PODIATRISTS IN THE NEWS
NY Podiatrist Discusses Causes of Bromhidrosis

Blame shoes and socks. Enclosing the foot increases heat and traps sweat, said Robert Kornfeld, DPM, a doctor of podiatric medicine who runs a holistic and restorative foot and ankle practice in New York. "If the foot is allowed to [air out] efficiently, you don't build up the breeding ground for bacteria," Kornfeld told Live Science.

Dr. Robert Kornfeld

Dr. Kornfeld said he advocates dietary changes, including getting rid of refined sugars, to strengthen the immune system and keep pathogenic bacteria at bay. He also suggested a roll-on antiperspirant applied to the foot. People who are chronically stressed also tend to sweat more, he said, so foot odor is sometimes a sign of a wider systemic issue. "What's interesting about foot odor is a lot of people are so embarrassed they won't even go to a doctor," Kornfeld said. "But there's help out there."

VA Podiatrist is Keynote Speaker At GWU Pre-med Student Association

Ben Pearl, DPM was the keynote speaker last weekend at the George Washington University (GWU) Chapter of the American Medical Physiology Club where he presented a keynote address titiled, “Regenerative Medicine in The New Age of Healthcare”. Dr. Pearl spoke to the students about the importance of collaboration between specialists and internists in healthcare as well as the emerging science of regenerative medicine.

Dr. Ben Pearl (5th from right, back row) with GWU pre-med students.

Pearl was impressed by the diversity of the students spanning disciplines from public health to biomedical engineering. Students were intrigued by a parabiosis mouse study that showed migration of stem cells from one mouse to another injected with placenta growth factors. Pearl concluded by telling the students that if they follow their passions, they will find the path to success.

PRACTICE MANAGEMENT TIP OF THE DAY
How to Have a Well-run Practice - Part 5

The daily grind of seeing patients can sometimes be, well, grinding. It’s important to take a step back and reconsider how you do things. There’s likely another way to achieve the same, if not better, result, especially when it comes to staffing. Here’s a sample of what 943 practice owners, managers, and physicians say has helped them create a better environment.

5. Give your employees the freedom to do their jobs. Don't micromanage them down to the level of quitting.

I interview for disposition, mainly, and then the candidate’s ability to learn a new skill set. I can train someone who is motivated, but I cannot change dispositions.

CODINGLINE CORNER
Query: CPT 12032

Recently, I had a melanoma in situ removed from my posterior right calf per a dermatologist. The lesion was 2.1 to 3.0 cm and coded CPT 11623 but the additional code of suturing intermediate wound CPT 12032 was added to the bill. I questioned this as I do not bill it when excising such lesions despite undermining and retention sutures. The surgeon called and said that she closed the subcutaneous tissue so the use of an intermediate code is normal for dermologists to bill automatically. I looked up by definition of excision (the operative report said “to the layer of adipose tissue”), thus this intermediate code seems a bit much.

Do podiatrists use this code for excisions? My thinking is you excise, you close; if a lot of work, maybe the code CPT 13101 could have been used for undermining and retention sutures. Thank you for giving me info to work with since I was billed another $600 for intermediate sutures! Sutures!

Response: First off, there is nothing that is “normal”. Each procedure stands on its own. In the CPT book, it states you can bill for intermediate and complex closure. But that requires that this type of closure is medically necessary and reasonable. Was deeper closure required or is this billing just a matter or routine (i.e. normal) to get another $600. You might want to ask for the op note to see the actual closure in your case. Ultimately, anything you bill needs to be substantiated in the op-report.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Loretta Logan, DPM, MPH

As chair of the Department of Orthopedics and Pediatrics at the New York College of Podiatric Medicine (NYCPM), I feel it is necessary to respond to your recent comments regarding the current state of orthopedic training at the schools of podiatric medicine. While I agree with the consensus that the younger practitioners entering practice have placed surgery at the head of their treatment plans, the first sentence in point #1, “The undergraduate level of biomechanical training is apparently shameful as compared to when I was at ICPM 1977-1981” was particularly troubling.

I would be happy to share curriculum documents with you, which show the evolution and expansion of orthopedic training from...

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From: Steven J. Kaniadakis, DPM

It seems that podiatry school programs are pushing for students to get more biomechanics instruction from their residency programs and other post-graduate programs. Podiatrists have  typically been very well versed in these (and other) subjects, including reading and understanding x-rays and radiology, surgery skills, etc.

Generally, podiatrists have been very well educated before residency and other post-grad programs compared to other medical school students. Even some practicing MD physicians do not even know which way to hold an x-ray (unless they were in a radiology residency or fellowship specialty program). However, generally speaking, MDs are much better versed in labs, etc.
