Diabetes: Important New Recommendations in ADA 2017 Care Standards
New recommendations for 2017 include:

Assess patients for psychological problems, including diabetes distress, depression, anxiety, and disordered eating.

Determine patients' social context, including potential financial barriers, food insecurity, and housing stability.
-
Prolonged sitting should be interrupted every 30 min with short bouts of physical activity.
-
Clinically significant hypoglycemia now defined as ≤54 mg/dL, with “alert value” for taking action at ≤70 mg/dL.

Query: Pitting and Discolored Hallux Nails
I saw this healthy, active 33 year old female in May regarding bilateral hallux nail discoloration. She is not taking any medications. She believed she damaged the nails with a pair of new shoes. I told her to allow the nails to grow and then return for re-evaluation. During the summer, her family doctor prescribed cortisone cream, psoriasis medication, and did 3 fungal cultures, all negative.

I recently examined her nails and they look the same as previously, without growth distally. There is an area laterally, midline, that is discolored and pitted. X-rays were negative for a subungual exostosis. What I find unusual is that the pitting occurred at the same time and that the areas are in exactly the same location bilaterally. Any suggestions?

Gerald Joseph Gambale, DPM, 65, of Port Richey, FL, died Dec. 16, 2016. He was a USMC veteran and practiced podiatry in Bay Ridge, Brookyn, NY. He is survived by his wife, Ann; eight nieces; a nephew and a great-nephew.

I agree with Dr. Rosenblatt about gastroc equinus, but I take exception that the first metatarsal elevation "without internal fixation, after MIS surgery... is guaranteed." Some MIS doctors believe this and do multiple lessor metatarsal osteotomies because they claim patients will get lesions under the metatarsal heads, especially the second metatarsal, which is usually long before the bunion surgery. I have rarely seen a second metatarsal lesion after a Katzen Wilson, Wilson, or Isham Reverdin bunionectomy.
On rare occasions, I have had to later do a 2nd or 2nd, 3rd or 2nd, 3rd, and 4th metatarsal osteotomies after one of these MIS bunion surgeries. A lot of doctors who do these surgeries agree with me. If I were seeing a lot of 2nd metatarsal head lesions after these bunion surgeries, I would do a second metatarsal osteotomy at the same time, but that has not been my experience. On severe bunions, I have done these bunion procedures twice, and the patient usually accepts this since these procedures can be done on geriatrics in their 80s, because they are ambulatory procedures. I usually do them on patients in their 70s or 60s if they are in good health.

I agree with most of the post Dr. Rosenblatt has eloquently described. It is essential that surgeons address gastrocnemius equinus when the deformity presents. It’s equally vital that we incorporate bio-mechanical exams in our surgical endeavors.
I do take some exception with his conclusion that performing a MIS bunion correction without fixation will always lead to second metatarsal transfer pain. The research in this area contradicts this statement. Non-fixated first metatarsal osteotomies such as the landmark research Dr. Dale Austin gave us is a...

Dr. Len Schwartz is offering poor advice. To ask a patient in the treatment room to give an unrehearsed video testimonial is nothing less than entrapment. Does this type of behavior promote a quality doctor-patient relationship based on trust? Does the patient have time to consider that their privacy as a patient is being compromised?
Having a negative review online is a fact of life in today’s world. Does the author of a negative review have a valid point? Does something need to be changed in your office? The idea of having a bank of positive comments posted online via entrapped patients is fraudulent. Grow a practice via sound and honest practice management, and that especially includes the staff and their attitudes, and the negative reviews won’t be as influential.
