PICA Awards Full-Tuition Scholarship to KSUCPM Student

This year’s recipient of the PICA full-tuition scholarship is fourth-year student, Yash Bhalala at Kent State University College of Podiatric Medicine (KSUCPM). Yash was nominated as KSUCPM student-of-the-year by his peers. He is known as the first student to arrive on campus and the last student to leave. Yash is also known for never saying “no” to assist a student for tutoring. He has excelled in clinical settings and demonstrates great compassion and attention to detail when caring for patients.

NY Podiatrist Discusses Benefits of Compression Socks
 
Whether you’re an avid runner, a frequent customer at your local gym or a casual walker, you may be missing out on not-so-secret benefits that compression socks have to offer. Michael Mazer, DPM, a podiatrist and associate of the American College of Foot & Ankle Surgeons, told HuffPost that these particular benefits include increased stamina and less lower extremity discomfort post-exercise.

Query: Repeated Wart Treatment
 
I have been hearing some confusion regarding wart debridement using CPT 17110. Due to the nature of warts, it is unlikely that one debridement fully eradicates the issue. Is billing CPT 17110 each time a patient comes in for debridement incorrect since the description states “destruction?” I do not see anything in the LCD limiting this to one time treatment. I do see a 10-day global period. I was advised that after billing CPT 17110 one time, following visits will need to be billed as an E/M. Can anyone provide some clarification as to why this would be necessary?

Response: Let’s make sure you understand CPT 17110-Destruction (e.g., laser surgery, electrosurgery, cryosurgery, chemosurgery, surgical curettement), of benign lesions other than skin tags or cutaneous vascular proliferative lesions; up to 14 lesions.
 
Where is there anything above stated that this is a debridement service? Certainly you might pare or debride necrotic tissue but the goal is to do something destructive. If you don’t destroy any tissue, then do not use CPT 17110! As for its repeated use, it has a 10-day global, so you would not perform another destructive treatment when medically necessary for 2-3 weeks, allowing the previous method to destroy the wart tissue. If it did not completely destroy it, then treat it again knowing that you're attempting to eradicate the viral, diseased tissue. If after a few tries nothing changes, then you probably should pick a different more definitive treatment. If you are performing a minor surgical service that is not an E&M, whoever recommended that is just wrong. If you prescribe medication, then that most likely would make it an E&M as long as you satisfy the 2021 E&M guidelines.

Much has been said of board certification primarily being a "protection of the public". Does that mean that those not board certified by ABFAS are not safe to do surgery and a danger to the public? If this is the case, take the residency class of 2014 which have had 8 years to get board certification (the max is 7 years). Per the ABFAS website, 118 have passed RRA case review and 259 passed foot case review.
 
There were 525 in this residency class. If all those doctors also passed the CBPS exam (unlikely), then 22% of the residency class are deemed safe to do rearfoot surgery and only 49% are deemed safe to do any surgery. Thus, either more than half of our 3-year surgically trained podiatrists are not doing surgery or are doing it in a dangerous manner, possibly harming the public. If either one of these is true, then the residency model needs to have a complete overhaul. More likely, the board certification process is not a good measure of surgical competency of a podiatrist, and it needs an overhaul.

I would like to address the ongoing issues of CAQ and the Boards and make some salient points to clarify many of the statements made. These are my personal opinions and what I have observed in an academic setting as well as being a residency and fellowship director:
 
• Board certification demonstrates minimal competency; it does not demonstrate the fact that someone excels in any form of surgery or medicine. In fact, Board Certification tests Minimal Competency as the definition below states:...

It’s time for ONE BOARD CERTIFICATION. It has been over 20 years since I was president of ACFAOM (now ACPM), and I sit on the sidelines wondering what’s the overwhelming purpose to a CAQ in surgery?  As I was board-certified in both surgery (ABPS) orthopedics (ABPO) and medicine (ABPOPPM), I recognize that most systems have flaws and our certification process is no exception. There are many people, while in their residency, who have logged cases that they did not personally perform, and I can go on and on with our dirty laundry; but, what’s the purpose? For years, the Holy Grail of success has been ABPS (now ABFAS) certification. But over the last decade, our education model has changed, with three years of residency training across the country and post-fellowships for added information beyond this point. It’s time for change. 
 
I believe Lee Rogers is an honorable, bold, forward-thinking leader in our profession who is scratching an itch that we all have. It’s time for one board certification for our profession. One board will allow CAQ in different areas of expertise while eliminating the problems with third-party payers, hospital staffs, and political foes. Who’s brave enough to agree?
