PODIATRISTS IN THE NEWS
NY Podiatrist Discusses Hallux Joint Pain

When it comes to pain in the big toe joint, there are many culprits. Some are minor, and you can use self-help steps such as rest, ice, compression, and elevation (RICE) and over-the-counter anti-inflammatories to ease the pain. But when the pain lasts for more than a day, you may need a medical evaluation to determine the root cause and specific treatment, we hear from board-certified foot surgeon Johanna S. Youner, DPM.

Dr. Johanna Youner

Dr. Youner cites six reasons for pain in the big toe joint: 1. Wearing the wrong shoes 2. Arthritis (hallux rigidus) 3. Gout 4. Injury 5. Bunions 6. Sesamoiditis. Foot pain is not normal. An examination by a podiatrist can provide a diagnosis and appropriate treatment plan.

PODIATRISTS AND SPORTS MEDICINE
NY Podiatrists Participate in Winter Special Olympics

On December 7, 2024, the Winter Special Olympics was held at the Javitz Center in New York. New York College of Podiatric Medicine (NYCPM) alums Craig Herman, DPM and Samantha Landau, DPM were in attendance as leaders part of the Special Olympics Healthy Athletes. Free health screenings and education to Special Olympics athletes in a welcoming, fun environment were provided.

Dr. Landau is a faculty member in NYCPM's Orthopedics and Pediatrics Department. Many students from the third-year orthopedics rotation were present. As volunteers, the students assisted in performing physicals, biomechanical exams, gait exams, and shoe evaluations for the athletes.

PODIATRISTS IN THE COMMUNITY - PART 1
IN Podiatrist Named 2024 Good Neighbor for Valparaiso

The City of Valparaiso has named Paul Sommer, DPM as its Good Neighbor recipient for 2024, recognizing his exemplary commitment to the community. “Paul Sommer sees a need and works to fill it, benefiting so many, including the local running community, families of kids with disabilities and those with ALS, among others. He leverages his love for endurance sports to raise funds for causes he cares about, and we’re so pleased to highlight his good work,” said Mayor Jon Costas.

(L-R) Jon Costas and Dr. Paul Sommer

Dr. Sommer is a Valparaiso podiatrist who has lived in Valparaiso for more than 50 years. He has translated his talent for endurance sports into ways to raise funds for philanthropic causes. In 2011, he carried Kyle Polhemus through the Valpo triathlon to raise money for those with Duchenne’s Muscular Dystrophy. Later, he completed a long distance open water swim to raise funds for Kyle’s needs and to help begin the Kyle’s Legacy Foundation in Kyle’s name.

PODIATRISTS IN THE COMMUNITY - PART 2
FL Podiatrist Launches Walk with a Doc Program

DeLand Foot and Leg Center is encouraging Volusia and Lake County residents to take a step toward better health with Walk with a Doc, a health program that brings doctors and patients together to walk every 3rd Monday in historic Downtown DeLand, FL. Walk with a Doc is an international non-profit organization whose mission is to inspire communities through movement and conversation.

Dr. Jenneffer Pulapaka

"This program has had tremendous participation and success in hundreds of cities around the world," said Jenneffer Pulapaka, DPM, podiatric lifestyle physician. "There's no question that increasing exercise, even moderately, reduces the risks of many diseases, including peripheral vascular disease, coronary heart disease, breast and colon cancer, and Type 2 diabetes," said Dr. Pulapaka. "Research has even shown that you could gain two hours of life for each hour that you exercise regularly."

PRACTICE MANAGEMENT TIP OF THE DAY
Beware of These Malpractice Landmines

EHRs are increasingly leading to malpractice lawsuits. While EHR-related lawsuits still make up only a small number of lawsuits, user error is the cause of 64 percent of EHR-related malpractice claims, according to the Doctors Company. Practices need to focus on eliminating several common problems, mostly tied to user actions, which have led to the majority of EHR-related suits.

3. Use of Templates

Jeffrey Kagan, MD, a Newington, CT-based internist, has experience reviewing malpractice cases for attorneys. One trend he has seen related to EHR-induced malpractice suits comes from template use. "Templates are supposed to help us do something more comprehensive, but often our templates have a lot of old information that carries forward,” he says.

In order to avoid falling into this trap, Dr. Kagan advises physicians to proofread and modify templates if they are going to be used.

QUERIES (PODIATRISTS AND THE LAW)
From: Jon Purdy, DPM

What is the liability of the compounding pharmacies in these schemes? It seems to me that if false representation was given as a legal endeavor, there would be some liability.

RESPONSES/COMMENTS (CLINICAL)
From: David T Weiss, DPM,  R. Alex Dellinger, DPM

Regarding the above question, it is common for a nail to grow poorly due to the underlying bone. Remember, the nail grows right on top of the distal phalanx. If there is an underlying bony or cartilage defect, the growth of the nail will continue to be pathological. Once the defect is surgically removed, the nail matrix typically creates more cosmetically pleasing nail tissue.

It takes SEVERAL nail cycles for this to happen, and the patient needs to be educated that it will take a while before they notice improvement. In addition, I will have my patients take collagen powder and Biotin. I published a recent paper on this condition and the treatment options involved in managing the growth of the nail.

RESPONSES/COMMENTS (MEDICARE)
From: Ron Freireich, DPM

WOW! a 1.8% increase

1.8%-2.6% (current inflation rate) = We still lose

And please don't say it's better than a 2.8% pay cut. Either way, physicians are STILL on the losing end. No other profession would put up with this year after year.

RESPONSES/COMMENTS (GUEST EDITORIALS)
From: Steven Spinner, DPM

Although I sometimes get cross-eyed reading posts from Dr. Tomzcak and my good friend Dr. Jacobs, give me a moment to dumb things down for the rest of us. In my simple mind, the question actually is simple. Should we confer board certification for all podiatrists equally regardless of their skills or training? Or should we have some type of recognition for those of us who demonstrate a significant achievement in any one area? You pick it…. surgery, wound care, limb salvage, medicine, etc, etc. Think about your child, wife, Mother…wouldn’t you want to have some kind of assurance that the DPM who was going to take a knife to them had to jump through some hoops to get that surgical credential?

And I know that our certification process is not perfect…but it at least sets some type of threshold. And why is ABPM so hellbent on one board? Why aren’t they as proud and protective of their credential as is ABFAS? Like Sisyphus, I have been pushing this boulder uphill for many years. My arms are about to give out. When I turn around, I hope to see some of my younger colleagues ready to take over. I don’t think this is that complicated.

RESPONSES/COMMENTS (OBITUARIES)

I’m sure I’m in touch with sadness the way the rest of our profession is with the passing of Robert Baron. I had the opportunity to serve on the board of directors of the American College of Podiatric Medicine and Orthopedics (now called the American College of Podiatric Medicine) with Bob for close to 10 years. He followed my presidency, and took the reins of this organization from 2001 to 2002.

Throughout this time, he was a staunch fighter for equity in education training among all graduates. I’m sure those who knew him as a teacher can comment on his proudness in his sub-specialty of radiology; but I can truly say as a political leader, he was a quiet giant behind the scenes and everyone who had the opportunity to work with him was a better leader as a result. While we’ve lost touch over the last many years, I do always consider our time together blessed and know that he made a difference in our profession. Rest in peace, Bob.
