KY Podiatrist Explains Why Diabetics Should Wear White Socks
 
Chirag Patel, DPM, a podiatrist for New Horizons Foot and Ankle Associates in Lexington, Kentucky, says that the color of socks diabetics choose makes a difference. In particular, he recommends wearing white socks, which can help alert them to foot problems that they may not notice due to reduced sensitivity in the feet.

"Diabetics tend to lose sensation on the bottom part of their feet, resulting in higher risk of ulcers and wounds," Dr. Patel explains. "If a non-diabetic were to have a pebble in their shoe, they would immediately remove it due to the discomfort. A diabetic with loss of protective sensation would not feel the pebble and continue to wear the shoe. Of course, this could result in compromise of the skin on the bottom of the foot... such as tenderness, a cut, or worse—an ulcer." By wearing white socks, they would be more likely to notice other symptoms of a skin problem such as drainage or bleeding.

Managing the financial aspects of a medical practice can be challenging, especially while navigating the increasing complexities of healthcare reimbursement systems. Here are some specific areas you can control that will help enhance your practice finances, ensuring a healthier financial future.
 
2. Ensure accurate billing. 
 
By controlling what and how patients are billed for services received, you can clearly communicate the complexity and extent of your patients' medical care and ensure fair and accurate charges. It is critical that the proper amount is billed for each code and that the correct insurance company is billed. Additionally, you can use your clinical judgment to determine the appropriate level of service and billing codes that align with the patient's condition and the resources utilized during the encounter. Accurate billing also helps promote transparency and trust in the healthcare system and reflects a high level of ethical standards for the practice.

Ronald J. Bruscia, DPM, a foot and ankle surgeon in Katy, Texas, passed away on February 4, 2023, at the age of 75. He cared for podiatric patients in Katy and the greater Houston area for more than 30 years.

Bruscia received his Doctor of Podiatric Medicine from the Ohio College of Podiatric Medicine in Cleveland, Ohio. He then completed his foot and ankle surgical training through the Harris County Podiatric Residency Program in Houston, Texas. He was a Fellow of the American College of Sports Medicine and was board certified through the American Board of Multiple Specialties in Podiatry.

Since the state legislatures pass the laws governing the scope of practice in each state, how would you go about accomplishing the task of convincing all 50 legislatures to agree on a scope of practice? It might be easier to pass the 28th amendment to the US Constitution defining a podiatric scope of practice. The Equal Rights Amendment still hasn't been passed, so taking up the question of what podiatry should be in all the states and territories seems to be a long way away.

Editor's Comment: The Chinese philosopher Laozi said, "A journey of a thousand miles begins with a single step." Attaining a uniform podiatry billl would require the APMA to get neighboring states to Florida and Georgia to adopt their scope of practice and continue to spread this process to neighboring states.
 
Of course, I have always supported a different path such as changing our degree. Regrettably, even though PM News polls show that the majority of practicing podiatrists supports this, podiatric leadership does not.

Please know that APMA has in fact undertaken significant research among both members and non-members to understand the value they derive (or do not derive) from their organizations. APMA conducts research among its members on a regular basis and most recently worked with a nationally known research firm in 2021 to complete research on our member value proposition. That research, which included qualitative and quantitative measures among nearly 1,000 physicians, demonstrated that advocacy was a top priority among podiatrists. APMA supports and defends members’ ability to practice every day. You can learn more about recent advocacy victories (including several in the reimbursement arena) at www.apma.org/victories. Our research also demonstrated that members and non-members alike have varied priorities depending on their level of experience, practice setting, engagement with their associations, and more. I was around in the mid-eighties when the nail avulsion guidelines were established. Right then, I knew there was a problem. The issue was one of enforced overtreatment. To get paid to relieve a potentially dangerous condition, especially in geriatric and diabetic patients, was to make one or more injections to a toe with local anesthesia, when freeze spray anesthesia was often adequate. Using freeze spray also cut the time to a fraction of the other procedure. And when you are dealing with an anxious patient, time is trauma.
 
Also, strange requirements were made to incise eponychial tissue that seemed to be 'make work' type of actions that were not clearly correlated to the issue that was causing the pain and incipient infection. In other words, I was on the razor's edge of 'assault and battery'. All that was required was removal of 'the offending spicule" and...

DPM degree required; must be board certified and licensed or eligible for a license to practice podiatric medicine.
