Pain on Top of Foot May Indicate a Bone Spur: CA Podiatrist

"Foot pain can be debilitating, affecting every aspect of life," says Albert Nejat, DPM, a podiatrist and foot surgeon in Los Angeles. "Fortunately, most foot pains can be addressed with simple measures, like changing habits, doing stretches, wearing different shoes, and altering your training practices."

Dr. Albert Nejat

Pain on the top of the foot may indicate a bone spur. "A bony prominence can get irritated by ill-fitting shoes putting pressure on that area," Dr. Nejat says. "You can simply change the way your shoelaces are threaded so it avoids the bump, and doesn't put additional strain on it." Cortisone injections and anti-inflammatories can help reduce pain in the meantime.

NY Podiatrist Publishes Autobiography

Vincentian-born podiatrist and former New York City Councilman, Kendall Stewart, DPM has written his first book, recapping his life growing up on Union Island, in the southern St. Vincent Grenadines, to becoming a New York City elected official. “I wrote it because I wanted individuals to understand some of the struggles I went through to achieve my goals and encourage readers to not let barriers hinder them from reaching greater heights,” Dr. Stewart, who served in the City Council from 2002-2009, told Caribbean Life about An Island Boy’s Story: From Union Island to New York City Council.

An Island Boy's Story

Dr. Stewart said he began practicing podiatric medicine in 1981, and also tutored students working towards their high school equivalency examinations between 1979 and 1981. Two years later, he said he became director and chief surgeon at Comprehensive Foot Care in Elmhurst, Queens, a position he held up to 1986. That same year, Dr. Stewart said he became director and chief surgeon at Unique Foot Care of Brooklyn until retiring in 2019.

DrChrono App Helps Patients Access Their Records: CA Podiatrist

DrChrono Inc., a company developing the essential platform and services for modern medical practices, has updated its medical platform to allow patients to share important and relevant information with their DrChrono provider directly from the Apple Health app with iOS 15. Beginning this fall, patients whose providers use DrChrono will be able to share certain types of data - such as heart rate, detected falls, hours of sleep, exercise minutes, labs, and more - in a secure and private way, allowing them to have more informed, meaningful conversations with their doctor.

Dr. Ali Sadrieh

“The movement to provide patients direct access to their records is long overdue, and all along, DrChrono has been at the forefront of this. Now taking advantage of Apple’s newest features, DrChrono is once again ahead of the race in providing what matters most to patients - an excellent experience and ease of sharing data with and from their providers,” said Ali Sadrieh, DPM, founder and surgeon at Evo Advanced Foot Surgery.

PRACTICE MANAGEMENT TIP OF THE DAY
5 Can’t Miss Financial Strategies for Practices - Part 2

2) Analyze Denial and Accounts Receivable

The most successful practices analyze their claims denials and take action to reduce them. They also closely track accounts receivable.
  • Compile a list of denials by payer and see why they are happening and take action to minimize them.
  • Group your accounts receivable and analyze those that fall into different aging groups, such as 30 days and 60 days old to determine the cause. If it’s something you can control, take action so you get paid sooner.
  • Make sure all claims are filed in a timely manner and that all denials are appealed.
  • Your billing department should take aggressive action to eliminate all denials and delays, whether from payers or patients, to keep the cash flow positive.

RESPONSES/COMMENTS (DME)
RE: MACs Resume Medical Review on a Post-Payment Basis
From: Paul Kesselman, DPM

Beginning August 2020, Medicare Administrative Contractors (MACs) resumed post-payment reviews of items and services with dates of service before March 2020. MACs may now begin conducting post-payment medical reviews for later dates of service. The Targeted Probe and Educate program (intensive education to assess provider compliance through up to 3 rounds of review) will restart later. The MACs will continue to offer detailed review decisions and education as appropriate.

Interpretation: Claims with DOS March 2020+ are now subject to a post-payment audit process. However, the waiver for certain LCD and NSC requirements (e.g. certain, but not, all face-to-face meetings, written proof of delivery-WPOD, etc.) are still in place. Upon review, if your chart does not document the absence of waived LCD and NSC requirements due to COVID (e.g. patient refused to sign a WPOD because they did not want to handle your pen/paper due to COVID fears), then your claim will be subject to recoupment. This is true for all MACs, DMEs, and LCDs.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Bahaa Amer, DPM

I don’t think we as podiatrists should focus our effort on becoming equal to MD/DO/NP in providing primary care because simply we are not trained to do primary care. We should focus our efforts in getting equal in things that we do best, for example, wound care. I took the CWSP board certification exam which is the same exam that MDs and DOs take, but unfortunately most of the wound care centers and hospitals prefer to hire an MD or DO because they can treat wounds on the whole body vs. podiatrists who can only treat lower extremity wounds, which I find very absurd and unfair to us as podiatrists.

Simply (and I say that with the most humble way), we treat and understand wounds better than anyone else in the medical field. If the American Board of Wound Management will treat us equal to MDs and DOs in terms of the board certification exam, then that should be reflected in the scope of practice and the ability to treat wounds on the whole body. I am a foreign medical graduate and proud to be a podiatrist, but our podiatry education does not qualify us to provide primary care. However, it does qualify us to become lower extremity experts medically, surgically, and in wound care.
