After an extensive search, the American Podiatric Medical Student Association (APMSA) has announced the appointment of Ms. Heather Keith as its new Executive Director. Ms. Keith brings a wealth of experience to her new role with more than 25 years of association experience. She has extensive knowledge in all aspects of volunteer-governed organizations, including operations management, meeting planning, advocacy, marketing, and information technology.

Ms. Keith has a B.A. in Government and International Politics from George Mason University. She is a graduate of the U.S. Chamber of Commerce Institute of Organization Management (IOM). Sev Hrywnak, DPM, MD served on the faculty of Columbia College this past fall semester as a guest lecturer in the Department of Business and Entrepreneurship teaching economics. "It was a great experience to teach economics to undergrads as they prepare to enter the business world," stated Hrywnak.

"The course covered micro-economics and macro-economics as it pertains to entrepreneurship. Knowing how to interpret the economic data can only help you in any type of business. Courses in practice management and business law should be part of every podiatry college to prepare the podiatry students in the realities of the business world," added Hrywnak.

Researcher Pursues Synthetic ‘Scaffolds’ for Muscle Regeneration
Miqin Zhang, a professor of materials science and engineering at the University of Washington, is looking for ways to help the body heal itself when injury, disease, or surgery cause large–scale damage to one type of tissue in particular: skeletal muscle. Muscles have a limited ability to regenerate, repair, and re-align themselves properly after certain types of damage. Zhang and her team are taking a synthetic approach to muscle regeneration. Their goal is to create a synthetic, porous, biologically compatible “scaffold” that mimics the normal extracellular environment of skeletal muscle — onto which human cells could migrate and grow new replacement fibers.
As she recently showed in a review article published online Nov. 16 in the journal Advanced Materials, this endeavor builds on decades of work into the growth, repair, and behavior of normal skeletal muscle, but also relies on keen knowledge of engineering and materials science.

10 Collection Tactics for High-Deductible Patients - Part 10
Patient collections are much more than collecting $10 co-pays: those days are long gone. Patients are paying a greater percentage of what you get paid and will be paying an ever higher percentage in the years to come. Smart and consistent patient collections tactics must be a part of every practice's financial strategy. Though the rate of healthcare spending may have slowed, the share absorbed by patients has done the opposite. Here are a few basics for improving your patient collections.
10. Have a heart: There are some patients in poor or dire health who simply don't have the money. There will be times when we care for patients with no expectation of getting paid. It's okay.

My first thought on this patient is that this is trauma related as the lysis of the nail plate is present in generally the same area of the hallux. Does the patient wear nylons? Does the patient wear tight shoes? Is there a forefoot deformity which may traumatize the nails?
You stated that the dermatologist used cortisone cream and psoriasis medication without any response. But the only treatment for psoriatic nails is injectable steroid into the matrix - and it appears that this was not tried. So if this is not biomechanical - and the shoes are not too short. I would try steroid injections into the matrix, q 6 weeks.

It appears on the clinical photograph that has been submitted that the distal middle-lateral portions of both hallux nails are in a state of onycholysis (loosened from the nail bed) with the halluces impinging onto the second toes of both feet. The patient appears in the limited photograph to have a bilateral hallux valgus deformity.
The loosened or onycholytic portions of the hallux nails are the result of biomechanical trauma from shoe wear which may accentuate the hallux valgus deformity. The nail biopsies and fungal nail cultures are negative for psoriatic disease or dematophytic infection. It should be noted, however, that these regions of nail that are loosened from the nail bed are susceptible to secondary fungal colonization, and proper foot hygiene should be stressed to the patient.

About 50% of toenail disorders may be attributed to diseases mimicking onychomycosis. That includes the commonly occurring trauma due to the friction of the big toe against the shoe (M. A. J. Allevato, "Diseases Mimicking Onychomycosis", Clinics in Dermatology, 28: 164-177, 2010). Your patient’s hallux nails might be caused by footwear trauma. About a year ago, my two big toenails were injured by a metal bar, resulting in some dark bruises. Both nails stopped growing for about six months until I applied my Dr. Win’s Fungal Nail Therapy solution twice daily to the cuticle areas. Healthy nails quickly grew out in about five months, which was very quick considering my age of 77.

Has anyone else tried to Google the term "Modified Katzen-Wilson osteotomy." If so, are you as disappointed with the findings as I was?
It will be helpful, to the reader, to know the definition of terms applied to a discussion. And when a respondent justifies clinical practices with research evidence, to know those references. Otherwise, this is but a sound-bite.

A recent posting by Dr. Dave Williams and subsequent responses raised the question of pharmacogenetic testing and therapy. At the New York College of Podiatric Medicine, we teach during the first year, a year's course in human genetics. The first semester is Introduction to Human Genetics followed by a second semester course in Molecular Biology/Molecular Genetics. In the first semester, students learn classical Mendelian genetics, with emphasis on genetic diseases caused by single gene defects. A genetic counselor from the Department of Genetics and Genomic Sciences, Icahn School of Medicine of Mount Sinai gives a guest lecture on how to take a pedigree and each student is required to draw their family's pedigree. In the second semester, emphasis is on precision/personalized medicine, including pharmacogenetics/genomics.
The genetics of common complex diseases including Type 2 Diabetes is studied in depth. The Human Genome Project and ways to identify and study genes are explored. Lectures on use of DNA sequencing in clinical medine and pharmacogenetics are given by expert guest lecturers from the Department of Genetics and Genomic Sciences, Icahn School of Mount Sinai. The importance of genetic variants in determining the correct drug/dosage is discussed. The future role of precision/personalized medicine in podiatry is also emphasized. The course director at NYCPM is working with the Council of Faculties to include genetics in the podiatric medical school curriculum, and the working group has already decided on objectives.
We believe that our year-long genetics course prepares our student well for future changes in Podiatric Medicine and introduces them to the concept of precision/personalized medicine including pharmacogentics.

After 30+ years of wound care, you'd think I had heard it all. In the spirit of the holiday season - I had a new patient at our wound care center who presented with a Wagner Grade 3 diabetic foot wound of the left hallux. Duration of about 2-3 weeks. In taking the history of the patient, I asked what type of wound care she was using for the wound. With a big smile on her face, she stated that she applying Myrrh to the wound daily. When I asked which of the three Kings had prescribed that for her, she gave me this unusual look. Anyway, I left the room singing Christmas carols for good luck. God, I love this job.
