IN Podiatrist Provides Children's Shoe-Buying Advice

Fall represents one of the peak times for children's shoe shopping. Parents need to keep in mind these tips from podiatrist Dr. Richard Stanley and the American Podiatric Medical Association as they peruse the shoe aisle for the young feet in the family. Buy for fit, not size. Don't assume that because your child has grown out of one size that he or she needs the next size in the new pair of shoes, Stanley says. “The biggest mistake I see people make is buying a size instead of fit,” he said, adding that sizing may vary from brand to brand or even within a brand.

Dr. Richard Stanley

Don't buy shoes that are too stiff or too big. “Kids' feet are really flexible,” Stanley said, “and you don't want to have a shoe that's going to mold the foot.” Nor do you want a shoe that's too big. Loose shoes may slide up and down, causing blisters. If you can put your finger down the back of the heel, the shoe is too big, he said.

INTERNATIONAL PODIATRISTS IN THE NEWS

Many Diseases Can be Diagnosed Through Feet: St. Maarten Podiatrist

Podiatrist Zenia Duncan has found a wonderful spot in Philipsburg for her practice. Zenia studied podiatry at Instituut de Korte in the Netherlands and graduated in 2010. Healthy feet are responsible for a stable base that can prevent many body problems. In traditional Chinese medicine, reflexology shows the relationship between the foot and the body; that good care of the foot is really important to our well-being and health.

Zenia Duncan

Our feet are closely related to the internal organs and can reflect pathological changes in the body. The feet can therefore be used to diagnose many diseases. At the same time, many types of stimulations applied to specific reflecting areas or points on the foot can improve and preserve health, and prevent and treat diseases.

VA Podiatrist Re-enacts Civil War Medical Techniques

If modern-day surgeons could visit a Civil War field hospital, they'd recognize much of what they saw. They'd also be appalled. To demonstrate, Dr. John Ocheltree took an ungloved hand and probed with his finger to find the bullet that had injured his "patient." After "amputating" the man's leg on Saturday, Ocheltree wiped his bloody knife on his sleeve. Civil War surgeons were amazingly successful, Ocheltree said, but they also were burdened by what they didn't know, such as the germ theory of disease.

Dr. John Ocheltree (L) tends to "wounded" Civil War Soldier

With the "battle" raging nearby, Ocheltree was on duty at the 5th Virginia's field hospital, a tent set up on the lawn at Brompton. A Staunton podiatrist in real life, Ocheltree is also a student of Civil War medicine and was among the costumed participants in last weekend's events for the Battle of Fredericksburg. His great-great-grandfather, James Logan Dunlap of the 5th Virginia Infantry, was wounded in the battle 150 years ago. "I had to be here," he said.

PODIATRISTS AND THE LAW

Medical Board Suspends License of CA Podiatrist Accused of Sexual Battery

The medical license of a Torrance podiatrist accused of three counts of sexual battery against female patients was suspended Wednesday in Torrance Superior Court. The action came as Arnold Serkin, 67, pleaded not guilty to three charges of sexual battery by fraud that could send him to prison for six years.

The Department of Consumer Affairs Board of Podiatric Medicine had requested the court order on Serkin's license until the criminal case against him is resolved, spokesman Russ Heimeric said. A team of lawyers representing Serkin called the order "premature." Not until a preliminary hearing will it be known whether the evidence against him warrants a trial, defense attorneys say.

QUERIES (NON-CLINCAL)

Query: Source for Podiatric Animations?

Does anyone know of a good source for podiatric animations for both surgical procedures and common conditions? I would like to use these as part of my pre-op protocol and also for my marketing rep to use when doing lunch and learns with referrals sources.

RESPONSES / COMMENTS (CLINICAL)

RE: Tibial Sesamoid Fracture (Terry Nayfa, DPM)
From: Jeffrey Kass, DPM

I would like to throw out to the readers a possible treatment plan for the tibial sesamoid fracture. This is not something that I have tried, but it is something I considered a few months back on a sesamoid fracture that presented in my office. I am presenting it with the hopes of constructive critique in both the positive or negative.

What if one injected one of the "liquid bone" products on the market under ultrasound guidance after needling the region between the bony fragments. The intent would be to bridge the fragmented parts together. This would be followed by bone stimulation and off-weight-bearing. If it didn't work - at least you tried. If it did work, well you saved the patient from being cut open, which in an athlete could have a deleterious effect.

RE: Efficacy of "Pain" Creams (Tip Sullivan, DPM)
From: Elliot Udell, DPM

Dr. Sullivan is to be acknowledged for opening  up a community inquiry as to what compounded creams work for treating various types of foot and ankle pain. These compounds are very expensive and the patients have to pay the compounding pharmacies out-of-pocket, and the formulas for these compounded creams are vast.

At this point in time, we are dispensing various topical menthol-based creams which have other added ingredients. The creams that we have had a margin of success with are Cryoderm, Orthonesic Gel, and Relief cream. There are others. I would appreciate hearing any success stories specifically from colleagues who have used compounded creams.

RE: Efficacy of "Pain" Creams (Tip Sullivan, DPM)
From: Larry Kosova, DPM

For the last several years, my sport medicine practice has been using TPS from Total Pain Solutions. My rep's name is David Jones and his phone number is 847-337-7008. I have used this personally for about 6 months on myself, my wife, and both active children to relieve us from various problems including pain, before using it on patients. I have been amazed with the relief that I have experienced.

I am able to keep athletes of all calibers active with greatly reduced pain. The compound pharmacist uses very strong active medications, but in a cream form which has to be applied 3-4 times a day in small amounts. Some of the ingredients include diclofenac, baclofen, gabapentin, etc.

What I like about this company, is that if something is not working, you and/or the patient can call the pharmacist, and after getting more information from the patient, he will remake a "custom" formula that should work better. The customer service of this company has been some of the best I have experienced in 25 years of practice.

Disclaimer: I have no affiliation with this company.

RESPONSES / COMMENTS - (NON-CLINICAL) - PART 2

RE: Non-Participation and Healthcare Fraud (Larry Kobak, DPM)
From: Robert Kornfeld, DPM

I have no doubt at all that everything Dr. Kobak states is true and accurate. The insanity of it boggles the mind. It is no different than owning an electronics store, selling a Sharp flat screen TV, collecting the $4,000, and sending it home with the customer with all its benefits. Then, along comes a third-party and says to your store, you should have sold this person a Panasonic instead of a Sharp. You must return his money in full. He gets to keep the TV. You may not bill him.

What were we thinking? Does anyone else see the absurdity of the law? Am I the only one? How on Earth does an insurance company have the right to demand money back from a practitioner who contracted privately with a patient for mutually agreed upon services and fees? We have not only given them dictatorial power over us, but also given them immunity from crimes they commit, which are far, far worse than anything we could ever imagine.

Why are doctors continuing to clamber for participation and working so hard to adhere to strict compliance regulations, when all they want to do is disempower your Constitutional right to make a living? I would rather play soccer on the freeway. It's safer.

RESPONSES / COMMENTS (PRACTICE MANAGEMENT TIP OF THE DAY)

RE: Deliver Holiday Gifts to Referrers in January (Chuck Ross, DPM)
From: Lynn Homisak

While I loved Chuck Ross's suggestion of extending "thanks" to the office staff instead of the doctors, another alternative is to send a meaningful donation to a charitable organization in the physician's (or practice's) name, One of my clients did this for me last year (for autism)...and knowing that the "gift" he sent in my name positively impacted others touched my heart in ways that no gift basket could. Try it. Whether you give or receive, this is one gift that leaves you "feeling good" long after the fruit or cookies are gone.
