NY Podiatrist Diagnoses Acral Melanoma in Runner
George Mannes, a marathon runner, lost a nail on his left big toe; when it grew back, the nail bed was slightly discolored. At his yearly skin check, George’s dermatologist took note of the discoloration, but after George explained its origin, she did not investigate further. A few years later, however, George noticed a blood blister forming on his toe. He was referred by his primary caregiver’s office to a podiatrist. After a second referral, George found himself in the office of Bryan Markinson, DPM at Mount Sinai. Dr. Markinson was immediately concerned and did a biopsy. Within about a week, George received his official diagnosis: acral melanoma.

Greater awareness of this more uncommon type of melanoma among doctors and the general public could lead to earlier diagnoses and better outcomes. George is particularly impressed by Dr. Markinson’s "More Eyes on Melanoma" campaign, which is intended to raise awareness among a group of people who are especially well-positioned to notice the early signs of acral melanoma: nail techs working at nail salons.

PA Podiatrist Discusses Conservative Treatment of Flat Feet
If you wake up with tight, painful ankles or feet, that's a sign that your arches are falling — and you may have flat feet. Maryann P. Hartzell, DPM, medical director at the Temple University School of Podiatric Medicine, tells TODAY.com. In the earlier stages, even if you have pain, swelling and tightness due to fallen arches, "there are things that you can do to limit the progression,” Dr. Hartzell says. Treating flat feet typically begins with conservative measures, the experts say. The goal is not necessarily to rebuild your arch, but rather to limit the progression of the falling [arches], ease your symptoms and strengthen the surrounding muscles. However, when it comes to wearing over-the-counter orthotic inserts, Dr. Hartzell cautions that "you have to have the right ones."

Many of the options you'll find are actually too soft to be helpful for those with fallen arches, she says. "If you can flatten that orthotic down with your finger, it's not doing anything for you." Instead, look for a pair that's firm but with a layer of softer material on top to make them comfortable to wear for longer periods of time, she advises. And if you don't have luck with over-the-counter options, a podiatrist can make custom orthotic inserts for you. However, those tend to be expensive, Hartzell says, often in the $400 to $600 range, while over-the-counter options are typically available for around $40.

Conservative Therapies Recommended for Plantar Fasciitis: NY Podiatrist
Plantar fasciitis occurs in about 10 percent of the population—but likely impacts 22 percent of runners, according to data from the National Institutes of Health. It’s also more common in women than in men. The condition occurs when the plantar fascia, a.k.a. your foot arch, gets inflamed from the accumulation of stress over time, says podiatrist Bradley Schaeffer, DPM, a foot surgeon and owner of Central Park Sole in New York.

According to Dr. Schaeffer, treating plantar fasciitis requires a combination of therapy and treatments—think stretching, foot massage, and (occasionally) surgery, depending on the individual's foot needs. “Usually you’re going to need to aggressively stretch or do physical therapy in order to loosen up the plantar fascia on the bottom of the foot,” says Schaeffer. But Schaeffer says the best running shoes for plantar fasciitis could help alleviate and prevent arch and foot pain.

CA Podiatrist Attends NCAA Cross Country Championships
Amol Saxena, DPM, MPH traveled to Wisconsin with his first Achilles tendon surgery patient (from 35 years ago) and friend Joe Fabris. They became friends since the surgery and have traveled to many national championships. They met Dr. Saxena’s patient this year’s Olympic silver medalist in the Steeplechase, a Brigham Young University (BYU) alum, which won both the Men’s & Women’s NCAA championships.

Dr. Saxena had over a dozen athletes from many colleges competing, including a current BYU athlete he operated on 6 months ago, along with Women’s head coach Dilgeet Taylor. BYU Director Ed Eyestone has used Dr. Saxena as BYU's podiatric consultant for both teams since 2017.

ACPME Virtual Seminar Draws Over 375 Attendees
The Academy of Continuing Podiatric Medical Education (ACPME) held a two-day virtual seminar last weekend. The seminar which drew a near record attendance of over 375 podiatrists was organized by ACPME's scientific director Robert Marcus, DPM and Director of Continuing Medical Education David George, DPM.

5 Tips for Preparing for New Opportunities
We’ve seen a lot of positive movement to address some key issues leading to healthcare attrition and burnout. For example, there is stronger encouragement for work-life balance, more staff, mental health resources, and overall better support systems for physicians. However, there is still work to do and some considerations at the onset of a career move. I encourage physicians to look at these five areas before they consider a new role:
6. Rely on mentors
Embarking on a new role shouldn’t be a solo experience. Having a trusted network of mentors can provide invaluable guidance and support. There are so many scenarios and challenges you’ll be presented with in a leadership position and chances are, there are retired or semi-retired experts who have already been there. There is a lot to learn from their successes and failures. By tapping into a network of leaders who are still passionate about improving healthcare, and nurturing upcoming leaders, you can gain invaluable insight and guidance. Mentorship can also provide a sense of community and belonging, which is crucial to avoiding burnout.

All things considered, I think the readers of letters to PM News have a lot to be grateful for this Thanksgiving. Unfortunately, for those that consider gratitude an emotion, it can also be thought of as the shortest lived of all the emotions. How quickly we turn on the profession that has allowed us to raise a family, sheltered them in a nice dwelling, purchased cars, and sent our kids to college when we feel wronged. Just as quickly we condemn those insurance companies, hate the malpractice tariffs and the attorneys who are responsible for what we believe to be outrageous fees for protection against spurious accusations.
We also have the luxury of an almost real-time sounding board. If something in the realm of podiatry angers or upsets us, we have an immediately available venue through which we can, hopefully with tempered emotions, relieve the frustration and lessen its effects so that we aren’t as angry when we get home or take the resentment out on a week’s worth of patients. I’m talking about sending a letter to Barry Block, DPM, JD and PM News. Let me be upfront about...
