INTERNATIONAL PODIATRISTS IN THE COMMUNITY
UK Hero Soldier Goes from Military to Podiatry

Could you imagine your whole life changing after a conversation with a stranger? For Felix Lopez, 47, a chance encounter in the rain led him to an entirely new career. “I’d just finished a session at the barracks gym but it was pouring down, so I was sheltering until the rain stopped and got chatting to a U.S .serviceman who was waiting too.” When Felix mentioned he was leaving the parachute regiment, his new acquaintance asked about his plans.

Felix Lopez

“I said that some of my friends were doing physiotherapy and that I had always been interested in how things move. He said, ‘you want to be a podiatrist?’ I know it sounds ridiculous, because I’d never even heard of podiatry, but I decided to look into it. From there, I got into university, finishing my podiatry degree in 2001.” While working as a podiatrist in Manchester for NHS Trafford, Lopez took post-graduate courses in the surgical management of foot and ankle problems. “I finally completed my studies in 2016,” says Felix, who is now a consultant podiatric surgeon at the Queen Victoria Memorial Hospital in Kent.

PRACTICE MANAGEMENT TIP OF THE DAY
Considerations for Integrating Medical Transcriptions into Your Practice - Part 1

If you too are looking at leveraging transcription to streamline the operations of your practice, here are a few crucial considerations that will help you make an informed decision:

1) Consider your Workflow - When opting for transcription services, you can’t really follow a one-size-fits-all approach. Every practice is going to be different from the other.

Your workflow—from the preferences of your staff, your specialty, to your practice's specific needs—plays a vital role in influencing what type of transcription service will be most effective for your practice and to what extent should you be integrating it within your operations. It all depends upon how your practice functions, the volume of your documentation, and what your practice can best adapt to.

He received a degree of Doctor of Podiatric Medicine from Ohio College of Podiatric Medicine, in which he graduated cum laude. QUERIES (NON-CLINICAL)
Query: State Board Limitations for Online CME Credits

Why don't all 50 State Boards allow 100% virtual CMEs for the next licensing renewal period, since virtually all CMEs are now virtual?

RESPONSES/COMMENTS (DME)
From: Paul Kesselman, DPM

Each DME MAC has a provider portal where Same or Similar can be researched. My CGS is the portal for DME MAC Region B and C. Go to either of those DME MAC homepages and look for the MYCGS link. There are several hurdles in signing up. I’ve been working with their portal managers to make signing up and maintaining one’s subscription less frustrating.

Once on the MY CGS Portal, the claims history button provides a way to input previous “L“ codes and to a search history. There continue to be nuances with the portal which need updating and ...

RESPONSES/COMMENTS (CLINICAL) - PART 1A
From: Elliot Udell, DPM

Thank you Dr. Jacobs for giving us a link to an article which shows an association between arthralgias and myalgias and COVID-19. It is advisable that we all read this paper. What is different, however, between the query I posted and what was discussed in the paper is the timing. The patients who are presenting to us with painful soft tissue pathology are presenting six months after being symptom free of COVID-19. It is now the end of August and the majority of the patients I am seeing with these symptoms had COVID-19 in early March when the disease was rampant in our geographic area.

I am trying to gather information from all of our colleagues and the data gathered might be "meat" for another paper.

RESPONSES/COMMENTS (CLINICAL) - PART 1B
From: Dennis Shavelson, DPM

My anecdotal experience is that COVID-19 affects endothelial cells by creating an hypoxia and Dr. Jacobs article supports my stochastic hypothesis. In Cuomo and Coppola, et al.'s article, pro-inflammatory cytokines activate hypoxia-inducible factor 3α via epigenetic changes in mesenchymal stromal/stem cells, they determined that “hypoxia activates the expression of nuclear factor-kappa B (NF-κB), which in turn stimulates the release of pro-inflammatory cytokines, as interleukin 6 (IL6)” as discussed in Allen’s referenced article.

RESPONSES/COMMENTS (NON-CLINICAL)
From: Steven D Epstein, DPM

I prefer them during the day as they are. I admittedly don't have a super-busy practice so I can schedule the time out, but the evening times suggested by Dr. Kass would conflict with my personal, non-podiatry time. I did not get into this profession to eat, sleep, and drink it 24/7. On the other hand, they should all be on demand.

YOU CAN'T MAKE THESE THINGS UP

RE: Outrageous Shoe of the Day

Who knew a 2,000 year old shoe made of grass could last?

Here is an ancient Roman shoe from circa 79 CE made out of esparto grass (Boscoreale Antiquarium, Italy).
