Diabetics Need Periodic Exams to Assess Risks: IN Podiatrist
The majority of diabetics regularly check their blood sugar to make sure it stays at an acceptable level. However, they also need to check their feet just as often. Diabetes can affect the body in many ways. The longer diabetes is in place, the higher blood sugars can complicate blood flow and sensation, according to Baptist Health Floyd podiatrist Joseph Spires, DPM. He said it is “very important” for diabetics to check their feet on a regular basis to look for sores, open wounds, or an injury that can lead to more serious issues.

“Checking feet on a routine basis is a very important aspect, to see if there is a problem,” he said. “If diabetic patients were recently diagnosed, they need to come in and get a once over, and a better understanding of what that risk is for them as an individual and to make sure everything is on point long-term is pretty important. That may mean they need to come in every two weeks to once a year. It’s all about that risk.”

VA Podiatry Practice Partners With House of Mercy to Collect Shoes for Homeless
To help people experiencing homelessness be more prepared for the cold this winter, Williamsburg Podiatry (Michael Dente, DPM) is hosting its first Winter Shoe Drive for Williamsburg House of Mercy. The local podiatry practice is collecting gently used men’s and women’s winter boots, work boots, tennis shoes and new socks, as well as monetary donations.

2021 Year-end Financial Planning Checklist
The end of the year is a good time to check in on your financial plan to see if everything is on track or if some things need adjusting. You will be in a better position for 2022 the sooner these items are addressed and adjusted if they are off-track.
2. Max Out Retirement Accounts
The contribution limit for 401(k)s and 403(b)s in 2021 is $19,500. The catch-up contribution amount for those over age 50 is an additional $6,500. The deadline for making 401(k) and 403(b) contributions is 12/31/2021. The contribution limit for individual retirement accounts (IRAs) is $6,000 and an additional $1,000 for those over age 50. IRA contributions can be made up to the April 2022 tax filing deadline.

If I have a patient that would like a service that is not covered by their Medicare Advantage Plan, do I need to obtain an ABN? Are ABNs relevant for Medicare Advantage Plans?

Response: For Medicare Advantage Plans (MAP), unfortunately you can NOT use ABNs. Only traditional Medicare can you execute the ABN for fee-for-service Medicare. For MAPs, you must send in a request to the plan asking for them to provide in writing whether they do or do not cover the service you are planning to provide. They are required to provide to the beneficiary and you in writing a response regarding whether the service you perceive as not covered, is in fact, a non-covered service. Once received, then you can collect from the patient for those services in the future.

RE: Diabetic Peripheral Neuropathy Linked to Cognitive Decline in Type 2 Diabetes

Diabetic peripheral neuropathy is associated with a decline in cognition for people with type 2 diabetes, according to a study by JL Barzilay from the Division of Endocrinology at Emory University School of Medicine published in the Journal of Diabetes and its Complications. In his findings from the Glycemia Reduction Approaches in Diabetes Study (GRADE), diabetic peripheral neuropathy was associated with worse results in cognitive tests, but no association was found with cardiovascular autonomic neuropathy and cognition. Barzilay indicates that “If indeed subclinical peripheral neuropathy arises together with subclinical cognitive dysfunction in people with diabetes, it suggests that people with peripheral neuropathy may be at risk later on for more serious clinical cognitive dysfunction.”
In fully adjusted models, highly sensitive C-reactive protein, HbA1c, systolic blood pressure, and alcohol intake were not associated with diabetic peripheral neuropathy. However, LDL cholesterol was associated with definite diabetic peripheral neuropathy, but not continuous diabetic peripheral neuropathy. Waist circumference and urine albumin creatinine ratio were the strongest covariates in the association between diabetic peripheral neuropathy and cognitive impairment. “Recently there was a scientific study demonstrating that peripheral nerve stimulation leads to MRI changes in the parts of the brain associated with cognition,” Barzilay said. “Such scientific studies can help explain how and why such associations exist.

Congratulations to both David Armstrong, DPM and Larry Lavery, DPM. Dr. Armstrong was listed as the top-rated expert in diabetic foot in the world during the years 2011-2021. However, also outstanding is that this Medscape article acknowledges Larry Lavery, DPM as the number three rated expert in diabetic foot in the entire USA during the years 2011-2021.
The worldwide analysis is "based on 6,449 eligible articles published since 2011." Medscape is a biographical database that uses primarily MEDLINE published articles. It is produced by the National Center for Biotechnology Information (NCBI). This achievement is remarkable for both of these doctors and at the same time is the very best way the podiatric community, on a worldwide scale, can be recognized for our expertise and knowledge in the treatment of the diabetic foot. Their accomplishments cannot be understated.
Publishing scientific articles in podiatric publications is indeed important. But also equally as important is to publish in other sources for other fields of medicine to broaden our reach to inform other providers of the skills and knowledge offered by the podiatric physician.
