PODIATRISTS IN THE NEWS |
CA Podiatrist Discusses Onychomycosis
According to Tracey C. Vlahovic, DPM, a professor at the Samuel Merritt University College of Podiatric Medicine, Oakland, California, most cases of onychomycosis are caused by the dermatophytes Trichophyton rubrum and Trichophyton mentagrophytes, although the cause can also be a mixed infection. “Dermatophytes are going to impact the nails first, and molds may come in and join the party later,” she said at the Society of Dermatology Physician Associates (SDPA) 22nd Annual Fall Dermatology Conference.
|
Dr. Tracey Vlahovic |
“The distal subungual onychomycosis (DSO) type is still the most common, but don’t forget that onychomycosis and nail psoriasis can happen at the same time. What we can’t lose sight of is that onychomycosis is a disease of the nail bed, which ultimately affects the nail plate; it’s not a disease of the nail plate first.” Her diagnostic approach combines periodic acid-Schiff (PAS) staining with fungal culture “because I like to know the speciation,” she said. “PAS doesn’t give me the speciation; fungal cultures should. PCR can be expensive, but that can give me speciation.”

PODIATRISTS AND FOOTWEAR |
NC Podiatrist Suggests Alternatives to Walking Barefoot at Home
Whether it’s wearing a pair of flip-flops at a music fest or slipping on cute but unsupportive flats while exploring a new city on vacation, we’ve all made questionable footwear choices in the name of fashion. But as remote and hybrid jobs have us spending more time than ever at home, how much does what we put on our feet matter when no one’s looking? It turns out, quite a lot, especially if you’re prone to foot problems, podiatrist Elizabeth Daughtry, DPM tells SELF. It has always been an issue in general, but “COVID-19 really brought it to the forefront,” since people who were used to walking around a whole lot more (yep, in shoes) were now stuck at home sitting more, she says.
|
Dr. Elizabeth Daughtry |
When you’re off your feet for hours (say, at your desk or overnight), your plantar fascia tightens up, Dr. Daughtry says. If you go straight from your chair or your bed to stepping down without support (say, if you’re barefoot), you risk overstressing and irritating the taut tissue. For those who aren’t currently in pain but want to prevent foot issues around the house, Dr. Daughtry suggests a slide, slipper, or regular shoe that has substantial arch support but is also flexible enough to absorb impact when you step down.

CA Podiatrist Keynotes at IAWC Masterclass
This past weekend, Asti, Italy welcomed Professor David G. Armstrong, a global leader in diabetic foot research and limb preservation, as the keynote speaker at the Italian Academy of Wound Care (IAWC) Masterclass. Addressing an international audience of healthcare professionals, Dr. Armstrong presented groundbreaking insights into surgical reconstructive techniques aimed at healing chronic wounds and reducing the risk of diabetes-related amputations.
|
Prof. Claudio Ligresti, Chair of the IAWC Symposium, expressed his admiration for Dr. Armstrong’s contributions. |
Dr. Armstrong, Professor of Surgery and Neurological Surgery at the University of Southern California, emphasized the critical role of advanced interdisciplinary approaches in mitigating the devastating impact of diabetic foot complications. His lecture highlighted innovations in reconstructive surgery, tissue engineering, and personalized care strategies that have been pivotal in advancing the standard of care for patients with chronic wounds and limb-threatening ischemia.

PRACTICE MANAGEMENT TIP OF THE DAY |
5 Tips for Preparing for New Opportunities
We’ve seen a lot of positive movement to address some key issues leading to healthcare attrition and burnout. For example, there is stronger encouragement for work-life balance, more staff, mental health resources, and overall better support systems for physicians. However, there is still work to do and some considerations at the onset of a career move. I encourage physicians to look at these five areas before they consider a new role:
4. Don't be afraid to voice your concerns.
Transparency is key when considering a new role. If you have personal circumstances that could affect your job performance or have questions about the future of the practice, it is essential to address these concerns early on. Voicing potential issues during the recruitment process ensures that you and the practice are on the same page and that your needs are considered. Your recruiter should be a resource in this process, ensuring all your questions are answered and helping to address any potential issues before they become problematic.

RESPONSES/COMMENT (HEALTHCARE LEGISLATION) |
From: Neil H Hecht, DPM
All 50 state medical societies – and DC— as well as 77 national medical societies signed the letter to block the Medicare cuts scheduled for 2025. So my question immediately was which 57 or 58 US Senators DIDN’T sign the letter? Those names should be published and we need to lobby them very hard! Private practice is obviously disappearing rapidly and shows NO signs of recovery.
Neil H Hecht, DPM, Tarzana, CA
Editor's note: The entire letter along with the 41 signers can be read here. If your senator's name is not on the list, please contact them ASAP.
