UT Podiatrist Discusses Plantar Fasciitis
The plantar fascia can become aggravated by repetitive activity. “If you increase the number of times the heel hits the ground, that can cause plantar fasciitis,” explained St. George podiatrist Dr. Jeff Stewart. “We also see a number of people develop problems when they move to Southern Utah and are unaccustomed to our hard tile or wood floors.”

“If [conservative] methods fail, we generally try one of two things,” said Stewart. “A cortisone injection can help reduce swelling. Often a single injection will do the trick, but occasionally a second injection may be needed. Alternatively, we can try extracorporeal pulse activation therapy, or EPAT. This method uses sound waves to penetrate to the plantar fascia and stimulate the body’s healing response. We typically do one treatment a week for three weeks, with complete healing taking between nine to 12 weeks.”

High School Pals Wind Up Co-Owning Podiatry Practice
Even though his mother has been retired from his podiatry office for a month now, Dr. Patrick Nelson still gets to work with someone he considers family: his partner, Dr. Robert Greenhagen. The two 34-year-old foot doctors — they were born two days apart — have known each other since they wrestled against each other in grade school. They became friends when they attended Gross High School. And they both discovered they wanted to practice podiatry by spending time where Nelson’s mother, Carol, worked as a nurse, at the Foot & Ankle Center of Nebraska. Now, they are co-owners of the practice.

Nelson recalls running around the office with his two brothers while their mother worked. As he grew, Nelson returned a few times as a patient. In high school, Nelson told his parents that he wanted to become a physician. Greenhagen also had been considering medicine as a career, so at Patrick Nelson’s urging, he also visited the office during a school break and followed the doctors around. “It was awesome,” Greenhagen said. “I loved it. It was so much fun.” Nelson and Greenhagen earned undergraduate degrees from different universities but attended Des Moines University’s College of Podiatric Medicine together. “With both of us going into podiatry, we hoped that one day we would eventually be able to work together,” Nelson said. Greenhagen bought the practice from Dr. Robert Hilkemann late last year, when Hilkemann retired and ran, successfully, for the Nebraska Legislature. Nelson now owns part of the practice.

The practice is collecting new and nearly new shoes for children, men, and women through Jan. 9, 2015. Athletic, casual, and dress shoes may be donated; boots for children are especially needed. New pairs of socks also are being collected. Shoes will be distributed through the Salvation Army to those in need.

Query: Night Time Bilateral Met Head Pain in 3 Year Old
For the past three months, this 3-year old white, healthy female has been waking up 1-2 times a week from a deep sleep complaining of pain in the area of the distal 2-3 met heads bilaterally. She has no complaints during the daytime.

There are no edema or clinical signs. Both feet always hurt at the same time. Her sed rate is normal. Does anyone have any suggestions for a further work-up?

Regarding all this hoopla involving co-pays, insurance billing codes, etc.: Let us not forget that we as American podiatrists have an option to sign or not sign a contract with an insurance company or participate or not participate in government-funded insurance. What other professions do as far as billing should not be our concern. Try looking at ourselves and our own practices objectively. This debate is happening in this forum because we (along with MDs and DOs) allowed ourselves to be put in this situation, and we are too fearful and greedy to do what it would take to turn it around.

Let me start by stating that I have no financial interest in any digital x-ray company. When I lecture on technology issues, I always do it in a non-commercial manner because I have no financial interest in any company. I always remain objective and my number one concern is always for the podiatrists.
That being said, over the years, I have visited numerous podiatric offices and have firsthand knowledge of many of the digital x-ray systems on the market. I have to say the system being sold by FOZ Networks is one of the best on the market. The unit uses a CCD camera and forwards the image to the computer in about 6 seconds. The FOZ unit is also the only one on the market that offers free lifetime support at no charge. All of the other DR systems charge monthly maintenance fees, and most often those fees only cover the software. The support from FOZ is second to none; they are quick and responsive. They also work very closely with the manufacturer; so repairs, remediations and upgrades are handled quickly. The manufacturer is also very responsive to creating interfaces for all major EMR programs, so your x-rays can be put directly into patient files.
My recommendation to all podiatrists, when shopping for a digital x-ray unit, is to look beyond the bottom line price. Look for quality, and look at a 5-years' cost of ownership. Then you will find the real deal.

I have worked with quite a few different distributors and installers associated with A2D2 (along with many other digital x-ray devices) over the past few years, and I would highly recommend FOZ Networks for their pricing, professionalism, and customer service.
FOZ Networks has worked with half a dozen of our podiatrist super-group members within the past 4 months, all of whom have given glowing remarks regarding both the system and their services. It wasn’t hard. openclinical.org/os_OpenVistA.html. OpenVistA is the Open Source version of VistA (Veterans Health Information Systems and Technology Architecture), the healthcare information system developed by and for the U.S. Department of Veterans Affairs.
