High-Heeled Boots Put Women at Risk: Canadian Podiatrist
 
Wearing high-heeled boots puts women at risk for slipping on ice and snow and injuring themselves, a local podiatrist warns. “Wearing high heels makes you more unstable when walking or standing on dry surfaces, let alone slippery ones like ice or snow,” Dr. John Lanthier said. “A low-heeled winter boot is a lot more fashionable than a cast and crutches.”

He recommends women scuff up the soles of new boots or buy adhesive rubber soles for better traction. If, while wearing high-heeled boots, a woman's ankles roll inward or outward, she can break her ankles. If it twists, ligaments can be stretched or torn, causing a sprain. “This time of year I see a variety of broken bones in patients who have slipped on the ice,” the doctor said. He recommends that women who've hurt themselves get to a doctor as quickly as possible. The RICE method – rest, ice, compression and elevation – will help reduce swelling and pain.

NJ Podiatrist Provides Advice on Disposing Worthless Baseball Cards
 
Like the average American, the only thing I like more than following sports is making money. As a sports collector for most of my life, I’ve been able to feed both passions at a local sports card show held every few months. Attending it has been one of my guilty pleasures for 20 years. I recently spent most of the afternoon at the JP Sports Card Promotion Show in the White Plains County Center about 25 miles north of Manhattan buying, selling, and trading; but mostly interviewing some of the most active dealers and collectors for my new blog on sports collecting.

At the show, I heard at least a couple of middle-aged collectors asking dealers for advice on how to dispose of their worthless stash. “My Toys R Us sets sold for $6.99 in 1993,” groused Dr. Larry Price, a podiatrist from Westwood, New Jersey. “Today they sell on eBay for 99 cents. There are thousands of those sets.” The best advice: donate them to a children’s hospital.

Heisman Trophy Winner Wants to be a Podiatrist
 
On Saturday, Freshman FSU quarterback Jameis Winston was named the winner of the 2013 Heisman Award. He's the youngest player to ever win the award, and he won it with the seventh-largest margin of victory, earning 668 first place votes. "Hopefully I'll be another Deion Sanders or Bo Jackson," he told SI.com as a high school senior in 2011. "No [pro] quarterback plays two sports. I want to make a name for myself."

And there's one other thing - Winston, who had a 4.0 grade-point average in high school, said what he wants to do: "I always wanted to be a foot doctor."

Convicted NJ Podiatrist Loses Right to Ever Prescribe Drugs
 
Five doctors convicted of illegally selling prescriptions have lost the right to ever prescribe drugs in New Jersey, even if their medical licenses are someday reinstated. Each has been criminally convicted in federal or state courts. Dr. Robert H. Moss, a Williamstown podiatrist pleaded guilty in federal court to illegal distribution of oxycodone and was sentenced to 60 months in federal prison, authorities said. His medical license was revoked in November 2011.

I place a lot of wound VACs on patients 2-3 days following initial procedure for those wounds left open to drain. What modifier should be used for wound VAC placement (CPT 97605) following a transmetatarsal amputation procedure (CPT 28805) with the wound left open: modifier "-58" or modifier "-78"?

There seems to be an elevated 1st met head on the WB lateral view, which may be causing a new/continued FHL. That could be diagnosed with a foot typing. I suspect a flexible forefoot FFT. Short of surgery, consider a series of therapeutic common peroneal or posterior tibial nerve blocks a la Steinberg, incorporating cyanocobalamine. Add a neurophylic supplement like Neuremedy.
 
Cast, prescribe, and dispense a foot type-specific foot centering orthotic and incorporate peroneus longus compensatory strengthening exercises.

Neuroma or neuritis of the first interspace is highly unlikely following a bunionectomy with osteotomy. The etiology of your pain is most likely related to placing the fibular sesamoid from a deviated position to a more rectus position. 
 
As the fibular sesamoid begins to laterally deviate, the cristae erodes. During the Austin, we transposition the metatarsal head laterally, placing the sesamoid into what is essentially...

Dr. Roth makes a good point by pointing out that there are cases where a person was legitimately injured by a doctor or hospital's negligence and that person deserves to be compensated. At the annual SAM conference held every year in Orlando, Dr. Merritt lectures on avoiding errors that we, as podiatrists, may make. Other medical doctors have written books and there have been documentaries on CNN on the whole gamut of medical errors. The situation is bad and getting worse. 
 
Just today, an internist told me that his multi-doctor practice was just taken over by a corporation and he is now restricted from spending adequate time with any one patient. Other demands the new non-doctor owners are making on him are throwing him into the realm of not only making potential medical errors, but practicing unethically. 
 
With Obamacare coming around the corner and millions of new patients entering the medical patient pool, will doctors who even want to spend adequate time with patients have the time and resources to do so? Potential problems now face us, including increased potential for mistakes and negligence. I wonder if the tort system as we know it is what is needed to protect the lives of patients.

This so called "Cinderella" procedure seems like an irresponsible effort to promote a practice. "Preventative" bunion correction? What is being corrected? Prophylactic shortening or lengthening? Really? Any honest experienced foot surgeon will tell you that the "glass slipper" may not always fit so well after foot surgery!

In my experience as a consultant to help defend DPMs accused of Medicare fraud, by far the most common cause of action is "nail wedge resections without local anesthetic billed as surgery." It appears that the case of the MI podiatrist is also in this category.
 
There may be a chance of defending this case by "converting" some of these (billings) into routine foot care, if the patient has risk factors associated with the need for RFC. For example, I have a statistical reporting and analyzing system that could be used to "convert" these. But, if a doctor actually billed a surgery without a local anesthetic administered by injection, it is very difficult to defend. If it was a matter of only a few of these procedures, however, it can be "defendable."
 
In the worst situations, there were literally hundreds of these billings, with almost no charting to use to defend. I routinely publish articles in Podiatry Management to try to educate DPMs not to take this kind of risk. Yet, we continue to see them. NH practice is usually 30-60% non-compensated. Unless you are willing to accept this, you should not be going into them to practice podiatry, because I assure you, you are being watched. I beg of you, please think of your family and your future.
