Toe Separators Can Benefit Hammertoes, Bunions, and Neuromas: NY Podiatrist
 
“People with hammertoes, bunions, and neuromas will all benefit from toe separators,” says Emily Splichal, DPM. While some severe cases of these conditions could require more intense treatment, she still suggests that her patients with rigid hammertoes or severe bunions incorporate toe separators into their daily routines. “Bringing even a little mobility and alignment of the forefoot is beneficial,” she adds.

Particularly in the case of bunions, recent research backs her up: A small 2018 study of 56 women with moderate bunions published in the Journal of the American Podiatric Medical Association found that a combination treatment of toe separators with foot mobilization exercises resulted in a significant reduction in bunion severity after one year of use. Dr. Splichal has found that the results from toe separators tend to be more pronounced in her patients who wear them more regularly.

 Surgical Treatment Best Option for Chronic Ingrown Toenails: Spanish Podiatrist 
 
Regarding the treatment of ingrown toenails, Javier Alcalá, collaborator of the General Council of Colleges of Podiatrists of Spain and member of the Spanish Association of Podiatric Surgery, says it can be conservative or surgical: “For those patients who present the problem sporadically or eventually, a conservative treatment can be considered with periodic visits scheduled by the podiatrist as long as they are spaced apart in a reasonable time. On some occasions, it will be necessary to use anesthesia to eliminate the nail fragment (spicule) causing the problem and perform a proper canal cleaning.”

The best option without a doubt is to opt for surgical treatment. "Today, we perform interventions to narrow the nail with local anesthesia, completely painless and with a post-surgical procedure that requires practically no rest and no or minimal discomfort. To all this, we must add success rates very close to 100% and minimal complications, which makes this treatment the most recommended option when the problem is recurrent. It is a very safe, effective, and definitive treatment," says the expert.

Query: Trouble With Injections
 
Medicare has denied our recent codes for CPT 20550 as well as CPT 64455, for plantar fascia injections and neuroma injections, stating that these are “not medically necessary.” We are having the patient sign ABNs for these so that we can collect payment, however, these codes have been covered in the past. The diagnosis codes being used include M77.51, M77.52 for CPT 20550 as well as G67.61 and G57.62 to code for neuromas. Are we doing something wrong? My biller explained to me that because I am not an anesthesiologist, Medicare is denying the codes.

Response: Both of these are payable CPT codes by CMS. Check your EOMB as the denial codes listed might offer some insight as to why the claim was denied. I would then appeal the denial.
 
If the telephone person stated that not being an anesthesiologist was the denial basis then I would contact your state association. There could have been some computer/policy update done and something glitched that needs to be corrected as soon as possible.

"I believe there are at least a dozen states which allow nurse practitioners to practice independently, which also includes full rights to write prescriptions. I believe changing a state’s practice act to allow podiatrists to treat the foot and ankle medically and surgically and also provide primary care may be politically feasible in some states." - Herbert
 
If you look at the infographic, the cause is being pushed that an NP is just as capable as a doctor or PA. Whether we, as physicians, agreed with this or not appears to have no bearing on the future of the NP and healthcare.

RE: Scope of Practice of Nurse Practitioners

I first became involved with nurse practitioners about 30 years ago in Wyoming. The several that I knew were RNs who had completed a two-year RN program and I believe had an additional 18 months of training to become a certified nurse practitioner. They did not even have a bachelors degree at least at that time. At that time, my wife, who has a masters degree in nursing, was teaching in one of those two-year RN programs. If I had a cardiac, GI or allergy problem, I would not expect anyone providing primary care to treat me for it. I would expect a primary care provider to refer me to a specialist in that case.
 
The era of the plenary medical degree is over. Physicians should be providing specialty medical or surgical care. Most of the time, primary care, if done properly, simply means determining if a specialist referral is necessary. Podiatrists should be able to assess a surgical patient that they are licensed to perform surgery on. This would theoretically mean doing a complete body and systems assessment and determining if the patient has any problems that would preclude the intended surgery. If podiatrists cannot accomplish this type of whole body assessment, they should not be practicing!
