Overuse Injuries Often Caused by Exercise-induced Muscle Imbalance: MD Podiatrist
 
“The foot is complex and complicated,” says Damian Roussel, DPM, podiatrist at The Centers for Orthopedics, one of the largest orthopedic groups in the country. “Obviously their function is different, but in makeup, feet are somewhat similar to the hands in terms of the bones, muscles, tendons, and ligaments.” Dr. Roussel notes that although things like tendonitis and plantar fasciitis, can be aggravated by foot type, they are usually caused by exercise-induced muscle imbalance.

What should be your first steps if you have foot muscle issues? First, go to the podiatrist! “If a patient thinks they have something going on with their feet, they should get seen by a doctor to be evaluated,” says Roussel. If the doc confirms that there’s a soft tissue injury, Roussel says the move is to then head to a physical therapist, who can provide a series of stretches and strengthening exercises to help.

2. Prioritize the Patient Experience
 
Consider how your patients interact with your practice. Medical practices historically straddle the line between public service and for-profit business services. Unfortunately, many physicians have failed to implement the key essence of running a business: prioritizing customer care. Prioritize the patient experience by showcasing that you value their time rather than letting them wait past their scheduled appointment. For example, have someone in place to help Medicare beneficiaries navigate their Part D plan for insurance or what a patient can expect during cancer treatment. Prioritize the little touches, like addressing them by name and using active listening skills.
 
The more valued a patient feels, the more comfortable they'll be when sharing personal information. Prioritizing the patient experience builds trust beyond the basic confidentiality agreements. This open dialogue allows physicians to provide better care and prevents patient turnover.

Query: Casting Supplies
 
We recently casted a patient for functional foot orthoses and we billed for A4580 x 2. The claim was rejected. It was noted that we needed a “Q” code. Can you please advise?

Response: The carrier was correct to deny the A4580 as that code is no longer paid separately under Part B Medicare. Even a “Q” code is incorrect as those codes are not supposed to be used for anything but casting supplies for trauma such as fractures, sprains, dislocations.
 
My opinion is that the best code to use to indicate you are casting a patient for S0395 which is essentially defined as a slipper cast used during the impression for foot orthotics. Caution is that in many cases the third party payers will not cover this code if you are the medical provider dispensing the orthotics and they only will pay for it if you took the impression and the device was provided by another medical provider (supplier). You might want to check with the carrier prior to submitting your claim for further clarification.

I applaud Dr. Kesselman’s recent query about this subject. Podiatrists are bombarded with these chart requests via annoying several page faxes and threatening phone calls. We take the time to request payment for these services and generally get no response. As I stated in an earlier query, my office protocol is to demand their chart requests be sent via certified mail, not annoying faxes or telephone calls. These companies are not Medicare. They only rip Medicare off!

I don’t understand Dr. Kesselman’s posting. He informs us Medicare plans are being investigated by the OIG for False Claims Acts. Okay, If they are doing this, it is wrong and they will be punished accordingly. There is a disconnect in this posting when speaking to us as providers. Dr. Kesselman applauds those providers who “play hard ball” in releasing charts to these carriers. He further alludes that those who “roll over” and send charts should reconsider. 
 
Dr. Kesselman is tying together fraud being committed by a company with doctors giving chart notes. I fail to see the relationship. If a company wants to commit fraud, they’ll commit it. Doctors are obligated under their contracts to provide chart notes if requested. I understand if the request comes from a third party vendor, then one is entitled to be paid for the work it takes to forward the request. 
 
Let’s not confuse fraud by one party with contractual obligations of another least the second party get themselves in trouble as well. One of the issues at hand is the fact insurance companies can do as they please without much oversight. This is a problem that fails to be addressed.
