PODIATRISTS IN THE COMMUNITY - PART 1
IL Podiatrist Discusses Foot Problems With Seniors

Dr. Grant Gonzalez of the Foot & Ankle Center of Illinois recently met with senior residents and staff at the Eastland Retirement Community facility, an independent living center nestled on the family-owned farmland of Jane Moose in Auburn, IL. Meeting with residents in an informal setting was a perfect opportunity to explain some of the most common foot ailments that seniors experience and to offer simple solutions.

Dr. Grant Gonzalez

According to Dr. Gonzalez, “The senior population is expanding and I feel it is part of our role, as podiatrists, to help them get the most out of their future years. We understand that staying healthy is a challenge as you age, but a lot of foot problems and related injuries can be prevented through patient education and awareness forums," says  Gonzalez.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1
From: Elliot Udell, DPM

We utilize nerve conduction studies in our practice. In some cases, the results have told me nothing more than I knew before. In other cases, the information was very helpful in making proper and timely diagnoses and referrals. One case that vividly stands out in my mind was a patient who was not responding to therapy for bilateral heel pain. We had a mobile NCS service come in and perform the test. It pointed to a radiculopathy. I subsequently referred the patient to a back surgeon who, with the aid of MRI, discovered that the patient had a spinal fracture with fragments pressing on nerves which innervate the lower extremities. After surgical reduction, her feet felt better.

The study with the EMG which is done by neurologists and physiatrists can sometimes yield even more information. We have sent patients from time to time to have this test done; however, this test involves placing needles into the patient's legs, and many of these patients emerge from the study not being "happy campers" or non-adherent if they know what the test entails.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1BA
From: Stanley Goldstein, DPM, David S. Wolf, DPM

I have been using MD On-Line as a billing service and have been pleased. They are now part of Ability Network in IL.

Stanley Goldstein, DPM, Brooklyn, NY

I highly recommend Active Management (Activemds.com) for outsourcing your billing and collections. They have been a real asset to our practice, not only in billing and collections, but in their personal involvement in our practice management for over 10 years.

RESPONSES/COMMENTS (NON-CLINICAL) - PART 1B
From: Joe DeTrano

As someone who sold billing services and software to DPMs for almost 20 years but no longer has a horse in the race, I can offer this advice when searching  any firm:

-Make sure they handle podiatry and give you a podiatrist in your state as a reference. Podiatry billing is tricky and different. Get a firm that knows it.

-If the rate is too good to be true, it probably is.....You get what you pay for!

-Set Up a monthly meeting to go over A/R, what is outstanding, ...

Editor's note: Joe DeTrano's extended-length letter can be read here.
