MI Podiatrist Opines on the Role of Artificial Turf on Achilles Ruptures
According to Conway McLean, DPM, "Although relative only to the world of sports, the loss of Aaron Rodgers from the New York Jets roster for the remainder of the year (and possibly beyond?) has been called a tragedy. Three offensive plays into the season, the first game for his new team, after all the fanfare and hype, the star quarterback tears his Achilles tendon. The injury dominated the headlines for several new cycles. The event occurred at the stadium of the Jets which, interestingly, has artificial turf. Was the surface upon which the injury took place a factor?"
|
Dr. Conway McLean |
"Physicians specializing in sports medicine seem generally in agreement: a higher number of injuries, with a greater degree of severity, result from the use of artificial turf. Players are able to achieve greater velocities faster, leading to more physical stress to the structures connecting the body with the immovable object that is the earth. Because the foot is often locked in position, cleated to the turf, it’s the ankle and associated structures which must bear the greatest force. The studies are beginning to accumulate, yet little has changed in transitioning the professional football games away from artificial turf," says Dr. McLean.

PODIATRISTS AND FOOTWEAR |
KY Podiatrist Discusses When to Replace Shoes
When you get a pair of tennis shoes, you expect them to last you a long while. Turns out, that may not be the best thing for your feet! If you’re not replacing them often enough, podiatrists say that could be the reason the pep in your step gets replaced with pain. “80-90% who are having problems with their feet, the first question I ask them is what kind of shoe they are wearing, and how long have you been wearing them?” said Avneet Kaur, DPM, foot and ankle surgeon.
|
Dr. Avneet Kaur |
“If you’re having issues with your feet, fatigue, hip or knee pain, that’s a sign you need to buy a new shoe,” Dr. Kaur said. How often should you be replacing your shoes? It depends on your activity level, but Kaur says around every 500 miles or every eight months on average. If you’re an avid runner, you may want to consider a replacement every three to five months. “After that, the shoe insole will wear out. The thing to look into is the midsole compression lines,” said Kaur. “If you notice those compression lines that are there quite a bit, it’s time to get a new shoe.”

Every Diabetic Needs a Yearly Foot Screening: Malta Podiatrist
Every patient living with diabetes should go for a foot screening at least once a year, professors at the University of Malta’s Department of Podiatry urged. “All guidelines worldwide suggest that all patients with diabetes should have at least one annual screening,” Professor Alfred Gatt told The Malta Independent.
|
Dr. Alfred Gatt |
“Anyone who has diabetes is at risk of losing their foot. We have more than 10% of the population in Malta who have diabetes,” Gatt said, adding that some 500 partial or full foot amputations take place in Malta every year.

PRACTICE MANAGEMENT TIP OF THE DAY |
8 Tips to Improve Patient Satisfaction
It’s more important than ever to be able to track and monitor patient satisfaction and take steps to make improvements to meet the changing expectations of patients. To effectively gauge what patients think and identify areas for improvement, here are best practices to follow.
1. Keep it simple - You can’t do anything about a pre-set CAHPS survey, but you can control the length and complexity of a patient survey you send out. It shouldn’t take more than five minutes to complete and should focus on a single topic, like their recent visit, or changes you’ve made (like new technology or services). Don’t try to do it all in one survey.

NON-PODIATRISTS OFFERING PODIATRY SERVICES - PART 1 A |
From: Ivar E. Roth DPM, MPH
Since others are now claiming to offer our services, I guess podiatry has arrived as a profession with recognition. It seems like there should be a mechanism in place to stop “others” from using the word podiatry in their advertisements when they are not podiatrists.
Ivar E. Roth DPM, MPH, Newport Beach, CA
|
|
NON-PODIATRISTS OFFERING PODIATRY SERVICES - PART 1 B |
From: Lawrence Rubin, DPM
I want to call attention to yet another issue regarding nurses providing foot care services customarily provided by podiatrists. This was stated in a recent PM News post: "Samantha Pedersen, an advanced registered nurse practitioner, has joined Prosser Memorial Health and will be providing podiatry services at Prosser Orthopedic Center and wound care at Prosser Memorial Hospital."
The use of the term "podiatry services" is misleading. Only podiatrists can provide podiatry services, because "podiatry services" clearly means services provided by a podiatrist. It is common now for "podiatry services" to be improperly used synonymously with "foot care services." So, if this continues to occur, expect pedicurists who routinely file down hypertrophied nails to also be providing podiatry services.
Lawrence Rubin, DPM, Las Vegas, NV

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1 |
RE: Football Shoe Designed to Prevent Injuries
From: Mark H. Feldman, DPM
I am 82 now and will probably never see my preventive design realized. Over a year ago, I talked about my design for an ultramicro instant breakaway of a shoe sole or upper that would prevent foot, ankle, tib/fib, femur, and hip injuries in football. Our profession is blessed with mechanical, electrical, and electronic engineers.
I believe an astrophysics engineer will be needed to complete the concept. My thoughts and design are free to anyone who inquires. I hope the final design, production, and implementation of such a device is done by a podiatrist, even though it is more likely than not that I will never see the completed product.
Mark H. Feldman, DPM, Pompano Beach, FL

RESPONSES/COMMENTS (NON-CLINICAL) - PART 2 |
From:Ivar E. Roth, DPM, MPH
I would like to respond for several reasons to Dr. Manolian’s post. First, I was surprised by his statement. “I thought we established last year that you did a 2-year residency as there were only 2 three-year residencies when you graduated, none of which you matriculated in, but for some reason you keep mentioning it.”
I am very proud of the fact that I completed 3 years of podiatric surgical residency training back in 1985. For sure this was a rarity and for those that might not know, the first two years were at Franklin Boulevard Hospital with Dr. A.O. Mercado who is a PM News Hall of Famer. My third year was at Mount Sinai Medical Center in Chicago, a teaching hospital for Chicago Medical school as a PGY-3 in the department of surgery as their first...
Editor's note: Dr. Roth's extended-length letter can be read here.

RESPONSES/COMMENTS (CLINICAL) |
From: Rod Tomczak, DPM, MD, EdD
Last month, I posted a note in PM News about GLP-1 Receptor Agonists and gastric regurgitation and aspiration during anesthesia, questioning a correspondence from Dr. Gurnick and its accuracy. Here is a follow-up article printed from Medscape titled, "Jury Still Out on Stopping GLP-1 Receptor Agonists Prior to Elective Endoscopy". I think this is important when eliciting a patient review of systems and medications prior to elective podiatric surgery.
Rod Tomczak, DPM, MD, EdD, Columbus, OH
