AENS Presents 2021 Tinels Award to AZ Podiatrist
The Association of Extremity Nerve Surgeons (AENS) held their 2021 Annual Symposium in person at the Golden Hotel, Golden, CO November 4-7, 2021. This specialty medical educational meeting provided surgical labs, workshops, lectures, and research updates for peripheral nerve treatment focusing on the lower extremity. The Tinels Award is presented to a Fellow member who has shown dedication and commitment to nerve treatment during a professional history. The 2021 award was presented to Peyman Elison, DPM of Surprise, AZ.

Dr. Elison is part of the AENS surgical faculty and has volunteered his time to teach every surgical course AENS provides since 2011. He is patient with new physicians and takes time to explain the importance of the diagnostic process, as well as the surgical treatment options. He also serves on the surgical course mentoring committee.

AENS Announces New Fellows
The Association of Extremity Nerve Surgeons (AENS) has announced its 2021 Fellow Members: Chris Browning, DPM of Temple, TX, Nathan Larson, DPM of Bellingham, WA, and Porscha Bailey, DPM of Atlanta, GA. These new Fellows were recognized at the 2021 Association of Extremity Nerve Surgeons Annual symposium in Golden, CO on November 6, 2021. AENS President Maria Buitrago, DPM and Incoming President Tracy Basso, DPM presented the certificates.

Fellow members must complete specialty nerve training, submit 200 case logs, and are reviewed by the Fellow committee. Dr. Bailey also completed a 2-year nerve fellowship with U.S. Neuropathy Centers in Atlanta. These physicians worked hard to achieve Fellow status this year on the tails of COVID-19, and are anxious to continue to advance and treat peripheral nerve conditions.

The awards for Healthcare Heroes were determined by nominations and votes made by residents in the community.

A fashion-loving sports podiatrist who spent a decade researching the cause of high heel related foot pain said her wedding day led to her major breakthrough. Emily Smith, from Sydney, Australia was in pain after wearing stilettos all day. She was unable to walk but the aches led her to create an insole to make wearing high heels comfortable.

The key to launching a successful independent practice is proper planning. Knowing what steps to take and what mistakes to avoid is critical to ensuring a sustainable, profitable practice. Follow these five steps to get started down the right path:
2. Envision the Future - Once your mission is clearly defined, set long-term goals for your practice. You can’t get there in one step, but you can certainly strive for it from Day One. For example, do you want to grow your practice to include ancillary services, such as a sports medicine clinic? Do you want to launch your own line of supplements?
The type of practice you want to build will impact the tasks involved in starting it, such as getting credentialed with insurance companies, locating an initial space with room for future expansion, and hiring individuals with the necessary skills to make your practice successful.

RE: Nerve Surgery and Non-Weight-Bearing

I am writing this email as former president of the AENS and as someone who has performed over 500 tarsal tunnel surgeries. I have been privy to many patients with failed initial tarsal tunnel releases, and almost all of them have something in common. They were told by their surgeon to be non-weight-bearing for more than a week, some for a month or more. This makes no scientific or medical sense.
The nerves must glide and move as soon as possible after the surgery to prevent, NOT cause, the nerves to get stuck in the scar that all surgeries form. Yes, 20-40 years ago and more, the protocol was 3-4 weeks NWB and that is why the success rates were 50-70-%. Now, with better technique and understanding of the role of the distal release, the success rate is...

Congrats on your retirement George. Thanks for your advice. I would add though, that if you are conservative and do NOT sell surgery, you should inform patients and let them make the decision to have surgery or not; you will save yourself a lot of headaches. You will do less surgery, but since the patient made the decision, you will be safe from criticism.

After reading Dr. Wallace's "Advice from a Retiring Podiatrist", as another podiatrist contemplating retirement, several things come to mind.. My first thought was of Mickey Mantle, Willie Mays, Sandy Koufax, etc. who were "born too early". I believe Dr. Wallace would have been happier in our wonderful profession practicing as a chiropodist in the 1st half of the 20th century. It's hard to imagine practicing the same way and with the same techniques as when I graduated in 1971 and not trying to learn new innovations and techniques.
I can't help but feel sorry for him that his way of practicing has robbed him of the utter joy of permanently correcting thousands of painful deformities of his patients and the joy of trying new innovations that might have greatly improved his patients’ outcome and lifestyle. However, if that worked for Dr. Wallace, neither I nor anyone else should judge the way he practiced.
I find it hard to believe that Dr. Wallace's other such positive lifestyle choices, which I mostly agree with, are in such conflict with the advice he has given; with the possibility of permanently changing lives and allowing his patients to partake in the same joys of life, one such joy being long non-painful walks by providing the latest treatment options available, basically advising us to stay status quo as a profession. Anyway, if another close-to-retiring podiatrist might give advice, do whatever it takes to achieve the practice and life goals you have set for yourself, keep learning, and advancing your skills, give your patients the best and latest treatments available, and don't listen to anyone else's advice on how you should live your life or practice your profession.

On August 26th, PM News published my post on the disappearance of a nutraceutical - N acetyl cysteine - from the Amazon retail site. N acetyl cysteine is used by cystic fibrosis and covid patients as a preventative for respiratory symptoms. It is also mentioned by Peter McCullogh, MD, an expert in the treatment of COVID-19, and by the FLCCC, which is the "Frontline Covid Critical Care" alliance as part of the 'neutraceutical mix', along with zinc, selenium, etc., where research has shown they are useful in blunting the effect of the virus. It is still missing from Amazon. Look-a-likes, sound-a-likes abound on every site, but no "NAC".
It is not toxic, habit forming, nor effects blood clotting, etc. It may be no better nor worse than many other supplements. But we, the public, are being 'protected' from purchasing it. An additional note: I saw my internist in September and related this story to her. She surprisingly said, "Oh, they're using a lot of that in the hospital." We both did not have time to pursue what was a "You're kidding me!" moment. So, for the last 5 months, NAC has been unavailable on Amazon (thank G-d for ebay), but the hospitals are using it for the treatment of COVID-19? Is there someone out there who can explain this without expressing a political narrative?
