IL Podiatrist Discusses Onychomycosis
 
“The most common (nail) issue is onychomycosis,” says Patrick McEneaney, DPM of Northern Illinois Foot & Ankle Specialists. “This is typically the same fungus that causes athlete’s foot. It’s very transmittable and very easy to pick up walking around barefoot, where other people have either a nail fungus or an athlete’s foot infection.”

According to Dr. McEneaney, the best types of treatments for this are prescription topical medications, oral medications, and laser therapy. “Laser therapy is a cold laser which actually penetrates through the nail and kills the fungus from within the nail and the fungus that lives within the nail bed itself,” says Dr. McEneaney.

NJ Podiatrist Provides Advice on Plantar Fasciitis
 
"Plantar fasciitis is the inflammation or microscopic tearing of the plantar fascia—the tendon-like connective tissue that joins your heel to the front of your foot," explains podiatrist, Adam Kaplan, DPM, founder of Arcus Orthotics. It's one of the most common foot ailments that podiatrists see (about 10 percent of adults in the United States suffer from it), and it's caused by repetitive, micro-trauma to the foot.

According to Dr. Kaplan, "if you have acute pain, do not walk barefoot, even around your home. Wear sneakers inside at all times unless sleeping or showering," to support your feet. He also advises against wearing shoes with too much flexibility and no arch support. "You can always add custom orthotics to your running or walking shoes to help treat acute, and prevent chronic, plantar fasciitis. A modification can be added to help align the heel, support the arch, and modify the first metatarsal to reduce the strain and stress to the plantar fascia," Dr. Kaplan adds.

IL Podiatrist Lectures on Peptides in Wound Care 
 
Sev Hrywnak DPM, MD presented an original paper titled “The Use of Peptides in Wound Care” at the Association of Peptide Physicians meeting in Las Vegas, December 9-11. “Using peptides that accelerate wound healing will be the standard of care not just in wound care but in many clinical situations,” stated Dr. Hrywnak.

"Our two-year study compared standard wound care techniques for wound care patients with and without the use of injectable peptides, and those patients that received injectable peptides healed at an accelerated rate. Peptides such as BPC-157 along with others have shown to improve patient outcomes," said Hrywnak.

Query: Telehealth Billing
 
We have had an influx of phone calls that are burdening our clinical hours. We have been flooded with questions that pertain to patient care, clarification of orders, and home care instructions. Is there a way to bill for these calls? Each call can take 20-45 minutes for our medical assistants to complete. And we are trying to manage a full clinic at the same time.

Response: These are all interesting questions and challenges facing many healthcare providers. There are a myriad of telehealth codes which are available for reimbursement. Each has its own set of requirements and demands. For example, if the patient was seen within the past seven days or will be seen within 24 hours of the interaction, that interaction may or may not qualify for reimbursement.
 
To attempt to provide you with a general yes or no answer to your question would be almost impossible. There also many venues which can provide (at a cost of course) platforms by which you and your staff can manage these issues. There are also Principal Care Management Codes which you should look into. No doubt this is a daunting task and not one which can be easily resolved.

Dr. FitzPatrick states, ”I wonder how the patient will react to being billed two years after delivery of DME”. I can answer that question. The patient will have NO reaction as you have one year from the date of service to bill a Medicare patient. After one year, you will eat the loss as this is the Medicare rule.
