Len Schwartz: Once negative comment has been written, there is almost nothing you can do. What you can do today is start building a huge bank of positive comments in advance. Often, you can ask a patient in the treatment room to take out their phone and capture a 20 second video testimonial on their phone, which you upload to YouTube, Facebook, and Google or a quick review on these sites (from the patient's phone). These video comments will work magic. When someone writes a bad comment about you, it gets lost in the dozens of positive reviews.

UK Podiatrist Retires After 20 Years
After 20 years, Malvern podiatrist Mark Perkins is putting his feet up and retiring. Perkins, whose practice is in Malvern Link, came to podiatry after a successful career in the Royal Engineers. He and Susan Hubbard built up the practice and have provided complete footcare services, including biomechanics, nail and skin care, nail surgery, and many other services.

His practice will continue to run alongside that of Sue Hubbard with Rosie Thomson joining the team in clinic. Thomson, who comes from Evesham, trained at the same school of podiatry, in Birmingham, as the two principals.

Engineers Develop a New Non-invasive Method to Detect Infections in Prostheses
Engineers at the University of California San Diego have developed a new non–invasive method to detect infections in prostheses used for amputees, as well as for knee, hip, and other joint replacements. The method, which is at the proof of concept stage, consists of a simple imaging technique and an innovative material to coat the prostheses.
“Current methods to detect infection require patients to undergo burdensome imaging procedures, such as an MRI, CAT scan, or x–rays,” said Ken Loh, a professor of structural engineering at the Jacobs School of Engineering at UC San Diego and the lead researcher on the project. “By contrast, our method could be easily used in a doctor’s office or in the home and, potentially, provide quantitative diagnostic–relevant information about the extent and locations of the infection.”

CMS Finalizes 2018 Exchange Rules Ahead of Trump Administration
The CMS has made a handful of final changes to the Affordable Care Act's health insurance marketplaces for 2018, just a little more than a month before Donald Trump takes over the White House and congressional Republicans move to repeal the healthcare reform law. A final rule published late Friday cements many of the CMS' proposals from August. Some of the most notable changes involve the ACA's permanent risk-adjustment program, which funnels money from insurers with lower-cost enrollees to companies that have higher-cost enrollees.
Republicans have not coalesced around a plan to replace the ACA, leaving many wondering if it will be years before the law is changed or simply rebranded. Policymakers view changes to the exchanges in 2018 as vital, especially if the hope is to prevent health insurance companies from leaving the marketplaces due to the uncertainty, which would toss millions of people off their health coverage.

10 Collection Tactics for High-Deductible Patients- Part 8
Patient collections are much more than collecting $10 co-pays: those days are long gone. Patients are paying a greater percentage of what you get paid and will be paying an ever higher percentage in the years to come. Smart and consistent patient collections tactics must be a part of every practice's financial strategy. Though the rate of healthcare spending may have slowed, the share absorbed by patients has done the opposite. Here are a few basics for improving your patient collections.
8. Offer payment plans: It's hard for most patients to come up with $1,000 on the spot. Offer patients a chance to pay large balances over several months. We do not charge interest, but you have every right to do so.

RE: Bone Graft at Bunionectomy Site
Is CPT 20957 (bone graft with microvascular anastomosis; metatarsal) an appropriate code to use when an autologous bone graft for the bunionectomy site was used to rotate the cartilage of the head of the first metatarsal for treatment of hallux limitus/valgus? Can we also bill CPT 28296?

Response: Typically, rather than actually performing a microvascular anastomosis, surgeons use an autograph (from the same metatarsal) to address hallux limitus. If that is the case here, in my opinion, it is globally included in CPT 28296.

RE: MIS Metatarsal Osteotomy Surgery vs. Akin Osteotomy on Elderly Patients

There are complications from pressure under the 2nd metatarsal head post-surgery. Any 1st metatarsal osteotomy to correct a bunion deformity, whether it is held by internal fixation or not, is subject to leaving the 2nd metatarsal structurally "elongated." This can easily result in a painful plantar lesion and capsulitis under the 2nd metatarsal head.
This is why it is so important for biomechanical examination to be part of patient evaluation. If the patient has forefoot/gastrocnemius equinus and/or a short Achilles tendon, they will be putting a great deal more pressure on their forefoot during gait cycle. This will exacerbate pain under the 2nd metatarsal head. If the patient has hammertoes, this will...

Without addressing Dr. Peacock's use of the Katzen Modified MIS Wilson procedure for HAV/Bunion correction in general, I'd like to address his pre- and post-surgery x-ray angle interpretation from the x-rays he sent us to review. I can't agree with his measurements of either the pre-op and 6 week post-operative x-rays. I am not trying to be critical, but to be honest here. Here is what I measured from his x-rays sent in.

I commend Dr. Loshigian for his letter and stance with Castle Connolly. It is important for each of us to speak up for the profession where we can. For several years, their representative tried to sell me advertising space in the Top Doc issue in this area. Each year, I wrote a similar letter, essentially telling them I would consider an ad if and when they included podiatry as a classification. Now they have given up soliciting me, and unfortunately still haven't moved to a more enlightened inclusive focus. Keep up the good work Dr. Loshigian.
