PODIATRISTS IN THE NEWS |
Stem Cells are the Foundation of Regenerative Medicine
According to Conway McLean, DPM, "Stem cells are unique in that they are able to differentiate into various types of specialized cells. Once thought to be absent in the body by the time one reaches adulthood, they can be obtained from various sources including, fortunately, adult tissues. This fact is critically important in many regenerative techniques. Amniotic tissue has been used to great effect for wound healing. But another product in this category is injectables, used for joint and soft tissue problems, even those of cartilage. The material injected is obtained from a planned C-section and is loaded with growth factors. These are powerful signaling molecules which recruit stem cells to the region."
|
Dr. Conway McLean |
"Typically, a single injection of this substance into an arthritic joint acts to reduce pain and stiffness, consequently improving function. Amazingly, there is evidence this technology may act to stimulate the growth of actual joint cartilage, an ability heretofore unknown. And this leads to a key component of this discussion of healed tissue. It is not asymptomatic because of the presence of a medication but because the tissues are healthier," says Dr. McLean.

NY Podiatrists Publish Article in Journal of Perinatal Medicine
Father and son Rock G Positano, DPM, MPH and Rock CJay Positano, DPM, MPH of the Non-surgical Foot and Ankle Center at Hospital for Special Surgery in New York recently published an article in the Journal of Perinatal Medicine titled, "Common foot and ankle disorders in pregnancy: The role of diagnostic ultrasound".
|
(L-R) Drs. Rock G and Rock CJay Positano |
Diagnostic ultrasound provides a safe and reliable method to assess the extent of bone, ligament, and tendon trauma, as well as circulatory and clotting derangements in the pregnant patient without the use of potentially dangerous exposure of x-rays to the developing fetus. This article is published in one of the most prestigious Ob/Gyn journals both nationally and internationally and demonstrates how podiatry has integrated and contributed to other disciplines such as diagnostic imaging radiology and perinatology.

PRACTICE MANAGEMENT TIP OF THE DAY |
8 Tips for Avoiding Employee Handbook Crises
Many medical practices have an employee handbook, or employee policy handout, but fail to update them on a regular basis. There have been a large number of changes to employment law over the past couple of years. Here are some items you should be sure to include:
6. Benefits Have you reviewed your benefits to make sure they comply with federal and state laws? After the U.S. Supreme Court ruled that same sex marriages are legal in all states, many employers did not look at the wording of their benefits contracts. Several cases were brought against employers for having an insurance policy that did not cover same sex spouses.
If you do not have enough employees to be impacted by the Family Medical Leave Act, leave it out of your handbook and state you are not required to offer FMLA benefits. If you do have to comply with the FMLA, make sure your employees are told who to contact and that they must fill out the proper forms in order to be eligible. Look at items like bereavement leave and make sure you have taken into consideration same sex spouses.

QUERIES (DME) |
Query: Can a Nursing Home Podiatrist be a DME Provider?
Can a podiatrist with a nursing home practice be a DME provider of diabetic shoes and not have an office where patients are being seen? Is there an agency that can help in the application process.
Ronald K Norman, DPM, Beavercreek, OH

RESPONSES/COMMENTS (MEDICARE ADVANTAGE) |
From: Paul Kesselman, DPM
Dr. Richie is 100% accurate in his posting. To say that there have been many posts on this issue both here and in many medical publications nationwide would be an understatement. Unfortunately recent court cases have heavily favored the insurance industry and there is almost no sign of the fraud referred to as Medicare "Advantage" going away. This despite recent stories which have appeared in the news illustrating the amount of fraud both large and small Medicare Part C plans have committed.
The government has made it clear they want to pay a set fee to insure seniors and entirely transfer the financial risk to the insurance carriers. This is despite the fact that studies (aside from the fraud on data mining) have stated: Privatized senior care offered by Medicare Advantage insurance plans has led to higher costs for Medicare and is a drain on the Medicare trust fund.
There are several ways to kill the fraud: One is to outlaw insurance sponsored political campaigns; second is for medical providers to stop providing data which they are not legally obligated to provide; and the third is to decide whether or not you can afford to provide care to patients with reimbursement at 50-75% of Medicare rates. The latter, of course, have decreased about 30% adjusted for inflation over the last decade. Perhaps providers are both part of the problem and the solution.
Paul Kesselman, DPM, Oceanside, NY

RESPONSES/COMMENTS (CODING AND BILLING) |
From: Michael M. Rosenblatt, DPM
Cheers and congratulations to the PM News subscriber who passed his Medicare RFC audit! I’m sure that doctor’s chart notes very carefully delineated medical necessity and a careful description of how the patients demonstrated need, and exactly the services the doctor performed.
The doctor also had RECORDS of active billings for ancillary services that were not covered by Medicare. Considering this doctor’s knowledgeable experience, I hope that the successful DPM provides lectures to new podiatrists coming on the scene locally.
It would be both a tragedy and a shame for this NOT to be transmitted to other DPMs in his/her community. By doing this, the DPM may very well be saving lives in our practice community.
Michael M. Rosenblatt, DPM (Retired), Henderson, NV
