DMU-CPMS' White Coat Ceremony Includes 56 Students
On September 19, the Grand Ballroom of the Community Choice Credit Union Convention Center in Des Moines was buzzing with excitement as 457 clinical students from five programs at Des Moines University Medicine and Health Sciences received their white coats during the University's annual White Coat Ceremony.

The White Coat Ceremony marks the start of a student's medical education toward becoming a compassionate and competent healthcare provider. Participating in DMU's ceremony were 56 students in the Doctor of Podiatric Medicine program. Kevin Smith, DPM, PhD, MS, Dean of the College of Podiatric Medicine and Surgery, told students that their white coats symbolize responsibility, but true greatness in medicine comes from compassion, integrity, and the way providers care for others.

People with Diabetes Face Unique Risks: KS Podiatrist
We spend a lot of time on our feet, which makes it only natural we can have a variety of issues with them. “I see the issues just walking down the street. It’s very common. A lot of people are embarrassed about their feet so a lot of them don’t come in until the problem has gotten quite down the road,” said Henry Goppert, DPM, a podiatrist leading the recently re-opened Cotton O’Neil Foot and Ankle Clinic.

Dr. Goppert does foot checks for people with diabetes, who face unique risks. “If you have high blood sugar level, the proteins in blood actually are inhibited. You don’t really heal at all, wound-wise or incision-wise, for about 24 hours,” Dr. Goppert said. “It also can lead to nerve issues, nerve damage.”

The American College of Foot and Ankle Surgeons (ACFAS) announced that Arthrex, Inc., a global leader in minimally invasive orthopedics, has provided support for the ACFAS Clinical and Scientific Research Grant Program. This sponsorship strengthens the College’s mission to advance evidence-based care through high-quality clinical research and fosters innovation in orthopedic surgical procedures.
The ACFAS Clinical and Scientific Research Grant Program provides funding for investigator-initiated studies that address meaningful clinical questions, generate new knowledge, and translate research into improved patient care. Each year, grants of up to $40,000 are awarded to support projects conducted by ACFAS members, with priority given to multi-center trials, prospective studies, and research that demonstrates direct clinical relevance to foot and ankle surgery.

MEDICARE FRAUD AND ABUSE |
HHS Watchdog Calls Out Fraud and Abuse in Skin Substitute Market
The HHS Office of the Inspector General (OIG) called out sky-high spending on pricey skin substitutes and voiced concerns about potential waste, fraud, and abuse in a report released earlier this month. Spending on skin substitutes for the treatment of chronic wounds has soared over the last 2 years, reaching more than $10 billion in 2024 -- more than 15% of the $66 billion in total spending for all Medicare Part B drugs that year, according to the HHS watchdog.
The report also pointed to several "concerning characteristics" seen in skin substitute billing, including:

New providers for whom nearly 100% of claims are for skin substitutes

Providers filing multiple claims for a single date of service to bypass a $99,999.99 cap above which claims are rejected

Skin substitutes used for non-approved conditions or in excessive quantities
-
Regularly using skin substitutes during enrollees' first visits without first attempting treatment with more conservative options

RE: Risk Assessment Audits for Part C Plans

As expected, CMS has clamped down on their auditing of risk assessments performed on Part C plans. It remains unclear at this time whether this will change the burden on providers, as Part C plans continue to require network providers to cooperate and submit charts for review.
In reviewing this multi-page document (a snoozer), it appears that carriers will have a narrower number of diagnoses to report per patient chart (reduced from 5 to 2). Does this necessarily change how many patient chart requests specialists will be targeted for, or increase the risks for the family internist? For more information click here.
In other news, Becker’s reports that the a court decision in Texas ruled in favor of Humana. This decision removed almost 17% of Humana’s earnings ($900M) from potential risk assessment audits. Despite this setback (mostly based on a lack of proper notification and procedural), CMS continues to advocate that it will continue to aggressively pursue risk assessment audits of Medicare Part C plans.

Although many of us have disagreed with Drs. Kornfeld and Roth with regard to not accepting insurance as payment for our services, the times we live in might cause us to rethink our disagreements. I recently had cataract surgery. The doctors agreed to accept insurance for the removal of the cataract, but in order to use their laser machine to make and close the incisions and correct astigmatism, I had to cough up two thousand dollars, cash.
The bottom line is that this prominent and well-respected eye care center was not willing to give up taking insurance totally, but they found a good way to supplement their income by charging an out-of-pocket fee for part of the procedure. Perhaps Drs. Kornfeld and Roth can suggest ways in which podiatrists could accept insurance for certain services and charge out-of-pocket for others.
