4 Myths about Using Electronic Funds Transfer in Healthcare - Part 3

Myth 2: Getting funds via ACH is costly and requires special equipment.
 
Actually, the only item required to receive electronic fund transfers via ACH is a bank account, and signing up costs nothing. What’s more, electronic fund transfers via ACH are more cost- effective than many other forms of payment, including checks and virtual cards. Providers see an average savings of $3.04 for each check converted to an ACH payment.
 
There’s even more time and cost savings when electronic fund transfer is used in combination with electronic remittance advices (ERA), which enable the automated reconciliation of fund transfers, as well as automated posting to the provider’s billing system. Providers can save $7.21 per payment when they switch from checks and explanations of benefits to EFT and ERA. Some practices may need to upgrade their practice management or accounting software to accommodate automated posting, but it’s not necessary if a provider simply wants to receive EFTs.

Unless something grossly changed over the past few months, the whole question of whether or not any % injections work on neuromas is based on the availability and cost of dehydrated alcohol. The last time I checked, I could find no suppliers of pharmaceutical grade dehydrated alcohol that made this treatment economically viable. USP ethyl alcohol IS cheap and available, but the suppliers will not sell it for medical use. The few suppliers I found that DO sell pharmaceutical grade dehydrated ethyl alcohol were priced so ridiculously high that you would lose money offering this as a treatment. Where are all of you getting your meds and at what cost?

After reading several anecdotal responses to Dr. Bregman's post, it occurred to me that the ONE compelling question to ask is to what extent we, as clinicians, should utilize the anecdotal successful experience claims of our colleagues and forefathers relative to our own medical decision-making process? In my humble opinion, this is an extremely important question our younger, less experienced colleagues should ask, though it transcends our entire profession. You are all receiving incredible training, yet lack the years of repeated patient exposure to know what truly does and doesn't work in clinical practice. As such, while I recommend remaining open-minded, I also implore each of my colleagues to use significant grains of salt with respect to medical opinions posted on this site. 
 
Dr. Block is wonderful because of the openness of his forum, but that is potentially dangerous too. So, I implore you all to consider what the evidence-based medical research demonstrates for a given treatment that you are considering and allow that to serve as your...

I am glad people are getting great response to the alcohol injections, but none of these doctors who state their great results have data to back them up. It's anecdotal. Perhaps, as I suggested, just try Marcaine by itself. I bet you get the same results as with the 4% alcohol. I used to use it myself so I can say I did not have the success that people claim to have.
 
I would really like for those who use the alcohol to try injections without it on the next 5 patients (with 2-4 milligrams of Decadron, if you like), and you will see the same results. Think about it - why does it take many injections for it to work? The small amount of alcohol will not cumulatively kill the nerve. It's too small a dosage...not to mention that diluting it in that bottle of Marcaine takes away a lot of the dehydration process.

In the context of this academic discussion, none of us should diminish the honor and respect we all have for you and many of our other esteemed colleagues who  keep biomechancics and foot orthotics on the front burner at our conventions. When I went to school at NYCPM, back in the 1970s, there were conflicting opinions between the late great Drs. Schuster and Dr. Root. Both men taught all over and had their myriads of followers as well as labs that produced their orthotics. These pioneers of biomechanics were respected more because of who they were rather than on well written academic papers published akin to what Wrobel and his team wrote.
 
As Dr. Kesselman points out, the  opinions of podiatrists with high stature is not enough in this century to convince insurance companies to pay for orthotics. These people want to see current, well-written research papers. Unless we continue to invest in these studies, we cannot and should not expect anyone either in the insurance industry, orthopedics, or even well educated patients to believe that when we cast or scan patients in our offices for custom foot orthotics, we are not doing any more for them than what Dr. Scholls does when it has people stand on scanners at Walmart and take home pre-fabs.

I wish to do so in a broader context rather than just Dr. Wrobel’s article. By the way, James Wrobel, an acquaintance of mine, is an excellent podiatric researcher. 
 
ACFAOM believes that biomechanics and medicine are the cornerstones of contemporary podiatric practice. With that in mind, I wish to underscore the educational avenues available to practitioners and those still in training available from not only ACFAOM, but...

All of Dr. Udell's comments are correct, and his philosophical stance toward the purveyors of theoretical concepts is a cause for us to have natural skeptical attitudes. Science and art definitely coexist in the field of medicine, and to a great degree in the implementation of orthotic design. Where I disagree with Dr. Udell is the notion that this intermingling is a bad thing. Of course, differing concepts and opinions will surface and that's completely natural.

I had the opportunity to personally attend and testify (on behalf of APMA) at the hearings Medicare held in Baltimore on August 26, 2015 on several draft Medicare policies. This included two policies which will no doubt directly affect podiatrists' ability to provide necessary DME to their patients. 
 
One is on the ability for podiatrists to perform a required functional assessment on patients who require toe fillers (L5000-L5020). The current draft policy does not include DPMs as licensed certified medical professionals in the group who can perform such assessments.
