NY Podiatrist With Parkinson's Disease Teaches Zumba Classes
For evidence of the benefits of Zumba on Parkinson’s, look no further than Dr. Beth Hochstein. The 41-year-old retired foot surgeon was diagnosed with the young-onset form of the condition six years ago. "First, I noticed my little finger started to lock, and then my thumb. I ignored it, but then a month later my whole right hand started to tremor and I went to a doctor. After my diagnosis, the consultant advised me to exercise. A friend suggested Zumba, and I was hooked after the first class."

"Three years later, I retired from my medical practice and decided to train as a Zumba instructor. Now I teach a class specifically for those like me with Parkinson’s. I have about ten students of different levels of ability. We don’t know why it helps yet, but I know it does," said Hochstein.

Aussie Podiatrists's Expansion Includes Plans for Foot and Lower Limb Hospital
Brisbane is going to become a "foot hub for all Australia", according to one nominee for the 2013 Lord Mayor's Business Awards. Brisbane born Darren Stewart co-founded My FootDr podiatry centers with Greg Dower, and together their business has expanded from one lonely practice in Bulimba in 2004 to 19 locations across Queensland - and one in Singapore. “We have a unique brand of podiatry which is different from the traditional wait and see approach,” said Stewart. “People want to be treated quickly these days, and now we can do in half an hour what used to take two to three weeks with our focus on innovation and new technology.”

“Brisbane is going to be a hub for Australia, all our work we've done out of Brisbane, but we want to take our services around the country,” he said. The pair's vision for the future even includes developing a multi-disciplinary foot and lower limb hospital for Brisbane. “We're looking to expand into China early next month or the month after because they don't have podiatry services there, and the growing middle class now demands the high quality of healthcare that is available elsewhere in the world,” said Stewart.

Don't Demotivate Staff - Part 2
You need all hands on deck—committed to the job and focused on reaching your goals. If you make these mistakes, however, you’ll end up with a bunch of unmotivated, disgruntled staff who won’t give it their best:
4. Making implied threats. Even if you are respectful and diplomatic, a “do this or else” attitude will destroy employees’ morale. Example: “Responding to an employee’s objection to take on an assignment that is out of the scope of the person’s expertise or skill set with “Well, in order to justify keeping you on, I need you to take on more work” may quickly send someone looking for another job.
5. Failing to follow through. Nothing is more frustrating than a boss who solicits feedback, and then does nothing with the information. It wastes employees’ time and shows a complete lack of respect.
6. Overlooking creative thinking and problem-solving skills. When employees take it upon themselves to improve a situation or offer you advice for doing so, don’t ignore it. Praise their initiative to encourage the positive behavior in the future.

Query: CO-97 Adjustment Reason Code
I billed for an office visit with an injection, and received a CO-97 "Evaluation and Management Services, with Direct Reference to Global Surgery Denials: Services submitted for the same patient by the same doctor on the same day as or within the post-op period of a major/minor procedure are bundled into the global surgery package and are not paid separately" adjustment reason code on the EOB.
How should this have been coded in order to get both E/M and injection paid?
Amanda Bartell, DPM, Jacksonville, FL
Response: To be paid for both an E/M service and a procedure on the same day, you would append the E/M service with a "-25" modifier.
That is assuming that the E/M component was "significant and separately identifiable" from the procedure performed. Otherwise the E/M component is included in the procedure allowance.

RE: 1st Annual Richard O. Schuster Memorial Seminar
From: Charlers Ross, DPM
As a regular reader of PM News, I have noted many mentions of the fine work that Dr. Armstrong is doing, yet I still believe that the biomechanics/orthopedics of podiatry goes virtually unnoticed.
This past Sunday, I was fortunate to have been invited to be included as a guest speaker at the 1st Richard O. Schuster Memorial Seminar held at NYCPM and was sponsored by a gift from Jerry Seinfeld in his grateful appreciation to...

RE: Using Smartphones to Document NH Visits
From: Joseph Borreggine, DPM
Any identifier information such as time, place, name and/or telephone number, etc. is enough to breach HIPAA when it comes to privacy. Most cell phones have GPS tracking built in. So, not so fast when you say it's all right to use them. Here is a good article on the topic.

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B | |
From: Richard A. Simmons, DPM
Dr. Brody writes: “1)If you have a low-cost digital camera that does not leave your office, what is the risk of you losing it outside your office? 2) What is the risk of losing or misplacing a smartphone outside your office?”
With all due respect, the topic is about using smartphones to document nursing home visits. The point behind using the smartphone in the nursing home is that the phone will provide longitude and latitude coordinates along with the date and time in order to establish where you were and when you were there. As we all know, we can manually change the date and time of low-cost digital cameras; however, smartphone digital images are embedded with a proprietary date and time stamp, over which the operator has no control.
I agree there would not be a need for a smartphone in an office setting, but that is not the subject of this conversation.

| RESPONSES/COMMENTS (NON-CLINICAL) - PART 1c | |
From: Michael L. Brody, DPM
E-PHI includes among other things: retinal scans, facial geometry, biometric information, and any other information that could be uniquely utilized to identify an individual.
Ray Posa states that we should "Feel free to document wounds with a smartphone." Ray stated that a picture of a wound is not e-PHI if the name of the picture is randomly generated. I agree that his interpretation of the rule has merit and validity. But an interpretation of the rule that a picture of a wound is e-PHI is also potentially...
