It is nice to hear from another perspective regarding indications and application of foot surgery techniques. I simply ask Dr. Lee to let us see the volume after 30 years of doing this, clinical pictures, and the x-rays of these 18-20 degree IM angle bunions that he has done using this technique at 5 years post-op. In my opinion, this technique in a person with a high IM angle is a “quick fix” and in my experience, it is doomed to long-term failure.


 


I suppose that if one explained to the patient that the need for future surgery is very likely, it would be more palatable or if your goal was short-term (i.e.-with an older minimally ambulatory patient), I could understand the rational of using it. On a younger patient with a significant structural deformity, I cannot rationalize its use. I will say that I do not have 30 years of experience using this technique and am willing to learn something new from someone with more experience.
