CT Podiatrist Discusses Sever's Disease
According to Robert Weiss, DPM, "One of the more common growth plate injuries is Sever’s disease. As the child runs, it puts more stress on the Achilles tendon behind the heel bone where it inserts and creates a constant pulling of the heel. This, in turn, leads to inflammation of the actual growth plate of the back of the heel bone."

"Treatment is directed toward the cause and effect of the condition, which is usually poor footgear with the lack of shock abortion, stretching and strengthening exercises, or conditioning programs. Ice therapy and biomechanical orthotics shoe inserts will control the motion and supply cushioning for increased shock absorption. Parents should look for tell-tale signs to help prevent child growth plate injuries, and thereby avoid adulthood problems," says Dr. Weiss.

Swimming Legend, Gary Hall contacted Ben Pearl, DPM for technical input on his upcoming swim coaching book Fundamentals of Fast Swimming. Hall, a former Olympic swimmer, will expand on "Doc" James Counsilman's seminal swim coaching manual. Dr. Pearl provided information on the difference between ankle and subtalar joint ranges of motion.

RE: Black Diabetics Lose Limbs at Triple the Rate of Others

Clinical Endocrinology News reported (August 31, 2020) that Black Americans with diabetes lose limbs at a rate triple that of others. The American Diabetes Association (ADA) has promoted an initiative to prevent unnecessary amputations as part of an unprecedented campaign to reduce racial disparities in diabetes care. The CEO of the ADA reports, “Every four minutes, someone is losing a limb from diabetes.” Obstacles to equitable care for diabetic patients at risk of amputation include the government’s decision not to endorse screening at-risk patients for vascular disease in the legs, to the inadequate incentives for certain specialists to move to underserved areas, to the health system’s failure to consider limb-saving options before surgeons apply a blade.
The ADA’s Health Equity Now campaign addresses the cost of diabetes care, nutrition, discrimination, and more. It was motivated by the racial health disparities that have been exposed by COVID-19, which has hit Black Americans with diabetes particularly hard. As part of the project, the association has built a Health Equity Bill of Rights, asserting that all diabetes patients are entitled to affordable drugs, healthy food, and the latest medical advances. The right to avoid preventable amputations is the only complication of uncontrolled diabetes that is included in the list. The ADA is encouraging practitioners, patients, and members of the public to ask their governors to support the project.

Those organizations include The International Foot and Ankle Federation, The New York College of Podiatric Medicine, Dermfoot, The Ohio, Missouri, Minnesota, Kansas, Colorado, Tennessee, Iowa, Nebraska, Georgia, and California Podiatric Medical Associations, and ACLES, as well as broadcasting our own events. Some of these events are one hour weekday evening events, others are half-day events on weekend mornings, and others are multi-day events. We have had events with starting times that align with Eastern, Central, Mountain, and Pacific time zones.

Dr. Barney, the short answer is that you were misinformed by the national coder you spoke with. The long answer is that there are certain diagnosis codes used with CPT codes 11720-11721, (as well as other routine foot care CPT codes) that must have a Q7, Q8, or Q9 modifier appended to them, to indicate circulatory issues. The class findings must be documented in the treatment note. Your MAC will have an LCD on their website for Routine Foot Care or Nail Debridement, or both, and there you will find the pertinent information on the use of Q modifiers.
