MI Podiatrist Discusses the Confusing State of Foot Supports - Part 2
 
According to Conway McLean, DPM, "Like eyeglasses, foot orthotics don’t alter anatomy. Instead, they change your biomechanics and should lead to healthier function. In a way, foot orthotics bring the ground up to the foot rather than gravity pulling the body, along with the arch, downward. A common example is a foot type which experiences excessive downward motion of the arch. Reducing this can prevent the damaging consequences of this pathologic foot motion termed pronation."

"If someone has an inkling that some discomfort may be related to their foot type or biomechanics, it’s reasonable to try an off-the-shelf product to see if it provides any benefits. If it does, a prescription orthotic will do it better. Different feet need different types of support. A physician nuanced in biomechanics is going to do the most detailed and specific analysis, which generally results in the most accurate evaluation of your biomechanics. This is vital to creating the most comfortable and effective foot orthotic," says Dr. McLean.

Lidocaine-Related Deaths and Poisonings Surge Over Past Decade
 
A recent study highlights a significant rise in lidocaine-related deaths and poisonings over the past decade, contrasting with a decline in adverse events from other local anesthetics. From 2016 to 2021, lidocaine poisoning reports increased by over 50%, while non-lidocaine poisonings decreased by 50%. Nearly all lidocaine deaths involved doses exceeding the recommended maximum of 500 mg, with some cases reporting doses as high as 48,000 mg. The study, published in BMJ Regional Anesthesia & Pain Medicine, underscores the need for improved administrative guidance, awareness of high-dose risks, and strategies to prevent and manage lidocaine toxicity. The research utilized data from the American Association of Poison Control Centers’ National Poison Data System, analyzing trends from 1983 to 2022.
 
The Takeaway: Enhanced administrative guidance, awareness of high-dose risks, and strategies are crucial to curb lidocaine toxicity.

1) This is way too small a study. Also, patients and doctors do not want improvement, we need resolution and hope for no recurrence.
2) This drug is very expensive to purchase.
3) Small sample group did not specify how many were foot warts (study said extremity warts)
4) Intralesional Bleomycin (blenoxane) much less costly, and well researched and well documented as an off-label wart treatment with results over the past 50 years.
 
If your wart treatments are failing, or you have patients with resistant or recalcitrant warts, under the right conditions, I suggest you try using Bleomycin before Cidofovir. Learn how to use it first; as with any medical or surgical or injection treatment, procedure, or technique, there certainly is a learning curve.

In his recent post about his concerns around the WISeR model, Dr. Borreggine notes, “Nothing from the APMA.” I’d like to correct this inaccuracy. Not only has APMA shared its concerns in a July 20 comment letter to CMS, but there have been multiple communications from APMA to members about the WISeR model through Weekly Focus emails, news articles on the home page of the APMA site, and a video from APMA’s health policy team.
 
In the comment letter sent to CMS in July, APMA raised the same issues around timely care for DFUs and other lower extremity wounds that Dr. Borreggine pointed out. I strongly encourage members to read their weekly email publications from APMA and check out the Latest News section on the home page of the website. There is important information in these communications about a wide variety of policy and payer issues, as well as critical calls to action. The more we work together to speak with one voice to CMS and other payers, the more traction our small profession will gain.

Regarding the question as to why CMS has acted so quickly with wound regulations, make no mistake, when The New York Times publishes an article that listed podiatry among the abusers, that got people’s attention. There’s a reason that the phrase “pigs get fat and hogs get slaughtered” applies here. The phrase “Drinking the Kool-Aid" does not as that would imply that those who abused, did not know, and if they truly believe that, they are only fooling themselves.

Let me make some corrections to my friend and colleague Joseph Borregine's comments on the WISer program. This is a voluntary program which is available in only six states - New Jersey, Ohio, Oklahoma, Texas, Arizona, and Washington. It is open to any provider practitioner, including DPMs who provide a subset of specific procedures. It is based on and does not usurp specific LCDs which have jurisdiction over those procedures. It is not biased for or against any specialist.
 
Dr. Borreggine specifically stated that dermatologists were exempt from this program. That is false! Rather, it should have stated that post Moh's surgery grafting is not subject to the WISeR program. The reason is because...

A few people have written in that they were part of ABLES, which no longer exists. The name withheld person states they did an RPR—PSR program, and CPME rules do not allow them to get boarded by ABPM. 
 
There is a real problem with this profession. It is rooted in non-sensical rules that only serve to hurt rather than help those who laid the foundation for the profession. Like name withheld, I did a two-year residency. I received the same certification: RPR/PSR-12. My current board is ABPM. If I can be boarded by ABPM, then someone with the same training credentials should be allowed to be grandfathered in, case closed. Stop, the B.S. 
 
Enough is enough. Whoever made these rules should be put in the Podiatric Hall of Shame. I call on ABPM, CPME, and whatever other acronyms are needed to rectify the situation and let people practice their profession. When will the insanity stop?
