CO Podiatrist Uses 3-D Printer to Produce Ultra-Thin Orthotics
 
Dr. Greg Stilwell is a retired podiatrist who has been researching orthotics for "minimalist" runners who use very thin running shoes instead of traditional running shoes. Because many of these runners have experienced injuries due to the shoes providing little impact protection for the feet and legs, Stilwell designed an insert that will work inside a minimalist running shoe, helping to prevent these injuries.

Looking to someday market the inserts, Stilwell was working with the San Juan College School of Trades and Technology Enterprise Center to develop a business plan, and the center referred him to David Scott and Scott Seavey who, along with their students, utilized the 3-D printer to make a mold for the inserts. It took the printer three hours to manufacture the mold. Stilwell hopes to continue to utilize the Enterprise Center to incubate his business, Hozhoni Health Services, and is currently researching manufacturing equipment that he can use to make the inserts locally.

NY Podiatrist Sells Custom-Made Shoes Designed to Last a Lifetime 
 
Choosing style over fit leads to long-term problems, says Dr. Bruce Pinker, a White Plains foot and ankle surgeon, including the inability to wear said shoes with any measure of comfort. “Feet have to be functional and most footwear is not functional,” he says. That realization led to a new business for Pinker: Dr. D-LuCS, a line of custom-made shoes for men and women. And they are popular: He estimates that he has designed and sold between a thousand and two thousand pairs a year since starting the business in 2010.

But Dr. D-LuCS will cost you; shoes are $2,795 and the boots are $3,995. Pinker defends the cost by noting that his shoes are handmade with leather specifically for each customer. And each pair is designed to last a lifetime. He offers a $349 refurbishing program in which customers can bring their shoes in at any time and have the outer soles, lining, laces, and details redone.

Retired NC Podiatrist Finds Success Keeping Things Dry
 
Summer is the high season for Dry Corp., an innovative Wilmington company that's all about keeping things, well, dry. Summer is the season when kids get out of school and become active – active enough to sometimes break an arm. The company's Dry Pro products seal up those casts, with a sleeve forming a vacuum to keep out the water, explains former podiatrist Dr. Roy Archambault, head of the company he started in 1998. It's something that saves vacations and trips back to the hospital for new casts.

The combination of innovative products and the company's culture of collaborative ideas and camaraderie are the ingredients for success, said Archambault, called Dr. Arch by his staff. That success was enough to earn the company the title "Small Business of 2013" by Business North Carolina magazine.

Query: Bone Biopsy
 
My billing department keeps changing my bone biopsy codes from CPT 20245 (biopsy, bone, open; deep [e.g., humerus, ischium, femur]) to CPT 20240 (biopsy, bone, open; superficial [e.g., ilium, sternum, spinous process, ribs, trochanter of femur]) when I perform a bone biopsy of a metatarsal during a transmetatarsal amputation. I interpret the codes to be superficial and deep. 
 
When simply taking a trocar biopsy or rongeur or other instrument through an already open deep wound, I believe it to be a superficial bone biopsy since it lies just beneath the subcutaneous tissue level. When, however, an incision is made with full dissection for complete exposure of the metatarsals and multiple biopsies are taken with clearance cuts, etc., I believe that to be a "deep" procedure. Am I interpreting this code incorrectly?

Response: No, you are not. Your billing department is correct that the open biopsy of bone you are performing is considered "superficial" (not even closely equal to a bone biopsy of the humerus or femur). 
 
Regarding your bone biopsy performed of "metatarsal during a transmetatarsal amputation," I'm a little confused. Since you are submitting the distal foot in its entirety to pathology following the transmetatarsal amputation, the only way you would be able to independently also bill and be reimbursed for a bone biopsy is separately operating to removal a portion of metatarsal bone proximal to the amputation site. Is that what you are doing? Why not request that pathology study the quality of the bone with culture of the proximal portion of the amputated foot?

I've read with amusement DPMs complaining because treating RFC patients conflicts with scheduling new patients. My advice would be learn how to run your practice instead of complaining about RFC. New patients should get priority booking. If your front desk isn't getting that new patient into your office the day they call or the next day, they'd better have a great reason and "there weren't any openings" isn't one of them. 
 
Competition among health professionals for patients is only going to increase. The solo practitioner shouldn't let any opportunity pass because his days are numbered, and when they're gone, he's gone.

Dr. Solomon recently made the following comments: "Even if we know our negotiated fee, in many instances, we aren't even allowed to collect it. I pose the question: "In what other business is it required that instant credit be extended to all customers, and with terms that are undefined and difficult to enforce?"
