IN Podiatrist Discusses Bunion Surgery

"When doing bunion surgery, we usually make a medial incision," says podiatrist Dr. James Hillaire. "It hides the scar, and when the scar contracts, it also helps hold the toe straight. It's better aesthically and functionally as well."

After bunion surgery, Dr. Hillaire recommends that patients look at the overall design of the shoe. "We want a square toe as opposed to a pointy toe. We want the heel to be about two thumbs high, not a a stiletto. You want a shoe with a more stable base," says Hillaire.

TX Podiatrist Re-Elected as Chairman of the Department of Surgery

Dr. Samuel Mendicino

The surgery department of West Houston Medical Center in Houston, Texas has re-elected Samuel S. Mendicino, DPM as its Chairman of the Department of Surgery. He will serve a second, two-year term beginning January 2013. Dr. Mendicino also serves as the hospital's director of its 3-year residency in podiatric medicine and surgery.

AGS Presents Distinguished Service Award to PA Podiatrist

On December 10, 2012, The American Geriatric Society presented Arthur E. Helfand, DPM their Distinguished Service Award at their annual meeting. The award was "in recognition of extraordinary contributions to the mission of the society." Helfand was a Founder, served fourteen years as a member of the Board of Directors of the Eastern Pennsylvania – Delaware Valley Geriatrics Society, and is a Past President.

Dr. Helfand is Professor Emeritus at Temple University, School of Podiatric Medicine, and a member of the Honorary Staffs at Temple University Hospital and Thomas Jefferson University Hospital. Dr. Helfand serves as a Consultant to Temple University’s Institute on Aging and is past Adjunct Professor of Medicine, Department of Medicine, Temple University School of Medicine.

Dr. David G. Armstrong, Professor of Surgery and Director of the University of Arizona's Southern Arizona Limb Salvage Alliance (SALSA) highlighted more than a dozen lecturers and delegates from some 45 nations at the International Diabetic Foot Symposium held this week in Cuba.

In regard to Dr. Kass' comment on injecting a "liquid bone" under ultrasound guidance and then appling a bone stimulator for a sesamoid fracture, the question that arises is coverage for these treatments by insurance. I think the proposed idea is a good conservative approach to try to heal the fracture, but I'm not sure about whether or not this would be covered.

I admit treatment outcomes for sesamoid fractures can be perplexing, and in some cases, less than ideal. Physiologic reasons exist. Previous literature discusses off-loading, PT, injections, and removal for symptomatic non-unions. I once read about an ORIF with small cortical screw placement across a sesamoid fracture that was tried, but never read any long-term follow up on its success, and whether any other similar attempts were tried; so, I naturally assumed the outcome was not very good, and the surgeon abandoned that particular approach.

I have utilized compounded medications increasingly over the past 3-4 years, including those from Total Pain Solutions, Trilogy, and Bellevue Pharmacy - for whom I am a consultant and do clinical research.

Compounding offers many advantages to patient care. In the case of topical pain management, I am able to prescribe anti-inflammatories for the patient in whom the use of an anti-inflammatories might otherwise be either contra-indicated or of concern, such as the patient with CHF, hypertension, renal dysfuction, or GI concerns, since these medications are typically associated with plasma levels 1% or less of the same medications administered orally. I never used “pain creams” in practice. That was until I was introduced to Motion Medicine™ while lecturing in Canada over two years ago. I was so impressed as to how effective this was that I became the U.S. importer.

I too became frustrated early in my career over the lack of a reasonable alternative for my patients with painful neuropathy who did not gain benefit from Neurontin and its sister medications. If you read the labels, most all of these rely mostly on the effects of menthol. The only alternative at the time was to have the local compounding pharmacy make and dispense a 2 oz jar containing a mixture of clonidine, lidocaine, and ketamine; and it cost $70 out-of-pocket and worked about 33% of the time.
