NY Podiatrist's Brain Cancer Non-Profit Joins Forces to Fund Research
 
StacheStrong and the Musella Foundation for Brain Tumor Research & Information have announced a joint effort to fund five separate groundbreaking research projects aimed at combating brain tumors. These non-profits have combined their resources to donate $160,000 towards cutting-edge research at UCLA, University of Illinois, Harvard, University of Chicago, and Columbia.

Response: The CDC broadly defines a “chronic illness” as one that has existed for at least one year and has been under active medical management or limits activities of daily living, or both. We see many chronic illnesses which could simply be a painful bunion that has existed for years but has now become symptomatic enough to require treatment or a change in prescribed therapy.

Any information you can provide is appreciated! Device used, cost, number of treatments, and your own personal experience will be appreciated.

Try increasing the Ativan to 2.5mg. or consider 10mg. of Valium which has a longer life. Watch out for interactions with other drugs. Also, patients sometimes complain of headache after Valium. It also has mild muscle relaxant properties.

I have not prescribed anti-anxiety medication for a long time for any type of surgery, including bone surgery, but if I have a very anxious patient, I prescribe Valium 5mg, two tablets, and ask the patient to take one tablet with a sip of water 2 hours before the surgery and if still anxious the second tablet 1 hour prior to the surgery. But what I really believe helps the patient's anxiety is to prepare them for surgery thoroughly, explain to them what to expect at the surgical center or hospital, step by step, and address their questions and concerns. That will help patients stay more relaxed and comfortable. My advice is to spend time with the patient going through the surgery and the process, and you may not need anxiolytics. I try to avoid them and I usually let the anesthesiologist handle anxiety issues.

A few years back, I wrote a book on pain management, but never found a publisher. There was a chapter on the use of potentiation so that you could obtain greater effect from a narcotic and use a lower dosage via use of a potentiator drug. There are different categories (tricyclics, anti-psychotics, anxiolytics, SSRIs), but they work the same way (the explanation of which is too long to cover here.)
 
Navane (Thiothixene), Xanax (alprazolam), and a few others have been used to varying effect. When I did an office procedure in my surgical room, I would have the patient take a Vicoprofen (hydrocodone and ibuprofen) [now discontinued] with a 5mg or 10mg Valium (diazepam) (dosage dependent upon body mass) with food about an hour pre-op. The majority of the patients slept through the procedure and the ones who were conscious appeared completely at ease and relaxed. The one med would potentiate the effect of the other without increased mu receptor activation and respiratory depression.

I read of the difficulty of Dr. Carnett getting his DPM license in the U.S. after an attempt to obtain licensure in China. When I examined the practice law in NV, I did not see a specified residency requirement. I do not know if it requires more than one year, but it only states that the residency must be "approved by the Board." I am not qualified to opine about his potential to take various board exams. 
 
Perhaps I should not speak for NV, since I was never licensed there. However, I live here. My understanding is that we only have about 170 podiatrists in the entire state, most of them in Las Vegas or Reno. We have an excellent practice law. I hope that the present DPMs here don't object to my suggestion of Dr. Carnett considering getting licensed here. 
 
I apologize if I am mistaken about the number of years for the residency requirement. I simply do not see it specified. Personally, I love living here. I have a fine DPM who takes care of me locally. He is the son of one of my classmates. I believe he had a multi-year residency. At the least, Dr. Carnett should consider filling out a NV application. I personally would like to see a "few more" DPMs licensed here. I do not know if that is the desire of the present licensees.
