, patients can be confused about their prior visits, and their friends may note they “are always covered”; so I have them do at least one ABN.

To accomplish your goal, consider adding a lateral flare and buttress to his/her footwear. High-tops work best. Lateral stability is hard to achieve even in an Arizona AFO unless there is extra lateral support. Elevate for equinus conditions. And don't forget about rocker-soles for limited sagittal plane motion. Shoe inserts play a big role, as well.

Dr. Rosenblatt changed the subject a bit, to one about his impression that the lack of in-person interaction is having a negative effect on our health. I feel that is an exaggeration. I have been using the Internet for social networking since 1999. In 2005, I began using video conferencing for my Internet international Rotary Club meetings. We had members from Madagascar, Italy, Germany, Portugal, Sweden, UK, and the U.S. Quickly, it took on the feel of in-person meetings.
Since March, much of my life has been via Zoom. Hospital MEC and credentials meetings are via Zoom. They have proved far more efficient. My Rotary Club is also using Zoom. We have guests join us from all over the world, and it feels normal. I love to go to meetings, but I am not going to be able to do that safely. I choose to make the most of new opportunities. My point: it's about one's attitude. Make the most of what you have. We are physicians. We can think rationally about the situation in which we find ourselves.
