MD Podiatrist Provides Reason to Wear Shoes in Your Home
 
Roughly two out of three Americans remove their shoes when they get home, according to a May 2023 poll from CBS News and YouGov. But if you’re in the camp who does it for cleanliness, is it really necessary? Turns out the experts don’t all agree. A reason to keep your shoes on has nothing to do with germs. Going barefoot for extended periods can lead to strain on your feet, particularly when walking and standing on hardwood floors. Like many podiatrists, Priya Parthasarathy, DPM, a board-certified podiatrist in Silver Spring, MD, has seen an increase in patients with foot problems since the pandemic.

With more people spending their days barefoot working from home, she says conditions such as plantar fasciitis and an inflammation of the ball of the foot called metatarsalgia have become more common. Barefoot walking doesn’t just affect your feet,” says Parthasarathy. “It can also migrate upward, causing pain in the knees and the hips and the back.”

MEDICARE ADVANTAGE NEWS
Cigna to Pay $172M Over Alleged Medicare Advantage Fraud
 
The Cigna Group will pay $172.3 million to resolve allegations that it violated the False Claims Act by submitting incorrect Medicare Advantage patient data to CMS to receive higher payments from the agency. The U.S. Attorney's Office for the Eastern District of Pennsylvania alleged Sept. 30 that Cigna also falsely certified that the submitted data was accurate, failed to withdraw the "untruthful" data, and did not repay CMS.
 
Cigna will use $135.3 million from the settlement to resolve the allegations from the Justice Department. The remaining $37 million will resolve allegations related to unsupported diagnoses for Medicare Advantage enrollees that received in-home services from Cigna. As part of the settlement, Cigna has entered into a five-year accountability and auditing agreement with HHS' Office of Inspector General, which will require company executives and board members to certify Cigna's compliance moving forward. The payer must also conduct annual risk assessments and submit to independent risk adjustment audits.

It is imperative to offer a few different things to ensure your staff is operating at the highest level possible, regardless of if they are licensed or not. It is imperative because ensuring they are trained well decreases liability issues and increases their value to your clinic.

Be sure keep a copy of a staff member’s check-offs in her file; also keep a binder of protocols for the staff’s reference.

I really enjoyed the September editorial on threats to podiatry. I was particularly sad to read "Millennials are not joiners”. I am not a joiner either and I have many reasons why I left. The medical associations do not reach out to new practitioners, unless they want a donation. When I started out, I was reading how the medical associations fought to allow podiatrists to perform amputations and have hospital privileges. That was great, but I am just getting my feet wet and haven’t seen a patient yet. What I am saying is, if they want professional loyalty, they should offer help and guidance to support success in our profession...even if they just made a phone call to see if there was anything they can do to help the young practioner avoid being cannibalized. A simple pamphlet with important information and numbers to contact, given to new licensees would be a simple gesture towards sowing membership. 
 
I also think the California Podiatric Medical Association (CPMA) should not require joining the APMA to become a member. In these difficult times, organizations that are supposed to help the profession should be looking for ways to give their constituents a break. When I was a member, I did reach out to the CPMA for advice, but they could not bother to provide any guidance. This was a reason I canceled my membership. If you want people to give you their support, then you need to be in contact with them, and show you care, before membership is up for renewal.

RE: CMS Proposed Reimbursement Skin Substitute LCD Changes  to be Rewritten. Current Coverage Remains Until That Time.

CMS has made a statement that the reimbursement changes proposed for skin substitutes and related products has been rescinded and the LCD will be re-drafted. Please note the following descriptor of the quote that came from CMS:
 
September 29, 2023: Skin substitute grafts/cellular and/or tissue based products for the treatment of diabetic for ulcers and ulcers will not become effective on 10/01/2023. A new proposed LCD will be published for comment and presented at an open meeting in the near future. In the meantime, current coverage has not changed.
 
APWH will closely monitor and update this newsletter as needed. We also monitor to make comment during open meetings and provide testimony if necessary.

To note Dr. Kesselman (with whom I hold high regard), the three CMS jurisdictions (CGS, Novitas and FCSO) retracted their proposed cellular tissue product for DFU/VLU was not because of spite or a “Shot Across the Bow”.
 
There has been an explosion of Q-coded amniotic allograft tissues on the market since 2016. More so during Covid. Start-up companies paid to have their AATB (American Association of Tissue Banks) certification and came out with products that had ZERO evidence for coverage under the LCDs. However, they were granted through the FDA under the Part 361 process of “homologous use” product pathway. What these local MAC administrators found was the significant uptick in POS 11 claim data resulting in expenditures for tissues that were either not medically necessary, or did not meet the qualification of...

Here in Oregon, we’ve been dealing with HealthNet MA plan since 2016 RE: modifier -25. Over these past 7 years, Oregon DPMs have lost thousands of dollars in denials, hundreds of hours of time, and experienced countless administrative burden to submit appeals/paperwork, etc. to “win” a battle with ONE insurance company for ONE code. Finally, this year, we had a breakthrough (after hundreds of emails and multiple meetings with HealthNet) and thought we resolved the issue. We were giving each other high fives and pats on the back for a job well done. Then last week, I received EOBs from 2 HealthNet patients denying payment for office visits. So, here we go again. Same as it ever was.
 
We call it a “victory” when we win an appeal for money we should have been reimbursed in the first place. I call it DISGUSTING and CRIMINAL. Money is being stolen from us every day! I followed APMA’s guidelines and played by the rules, but I’m sorry folks…it’s not...
