PA Podiatrist Discusses Natural Remedies for Common Foot Problems 
 
If your feet have been in hibernation thanks to all your boots and closed toe shoes, foot surgeon and author of Feet Naturally, Dr. Maasi J. Smith, has some tips to get them in sandal shape STAT!

1. If your feet or ankles tend to swell, eat watermelon. This fruit serves as a natural diuretic, helping to rid the body of fluid.
 
2. If foot odor is a problem for you, consider soaking your feet several times a week in a vinegar bath. Apple cider or white vinegar will do. Mix about a 1/2 cup per 1/2 gallon of water. Soak for about 10 minutes.

TX Podiatrist to Help Develop New Hospital-Based Wound Care Center

Dr. Richard C. Galperin will be part of a newly developed multi-specialty wound care center at the University General Hospital in Dallas, Texas. He will be part of a three-physician team specializing in the medical and surgical treatment of ulcers and wounds, including hyperbaric medicine. The center is set up to treat hospital-based patients, as well as to be utilized as an outpatient clinic. Wound care has been an important part of Dr. Galperin’s practice over the past 20 years. He currently serves as principal investigator for several diabetic ulcer studies in his private practice.

FL Podiatrist Indicted on Healthcare Fraud Charges
 
A federal grand jury has indicted a Lakeland podiatrist on charges of healthcare fraud. Dr. James F. Pruchniewski was arrested Friday, according to William Daniels, a spokesman for the U.S. Attorney's Office in Tampa. The grand jury's indictment alleges that Pruchniewski, as the owner and operator of the North Lakeland Foot Clinic, submitted false claims to Medicare. Pruchniewski pleaded not guilty to the charges, records show.

QUERIES (CLINICAL)
Query: Post-Injection Heel Complications
 
My patient is a 53 year old woman with a medical history significant for brain trauma and right-sided hemiparesis. She presented initially for heel pain on the left heel which was treated in December 2013 with stretching, taping, injections containing 1.5 cc 2% lidocaine mixed with 0.5 cc Kenalog-40, and 0.5 cc dexamethasone. She has adequate dorsiflexion in the ankle, so equinus is not an issue.  
 
Post-Injection Heel Complications
 
Following her injections, her pain resolved, but she then developed varicosities in the medial arch and blanching in the area plantar medial arch at the site of the injection. The varicosities were painful and treated with a mix of mometasone and lidocaine jelly. The blanching noted in January and February has now become erythematous but is without pain, heat, or fluctuance in the plantar medial arch. It blanches easily. Her medial calcaneal tubercle is also becoming symptomatic again. Suggestions?

Query: Are we legally allowed to ask a potential employee if he or she smokes?

Are we allowed to ask a person whom we are considering for the job if he or she smokes, and can that person be excluded from employment if the answer is yes? Would that be considered discrimination? We do not allow smoking in our office, but it would seem hypocritical if I lectured a patient on the importance of not smoking and they saw one our employees standing outside the building with a cigarette in her mouth.

This lesion looks very much like an enchondroma, and the history fits - it has been asymptomatic, discovered coincidentally to a different problem, as well as the radiologic findings. I would NOT do a biopsy at this point for several reasons. First, it is asymptomatic. Second, if indeed it is an enchondroma, it may be part of a larger condition where patients have multiple lesions elsewhere. Sometimes, enchondromatosis is associated with hemangiomas of the soft tissues. An orthopedic oncology consult should be done first. Those experienced in dealing with these lesions can make the diagnosis by radiology alone, and any extended imaging work-up would be more prudent than a biopsy at this time. The 20-year history is also highly tending towards it being benign, although the lesion is expansile, as some enchondromas can be. An orthopedic oncology consult is the BEST thing you can do for this patient.

Assuming you have some experience with bone tumors, I think your plan is appropriate. This looks like an enchondromatous lesion (look for Ollier’s disease) but it may be a chondrosarcoma or other ominous mass. If you are not used to packing bone or dealing with an osseous malignancy, refer this case to an appropriate colleague now instead of when and if you hit a wall.

These 'punched out' type lesions could be associated with Behcet's syndrome or a collagen vascular disease.  I recommend a punch biopsy of a lesion and an arthritis panel, in particular, to rule out collagen vascular diseases with secondary Raynaud's phenomena.

RE: Using Nitropaste to Increase Blood Flow

In this time of studies and verifiable proof of effectiveness, I can only offer what I have seen. 
 
I once used nitropaste on a patient, and tested perfusion with a laser Doppler. The digital perfusion increased 300% in five minutes. When I applied nitroglycerin paste as an RN (treating acute MI), I would apply the paste to a piece of paper and tape the paper to the person's chest. The PDR distinctly states...
