Numbness or swelling of the feet can mean many things according to Mitch Hulbert, DPM, from Columbia St. Mary's, part of Ascension. "It can be a cardiac problem. It could be an injury, fracture, or some type of a tendon rupture. It could be a problem of retention of fluids due to too much salt intake. It's very patient dependent."

The American Association of Colleges of Podiatric Medicine (AACPM) successfully launched a new, interactive DPM Mentors Network. Seeking to be an industry leader in the advancement of pre-health students' understanding of careers in podiatric medicine, the DPM Mentors Network is a broad step forward in assisting the placement of pre-health students with mentors for shadowing opportunities.

MS Podiatrist Opens Practice With Home-Type Feel in Hometown
 
Being a Southaven native, Robert W. Appling, DPM, knew that he wanted to open his podiatry practice in the heart of Mississippi's third-largest city. A toasty fireplace greets patients as they walk through the door. A Christmas tree, all decked out in tinsel and trappings, stands majestically in the corner.

Each treatment room — there are at least four, including an x-ray room — are decked out with a different motif and theme. There is a room dedicated to his cousin Baseball Hall of Famer Luke Appling, formerly of the Chicago White Sox. Another room is dedicated to America's veterans, with all the branches of the military represented. Another room is a "Mississippi" room which celebrates "all things Mississippi." "We're aiming for a home-type feel," Appling said.

Cushioned Shoe Inserts Won't Guard Against Injury: Review
 
The millions who run, walk, or play sports may think shoe inserts that cushion the foot can help prevent injuries. But a new review challenges that notion. Only orthotics, which are actually molded to a person's foot, seem to be able to do that job, Australian researchers reported. "There appears to be little merit in using shock-absorbing insoles for the prevention of injury, while there is some evidence that foot [orthotics] are effective for the prevention of some injuries like stress fractures and shin pain," said review author Daniel Bonanno. He is a lecturer in podiatry in the College of Science, Health, and Engineering at La Trobe University in Melbourne.

In the study, Bonanno and his colleagues analyzed 11 trials that evaluated orthotics and seven trials that evaluated shock-absorbing insoles. The main findings of the review were that foot orthotics seemed to help prevent overall injuries, shin pain, and some stress fractures of the foot and leg. Shock-absorbing insoles, however, did not. Specifically, foot orthotics reduced the risk of overall injury by 28 percent, while they cut the chances of a stress fracture by 41 percent. However, they did not lower the incidence of tendon or muscle injuries, or knee and back pain, the researchers said. Shock-absorbing insoles didn't lessen the risk of any type of injury. And one trial suggested they might even increase the risk of injury, the review found.

VA Finalizes Rule that Expands Scope of Nurse Practice
 
The Veteran Affairs Department has finalized a rule that will allow advanced practice registered nurses to practice to their full authority at VA facilities, however the new permission will not expand to certified registered nurse anesthetists following lobbying from anesthesiologists. The change has long been debated at the VA and in Congress but is opposed by the American Medical Association which immediately slammed the rule after its release Tuesday.
 
“This part of the VA's final rule will rewind the clock to an outdated model of care delivery that is not consistent with the current direction of the healthcare system,” Dr. Andrew Gurman, president of the AMA said in a statement, adding that state law should be followed. About half of states have full scope of practice laws for nurse practitioners. The VA believes the rule will make it easier for veterans to be seen by medical professionals by increasing the number of available primary-care providers. The new policy becomes effective January 14, 2017.

10 Collection Tactics for High-Deductible Patients - Part 5
 
Patient collections are much more than collecting $10 co-pays: those days are long gone. Patients are paying a greater percentage of what you get paid and will be paying an ever higher percentage in the years to come. Smart and consistent patient collections tactics must be a part of every practice's financial strategy. Though the rate of healthcare spending may have slowed, the share absorbed by patients has done the opposite. Here are a few basics for improving your patient collections. 
 
5. Collect a flat amount in advance: Many practices collect a flat amount in advance of procedures when it is unclear how much the patient will owe. After the claim has been adjudicated, either bill the patient the balance or issue a patient refund.

RE: Documentation and Quality of Care

There was an article with a study posted in PM News this year which demonstrated the decline in the actual time that physicians spend with their patients.  I don't remember the exact number, but I think it was less than a 1/3. I recently went to a lecture by a dean from a medical school who said the number of history and physicals one could perform in a day was 30% less.  
 
I guess that is good for jobs, as you'd need 30% more doctors or perhaps PAs. But instead of increasing positions, the quality of the H & P has diminished, increasing the number of medical errors. Hence, the lecture topic "Medical Errors & Omissions" is now required every two years by the State of Florida for license renewal.

The MIS Akin bunionectomy was very popular 40 years ago and had a lot of advantages such as minimal disability and was a relatively safe procedure that could also be performed on geriatrics. Dr Sullivan raises some good points that the correction is not nearly as good as a metatarsal osteotomy.

I would like to thank Dr. Lee for his kind comments. I agree both with Dr. Lee and with Dr. Sullivan. With respect to using the Akin as an isolated procedure for HAV, some limitations do exist academically. I have listed these limitations below. That is not to say that the rules cannot be broken and there is a vast void between academic outcomes and patient satisfaction as we all know. 
 
Here are the academic solo MIS Akin contraindications: 
 
1) IM angle over 15 degrees for procedure in HA cases.
