Borgess ProMed Family Practice has added Sarnarendra Miranpuri, DPM to the physician staff at its Woodbridge Hills location. Dr. Sarnarendra Miranpuri comes to Borgess from the Detroit Medical Center, where he was director of clinical research in the Division of Podiatric Surgery. He has worked extensively in clinical research, and speaks nationally and internationally on diabetes-related foot and ankle deformities.

Miranpuri has a podiatric medical degree from Barry University School of Graduate Medical Sciences in Miami Shores, FL. He also completed fellowship training in reconstructive surgery of the diabetic foot and ankle at the Detroit Medical Center.

Oral Antibiotics Equally Effective as IVs for Bone Infection Treatment in Kids 
 
A study suggests physicians should reconsider osteomyelitis treatment strategies. Researchers report in JAMA Pediatrics that treating osteomyelitis in children with oral antibiotics is equally effective as intravenous antibiotics without the potential complications of using a peripherally inserted central catheter (PICC line). The multi–institutional study, led by Children’s Hospital of Philadelphia’s (CHOP) and Cincinnati Children’s Hospital Medical Center, appeared Dec. 15 in the journal’s online edition. 
 
Based on their findings, the researchers – all part of the Pediatric Research in Inpatient Settings Network (PRIS) – suggest physicians reconsider using intravenous antibiotics to treat this condition because of that treatment method’s complications. “Complications such that come with using PICC lines include blood clots, sepsis and are quite serious,” said Samir Shah, MD, MSCE, director of Hospital Medicine at Cincinnati Children’s and study co–author. “We can avoid such complications by using oral antibiotics with excellent outcomes for children.”

Hospitals Buying Doc Practices May Reap Tax Savings Under IRS Ruling
 
A new Internal Revenue Service ruling could mean significant tax savings for investor-owned hospitals and management companies that operate physician practices in states with laws that bar the corporate practice of medicine. Many states have laws that allow only licensed physicians to employ individual physicians in the practice of medicine. The idea is to keep non-medical practitioners from exercising undue influence over doctors' clinical judgments. But as hospitals and other entities increasingly have acquired physician practices in recent years, they've had to perform some legal gymnastics.
 
Essentially, they've been acting like owners without actually being able to include the practices as subsidiaries on their tax returns. That means they couldn't use any of a practice's losses to offset taxes on other income, said Kelvin Ault, a partner at PricewaterhouseCoopers in Nashville who represents the organization that sought the ruling, which he declined to identify. But the IRS private-letter ruling, which was published in the Federal Register on December 19 allows Ault's client organization to consolidate its operations when filing federal taxes despite a state corporate practice of medicine law. The IRS private-letter ruling did not name the organization.

While last minute holiday shopping is probably the main thing on your mind, it’s important that you don’t let your tax strategy fall to the wayside. According to Patrice C. Washington, the Money Maven of the Steve Harvey Morning Show, waiting too late could affect your ability to benefit from some time-sensitive tax advantages. Here are three tax tips you can tackle at any income bracket.
 
3. Monitor Retirement Contributions: If you didn’t save the max of $17,500 in your 401(k) this year, there’s still a possibility your employer will allow you to make a lump sum contribution before December 31st. If you’re over the age of 50, you still have time to max out at $23,000. April 15, 2015 is the deadline for funding a traditional or Roth IRA for 2014 with either $5,500 for those under the age of 49 and $6,500 for individuals age 50 or older. “If you’re an entrepreneur or small business owner and want to save in a solo 401(k), make sure to at least open your plan by December 31st,” adds Ms. Washington. “You will then have until the April 15th tax deadline to fund it.”

Query: Revisional Lapidus Non-Union Coding
 
How would you code a revisional Lapidus-type procedure non-union using bone allograft?

Response: There are three codes that could apply:
CPT 28320 which is repair of non-union tarsal bone or 
CPT 28322 which is repair of non-union metatarsal bone or 
CPT 28740 which is an arthrodesis tarometatarsal joint, which you are repeating (i.e., you are repairing only the fusion part of your original CPT 28297 procedure). 
 
The first two are typically reserved for intraosseous non-union pathology. The attempted fusion of the first metatarsal and the medial cuneiform tried to "create" a single bone. The latter code takes what did not work, and repeats that portion of the procedure. Select the codes that you best feel reflects the procedure you perform. The use of allograft is not coded.

I believe the answer to Stephen Pessler's question about whether podiatrists have opened the door for nurses to treat "minor" foot problems is an unequivocal "Yes." There is a need and a public demand for medical care of minor foot problems. If podiatrists in any locale are not willing to provide it, others will. 
 
Much of this relatively recent "door opening" to nurses has to do with economics. With past insurance payments for surgical services being generous, and with little or no out-of-pocket expense for patients motivating them to opt for...
