Walking is Good Medicine for Healthy Aging: MI Podiatrist
According to Conway McLean, DPM, "The evidence to date supporting walking as a viable, successful form of exercise is irrefutable. And apparently, it is wonderful therapy for such a long list of conditions it should be part of the treatment program for nearly every disease and malady. Yet rarely if ever is it prescribed or discussed in detail, typically given only a cursory nod to its recommendation."
|
Dr. Conway McLean |
"The study results are in. Higher levels of physical activity are associated with reduced all-cause mortality. If you don’t want to spend your last years, which are often longer than imagined, confined to a recliner, then get outside. Explore nature by walking our many trails. Take a brisk stroll and breathe in the fresh air. Soak in the sunshine. Exercise shouldn’t be work, even though you are working out. The evidence is inescapable: too many studies have provided definitive evidence on the topic: the human body needs to move," says Dr.. McLean.

PRACTICE MANAGEMENT TIP OF THE DAY |
10 Collection Tactics for Your High-Deductible Patients
High-deductible health plans are on the rise. Here are 10 methods to ensure your practice collects from patients with these plans.
7. Consider credit card on file
With appropriate safeguards, practices can keep patients' credit cards on file and charge them for patient balances as long as the patient has given you written permission to do so. You do not want the liability of keeping their credit card information on-site, so use a certified and secure third-party to retain the information.

RESPONSES/COMMENTS (NON-CLINICAL) |
From: William DeFeo, DPM
After reading all the comments about unionizing, I feel compelled to add my two cents. Physicians have sold their souls to big business and it was just a matter of time for all of this to come to fruition. Corporate medicine has made countless millions of dollars on the backs of physicians. I saw the recent survey that stated that podiatrists make about $79 an hour. An HVAC specialist makes $145 an hour as a high school graduate and does much better than most podiatrists. To put a shingle out today requires 11 years of higher education and personal moral and legal responsibility. It's hard to put a price on that.
If you decide to put your own shingle out, you have the responsibility of being a businessman. There is, of course, kudos for taking care of people’s healthcare. But the sad reality is that for the amount of sweat and tears required, it’s starting to get ridiculous. The doctor suffers, and in the long run, so does the patient. I remember the old days; I retired after 45 years of practice and I don’t think we’ll ever see them again. I wish there was a solution that I could suggest, but I don’t think there is one.
William DeFeo, DPM, Allentown, PA

RESPONSES/COMMENTS (CLINICAL) |
From: Elliot Udell, DPM
Dr. Rubin is starting an interesting discussion. What rudimentary tests, learned at our academic clinics, should stand the test of time and what tests ultimately fade the moment the student graduates? For example, we learned the Buerger test when I was a student back in the late '70s but the test did not have longevity as compared to other tests that we still do today to evaluate arterial circulation.
Several years ago at a board meeting of the American Society of Podiatric Medicine, we tried to come up with a consensus on how to screen diabetics for peripheral vascular disease. Each practitioner was in some respected area of academia. Each had his or her pet set of tests that differed from the others in the room, including one who touted the Buerger test. We set out to formulate a consensus but realized that among the academicians in that room, there was and would not be a consensus.
Elliot Udell, DPM, Hicksville, NY

RESPONSES/COMMENTS PM ARTICLES) |
From: Paul Kesselman, DPM
Dr. King's article, "Code for What You Do" in the current issue of Podiatry Management, is exactly what providers should be doing. This article is exactly what my recent Letter to The Editor suggests. Don't be afraid to bill the level 3-5 visits, so long as the medical decision-making or time elements are properly documented. Don't be afraid to bill anything you do, so long as you have documented it. Those who have niche practices will be outliers. Is it so bad to be an outlier if you have a subspecialty area you mostly provide services within? No, just document those services well according to the policy and you should be fine, especially at the higher levels of appeal.
Auditors, especially at your payer level, are paid to fail you. They don't get a pat on the back if they just...
Editor's note: Dr. Kesselman's extended-length letter can be read here.

YOU CAN'T MAKE THESE THINGS UP |
RE: Bedside Manners
Many years ago, I made a house call on an old woman with a wicked sense of humor. When I knocked on the door, she told me to come right in and when I got there, she called out that she was in the bedroom and asked if I could care for her in there. No problem, as I sat at the foot of the bed and started working. As I worked, her phone rang. She picked it up and said "I can't talk to you now. I have a man in my bed" and hung up as I doubled over laughing.
