NJ Podiatrist Discusses Tinea Pedis
Athlete's foot—also known as tinea pedis—is a fungal infection that's at times painful, persistent, and highly contagious. "Fungal organisms like hot, wet, moist places, which makes shoes and socks the perfect breeding ground for athlete's foot," explains Alan Bass, DPM, an American Podiatric Medical Association podiatrist.

Luckily, most cases of athlete's foot are treatable with an over-the-counter cream, spray, or powder. Dr. Bass recommends a quick-drying antifungal spray or a powder if you don't have time to wait before putting on your socks and shoes, noting that messier, topical ointments work best when you are able to let your feet air out. Most importantly, Dr. Bass says, it's vital to see a podiatrist if the problem doesn't begin to clear within a couple of weeks of treating it with an OTC product.

PA Reviews Foot Surgery Decisions
Mike Downey, DPM reviewed his surgical decision-making process on Fit Foot U with Ben Pearl, DPM. Dr. Downey said that foot surgery must include a basic gait exam to determine relationships with the whole lower extremity. He added that the patient’s goals are critical in procedure selection.

Downey emphasized going from the known to the unknown to identify tissue planes with revision surgery. He encouraged foot and ankle surgeons to use 3.5x loupes like hand and neurosurgeons do. When asked what podiatrists should do with the vast array of retail foot solutions, he said that podiatrists have to educate the public to differentiate why podiatric surgeons' clinical expertise is important.

5 Best Practices for Cleaning a Medical Office
The CDC and OSHA have set industry standards for medical office cleaning. These research-based procedures ensure staff and patient safety, lower risks of healthcare associated infections and reduce cross-contamination. Here are the key areas for medical office staff to focus on when cleaning the clinic.
3. Clean examining rooms
All areas of examining rooms need a thorough cleaning after each use. These areas are susceptible to cross-contamination between doctors, nurses, and patients; therefore, it needs extra attention when cleaning. Clean the lights, countertops, and treatment chair with the proper disinfectants.
After each patient leaves, wipe down all doorknobs, cabinets, light switches, and other high-touch zones. These areas are extremely susceptible to germs as they often go unnoticed when cleaning. Keeping these areas disinfected will give patients peace of mind and allows staff to keep them safe from potential accidental infections.

Query: Ulceration Treatment In the Global Period
I have a patient with a chronic diabetic ulcer, which is now infected and has confirmed osteomyelitis. CPT 28810 was performed at the right hallux and first metatarsal. The ulceration that was present before was not directly incorporated in the surgery and is still there post-operatively. Can we bill for the subsequent treatment of that ulcer during the post-operative period or does it fall under global?

Response: The key, as always, is documentation. What was meant by “not directly incorporated in the surgery”? If the patient's problem was caused by the ulcer which then went onto osteomyelitis, then the ulcer treatment was “incorporated” in the surgery. I assume the underlying bone was resected. Just because the ulcer was not ellipsed does not make it a separate problem.
That said, what specific ulcer care are you rendering beyond possible dressing changes and some local wound debridement which is being done anyway? Be very clear in your notes as to what you are doing and why you believe this care you are rendering is beyond what would be commonly expected after a toe amputation/ulcer scenario. This will determine if it should be billed… and reimbursed.

Query: Source for Amniotic Fluid Injections for Plantar Fascitiis
I have a patient that I am treating for plantar fascitiis who would like to try amniotic fluid allograft injections for his condition. It is my understanding that insurance usually will not pay for this treatment, and the patient would like to pay cash even though the cost may be in the thousands. My question is: Does anybody have a source that I can contact for these injections?

I have to agree with Dr. Rosenblatt and disagree with Dr. Chaskin's use of the word "fiduciary". Dr. Rosenblatt's reference to his experiences rang a familiar ear to me as I too was headed towards a musical career. However, I too realized that either you were one of tens of thousands of gifted musicians who would be working or traveling when your families were home, poor, or if you were one in a million who would become famous. There are avocations such as music and art where most people do not become famous or financially secure. And there are vocations where most people do become financially secure. Thus, I made the choice to pursue a career in healthcare. That is the key word, I made a choice.
Dr. Rosenblatt is also absolutely correct, no institution can guarantee you anything in life and...

Comparing podiatry to music does not address the problem of high student debt and potentially low income. I graduated NYCPM 42 years ago. When I went into private practice, podiatry was a very lucrative field. We worked hard but we were paid very well. And our student debt was not even close to overwhelming or insurmountable. The problem is that in the early ‘90s, insurance companies created managed care (managed fees) and our then leadership pushed very hard to get podiatry covered under these plans.
Now, this many years later, tuitions have risen dramatically, managed care’s fees are a fraction of what they once were, and podiatrists still do not have parity with MDs and DOs. To run a practice, you must see an enormous amount of patients, which means...
