INTERNATIONAL PODIATRISTS IN THE NEWS
Hairy Toes Indicate Good Foot Circulation: UK Podiatrist

Everyone grows toe hair, but some people’s toes tend to look more Bigfoot than human. And if your toes are a touch more wooly than the average person’s, it may make you feel self-conscious. But, according to experts, you shouldn’t be. Having hairy toes is actually a sign of good health.

Jeremy Ousey

“Having hair on your toes is a good sign because it means that you have good circulation. You can’t produce hair on your toes without having a blood supply,” says Jeremy Ousey, a podiatrist based in the United Kingdom. “The further you get away from the heart, the poorer the circulation is going to be,” says Ousey. That means it’s most difficult to get blood to your toes, and if you’re growing five bushes on each foot, your blood is probably pumping correctly.

PRACTICE MANAGEMENT TIP OF THE DAY
3 Marketing Metrics for Medical Practices

There are a few metrics to consider when using marketing techniques to attract new patients to your practice.

2. The cost per acquisition (CPA)

However, not all initial callers will convert to paying patients. The 50 patients who made appointments can be plugged into the same equation, i.e., campaign costs divided by patients who became paying patients or $2,000 divided by 50 equals $40, representing the patient acquisition cost (PAC). Now, if each patient who entered the practice spends $800 over the patient's lifetime, that's an increase in income of $40,000, not shabby for $2,000 in marketing expenses.

CODINGLINE CORNER
Query: New MRSA Diagnosis and Treatment

I performed an incision and drainage of a skin abscess (CPT 10060) and the patient comes back within the global period for follow-up. I had taken a culture that now identifies MRSA which alters our original treatment plan. Can I charge for another E/M code within that original global period for this change in plan?

Codingline Archive

Response: I think it is customary when a culture is taken that the results are reviewed and antibiotic therapy adjusted accordingly. In my opinion, this is part of the global allowance. If you feel that things have changed, you need to document what “alters our original treatment plan” means. Did you simply add a second antibiotic? Did you have to go back in surgically? Did the condition worsen and you had to consult with an infectious disease specialist and change from oral antibiotics to intravenous therapy? You have to document why your scenario is different and why it should not be included in the global period.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1A
From: Steven Finer, DPM, Bret Ribotsky, DPM

Good Feet orthotics have been sold on cruise ships for a number of years. They are pieces of plastic sold in threes as they “correct your arches.” They have a customer stand and they pull the person forward, then do it with the device in place and there is no movement. Usually the audience is amazed. The usual price is $1,000. I pointed out to one of the sales people that you could perform that trick with a stale piece of rye bread under a heel.

Steven Finer, DPM, Philadelphia, PA

What we learned from this incredible back-and-forth discussion is that people are willing to pay over $1,000 for orthotics. This begs the question why don’t those in this profession collectively charge at least that price. A few years ago, one of my children was living in Manhattan and needed a pair of orthotics. I sent them to a well-respected DPM  for those devices. He offered me a 50% discount and it only cost me $1,000; therefore, his normal price was $2,000 for a pair of orthotics.

Maybe this is a wake-up call that we should all be hearing - if OTC devices are going for over $1,000, then custom devices should go for quite a bit more. Has the market spoken?

Bret Ribotsky, DPM, Fort Lauderdale, FL

RESPONSES/COMMENTS (NON-CLINICAL) - PART
From: Brian Kashan, DPM

Kudos to Dr. Stewart for taking the time and action to try to counter and educate patients on the scam otherwise known as "The Good Feet Store". They have been inundating the media with advertisements for an over-the-counter device that is more useful for swatting flies than supporting the foot. They use actors and convincing scripts that are so effective and well done that I can see why patients to go there. The problem is the products are ineffective and the advertising is misleading.

Dr. Stewart did an excellent job in making his short video to demonstrate the differences between what podiatrists can do and what over-the-counter devices don't do. In my opinion, educating the public on what podiatry does is one of the keys to our existence. There will constantly be other professions and other products that come up to take business away from us. A shortfall of our professional organizations, in my opinion, is not doing enough to educate the public on these matters. An individual podiatrist should not have to do this because our professional societies need to be proactive.

This does not negate the responsibility that we have as individual practitioners in educating our patients and internally promoting our services. Our office recently sent an email to all of our patients about this very issue to try to educate them so that they don't become victims of this scam and waste thousands of dollars on ineffective products. I believe that every podiatrist should be making every effort possible to educate their patients on the services they provide.

RESPONSES/COMMENTS (NON-CLINICAL) -PART 2
From: Steven Kravitz, DPM

Dr. Sherman is correct in all that he said, except for the statement, "they are not limiting home study CME through the end of 2022." Actually, it is that at the end of 2022 RENEWAL which already passed in March 2022.

Richard Orlandi, DPM, Babylon, NY

I want to thank Dr. Sherman for his comments yesterday regarding my letter on the above topic. He indicated that North Carolina does not allow virtual conferences for CME for state licensure. That is not the case, and currently all 25 credits required by the state can be provided through virtual conferences. That is up for reconsideration in June 2023.

The misstatement in my post on CME requirements for virtual conferences for licensure renewal for Florida and the incorrect statement by Dr. Sherman for North Carolina indicates the lack of clarity in these regulations. It emphasizes the need for podiatrists, when in doubt, to check their state society and the state board for CME requirements for state licensure through virtual educational meetings that are in place at that time. These regulations change and what may have been in place yesterday may not be in place currently.

Steven Kravitz, DPM, Winston Salem, NC
