PODIATRISTS IN THE NEWS
CA Podiatrist Discusses Home Treatment of Ingrown Toenails

If you’re already suffering from an ingrown toenail, what should you do? You can try soaking the toe in warm (room temperature) water with Epsom salts, says Michael Coyer, DPM, a foot and ankle surgeon who works at Orange County Podiatry and is a fellow member of the ACFAS.

Dr. Michael Coyer

This may soften the skin surrounding the toenail and help to reduce inflammation and pain. It is important for the water to not be too hot in order to prevent damage to the surrounding tissues, Dr. Coyer says.

PODIATRISTS AND FOOTWEAR
NY Podiatrist Discusses Choosing Footwear for Those With Plantar Fasciitis

When you're on your feet all day for work, the last thing you want is to feel pain in your feet. Orthopedic shoes provide plenty of arch support and cushioning for the heel, along with support for overpronation, which Bruce Pinker, DPM, at Progressive Foot Care, also suggests for those with plantar fasciitis. The wider toe box makes a great choice for people with bunions, swollen feet, or hammertoes, as well.

Dr. Bruce Pinker

Both soft and firm shoes can work for plantar fasciitis relief. "It matters less whether they are hard and soft and more whether they provide proper structure for the arch on the bottom of the foot," says Dr. Pinker.

PODIATRIC PODCASTS
Canadian/U.S. Trained Podiatrist Discusses Peroneus Longus Injury

Ben Pearl, DPM interviewed Mireille Blanchette, DPM. Now in New Jersey, she described how being from Canada and having done part of her training there shaped the way she practices. Her town was not affluent. People purchased corn pads for hammertoes where she worked as a drugstore clerk. Podiatric care was not readily available. When she received training in the U.S., her mentor Dominic Catanese, DPM gave her a foundation of bedside manner and working together with mutual trust.

Click on Dr. Mireille Blanchette's photo to view short video.

Dr. Blanchett shared an interesting approach blending humor and patient education on social media platforms like Instagram. She reviewed treatment algorithms for peroneal and posterior tibial tendonitis, shin splints, and Sever's disease.

RELEVANT RESEARCH
CA Podiatrist Reports Data on Gene Therapy Study at Vascular Biology Meeting

David G. Armstrong, Professor of Surgery and Director of the Southwestern Academic Limb Salvage Alliance (SALSA) at the Keck School of Medicine of the University of Southern California (USC), at the International Vascular Biology Meeting (IVBM), held in Oakland, California last weekend, reported on the first results of a novel phase 2 study of gene therapy to heal wounds and reduce amputations in people with vascular disease.

Dr. David Armstrong

The study, which consists of injecting fragments of a virus into muscle, may help grow blood vessels and improve circulation, thereby increasing the chances of healing. The study, which is currently 90% enrolled, is expected to have its final results available some time next year. Armstrong became the first podiatric surgeon ever to speak to this group.

PRACTICE MANAGEMENT TIP OF THE DAY
Six Strategies to Mitigate Provider Burnout

Here are six strategies to mitigate provider burnout in your practice.

3. Ensure good work-life integration, not work-life balance.

Work is an integral part of physician life. It need not be a negative force competing for employees’ time. Develop an organizational culture that fosters camaraderie with the team to achieve a feeling of fellowship and common loyalty among fellow physicians, so work is more meaningful, and they feel connected to the incredible work they do. Camaraderie, social connection, and positive and supportive relationships with peers are vital components of engagement, satisfaction and well-being. Creating connections can allow for a better personal understanding of colleagues and build engagement, a positive work environment, and a sense of community in the workplace.

RESPONSES/COMMENTS (MEDICARE ADVANTAGE)
From: Judd Davis, DPM

Dr. Kesselman, I too have a tough time understanding why there is not more outrage and commentary on this topic? My guess is that some are afraid to speak up or mostly that people feel powerless to do anything about these huge companies defrauding the taxpayers of billions. Those insurance companies have nearly unlimited funds and likely have huge clout with our politicians on Capitol Hill which may be why our government does nothing to stop it? Corruption? We should all be writing our congressmen to tell them how we disapprove of these egregious activities, selling any shares we may own in these publicly traded companies, and switching our personal health insurance plans to some that are not committing this fraud, if there are any?

Maybe the next time Ciox requests their chart audits we ALL refuse it, knowing darn good and well that this is a ploy to up-code those charts so United can get paid more. There has been some recent talk on...

RESPONSES/COMMENTS (PM EDITORIALS)
From: Robert Kornfeld, DPM

I have been reading the posts about the high cost of being a podiatric surgeon and would like to chime in for those of you who are sick and tired of fighting with insurance companies and being forced into high volume, high expense practices. I am a 1980 graduate of NYCPM. When I went into my own private practice in 1982, there were only cash patients, Medicare, and indemnity plans that paid 80% of everything you billed (there were no fee schedules back then). Some insurance companies paid based on location and estimated expense levels, but most paid 80%.

That golden goose died when podiatric surgeons started billing for multiple procedures with no bundling. This led insurance companies to look for more cost-effective (profitable) models. Hence, managed care hit the scene. Now, you were at the mercy of the...
