RELEVANT RESEARCH
Intralesional Cidofovir Promising for Treating Refractory Warts

A case series of children aged 7-17 years found that treatment with intralesional cidofovir was effective in healing recalcitrant warts, with several children experiencing complete resolution. The results were presented in a poster session at the Society for Pediatric Dermatology (SPD) 2025 Annual Meeting. “Treatment was well tolerated, and we were able to control pain,” study author Vivian Iloabuchi, BS, a third-year medical student at the Mayo Clinic Alix School of Medicine in Scottsdale, Arizona, told Medscape Medical News.

The results reported at the SPD meeting were from a case series of seven children aged 7-17 years with three or more cutaneous warts located on the extremities that had not responded to salicylic acid, cryotherapy, Candida antigen injections, oral cimetidine, and/or pulsed dye laser. Cidofovir 15 mg/mL was injected into each wart, at doses ranging from 2 mg to 13.5 mg, with a total dose per session from 4.5 mg to 30 mg.

8. Coach your team often

Staff who understand payer rules, coding changes, and follow-up protocols make fewer mistakes and collect more money. Quarterly “lunch-and-learns” on new payer policies pay for themselves, says Medical Economics in its roundup of tips to boost billing and collections.

RESPONSES/COMMENTS (BOARD CERTIFICATION)
From: Name Withheld

I also had been certified by the American Board of Lower Extremity Surgery (ABLES) for a long time until it was acquired by the ABMSP. The new board grants a 2-year certification verification period, after which one has to apply from scratch and sit for the exam. In light of this acquisition, I strongly believe the former ABLES diplomates should be able to re-certify with the new board like any other ABMSP member.

As an alternative solution, I did try to apply for the ABPM (American Board of Podiatric Medicine) certification, but was told by them that the CPME rules do not allow them to accept me; the residency program that I did 27 years ago, a CPME approved 2-year RPR/PSR-12 does not qualify me to apply for the ABPM certification. Back then, there weren't that many surgical residency programs available, and RPR/PSR-12 was a sought-after program. Thus, after 25 years of practice, I cannot apply to ABPM because of the restrictive CPME. I know several other podiatrists who did the residency program at my hospital, and who are in a similar situation.

I wonder what the CPME and the members of the profession have to say about this?

RESPONSES/COMMENTS (WOUND CARE)
From: Joseph Borreggine, DPM

Is anyone afraid of how this is going to affect the delivery of healthcare, especially those who need wound care, such as the diabetic and venous ulcer population? Funny how dermatology is exempt, even though they create wounds with their MOHs procedures. Of course, emergency rooms and hospitals probably need to be exempt since they’re not seeing chronic wounds.

Nothing from the APMA. This is a serious situation for our profession and for wound care in general. Delay of care and treatment is what this is looking like to me. The recently proposed LCD changes that have been delayed, I guess, were not going to suffice. So, what was next? A “voluntary” program for pre-payment audits and pre-authorization, all controlled by AI. I think just everybody is like “whatever”, especially if this does not affect your CMS MAC and/or jurisdiction. But, this is moving quickly. The notice was dropped last month from CMS and will be implemented on Jan 1, 2026. This is very unusual indeed since the government never moves this quickly. Why so fast? Has anyone posted a comment on regulations.gov?

The recent DOJ indictments showing $14.6 billion in waste, fraud, and abuse certainly should open some eyes, but I guess that did not either. If you read through those cases, you’ll see a number of doctors, companies, and wound care clinics were all involved in multiple states, but it is what it is. Whether that money will ever be recouped is another story. However, full steam ahead with a need policy on wound care will fix that. Hopefully, some recent articles PM News posted are going to create some meaningful discussion as we go forward. I think the amount of money that podiatrists can make in wound care will never be like it was over the last 10 to 15 years. I think the party is over.
