Don't Ignore Leg Pain: NY Podiatrist

Leg pain should not be ignored, says podiatrist Dr. Kelly Powers. "Specifically, if it is posterior and unilateral. It could be a sign of deep vein thrombosis, which means a blood clot. Blood clots can be stationary, but if they move, they can travel to the heart, lungs, or brain and cause a stroke or even death."

HISTORIC PODIATRISTS REMEMBERED
Late WV Podiatrist to be Inducted into Huntington Wall of Fame
 
The City of Huntington Foundation has selected five new inductees for the Greater Huntington Wall of Fame for 2015. This year's inductees are Troy F. Brown, Nancy J. Carter, Dr. H. Darrel Darby, David A. Glick, and Richard Michael Kirtner. The late H. Darrel Darby, DPM, was born in Quinwood, West Virginia. He worked as a coal miner prior to attending Anderson University Indiana, Marshall University in Huntington, and the Ohio College of Podiatric Medicine.

In 1999, Dr. Darby was named one of America's Most Influential Podiatrists by Podiatry Management Magazine. In 2004, he was awarded the Distinguished Service Citation from the American Podiatric Medical Association, and in 2002, The Herald-Dispatch honored Darby as its Business Innovator of the Year. Darby developed his international company DARCO International in 1987.

Jennifer White, Esq: From what I see, electronic medical records are a mess... the whole system. EHR is supposed to be a tool that helps, that streamlines a business and makes it more efficient. I have not found that to be true. EHR has created headaches that have nothing to do with care. Often, the basis of a lawsuit has to do with the charting issues. Prior to EHR, the way doctors and patients communicated was different. There are issues with “autofill” items. There are also times when staff members put something in the records inadvertently, or do not remove something that should be removed between visits. Physicians need to be on guard with their EHRs.

Prior to performing a biopsy, see if the lesion blanches when compressed to rule out if it is possibly vascular in origin.

With regard to the dermatological lesion that as of yet has not been biopsied, "if not now, then when?" Most lawsuits that I read about deal with a time factor. If you don't know what the lesion is, how do you think you will find out? What is the hesitation with doing a biopsy that is so minimally invasive and will yield the answer you are seeking? Unless, there is some contraindication that was not shared, the lesion should be biopsied.

I'm not sure which version of Dragon you are using, but I have been using Dragon Premium 13 in Traknet 3.0 for 6 months and, for me, it works great - much better than it did in 2.0. I don't use the dictation box either. Maybe there are some settings that you can change to get it to work. I would be happy to try to help you troubleshoot.

RE: Decrease in New Patient Visits

Unfortunately, this is a problem that in all likelihood exists to some extent in many offices and often is "falling under the radar." Not knowing how many providers are in your office makes it difficult for me to recommend what solution suits you. Here are some options:
1. In a larger group, a call center would take that responsibility out of the hands of the front office staff. I am in a larger group. All of our partners noticed an uptick in patient scheduling. The call center takes the worry out of ulterior motivations.
2. Use VoIP phone system. Calls can be recorded and sampled at a later time. However, I would let your employees know that you are recording. That will act as a deterrent. Also, check with state law on recording phone calls.
3. Use secret shoppers. Pick a day or several days to have several secret shoppers call posing as new patients. If turned away, have the secret shopper get the name of the staff member they spoke to, and the date and time the phone call took place. Check it against your schedule.
4. Briefly interview your new patients. Ask them if they easily made an appointment. What was the wait time to make that appointment?
5. Lastly, make your employees aware of your concerns. Compare new patient stats from last year to this year and ask them for their input. The Information could prove valuable and it puts them on notice.

Dr. Goldsmith correctly points out, in his recent article, the documentation needed to medically justify and bill for the U.S. guided injections into a joint (CPT 20604, 20606). All that they pay you is an approximate additional $30. Is it worth the risk of loss of time needed to get together medical justification and chart info (for a medical claim review) for such a small additional payment?  Not to me – I don’t even list these CPT codes on my office superbill.  
 
I’ll reserve my limited use of U.S. guided injections for deep bursae and cysts and/or neuromas – CPT 76942, where it is worth my time and effort and the medical justification is easily apparent. It is bad enough that they only reimburse a limited U.S. exam -CPT 76882 (.i.e., for plantar fasciitis) at less than $36. Please don’t insult me with less than $30 additional for adding U.S. to a joint injection (– you will get a larger reimbursement for "just looking" at the joint)!
