MI Podiatrist Discusses Bunions
 
According to Conway McLean, DPM, "Hallux valgus is more likely to occur with certain foot types. A common scenario is one in which the arch rolls down excessively (pronation). Preventing this extra motion of the arch will go a long way toward stopping or reducing the drift of the first metatarsal. How best to achieve this will depend on numerous factors. An accurate, detailed analysis of anatomy and biomechanics can provide this information, allowing the formulation of a plan for prevention. "Over a hundred different surgical procedures have been used to correct hallux valgus over the years. Many of these are historical, yet a variety of techniques continue to be utilized. Much depends on the age of the individual, their symptoms, the specifics of the deformity, as well as their biomechanical function. The conclusion is that one size does not and should not fit all."

"Not everyone needs to have a complete reconstruction, with cutting of the metatarsal bone and realignment of the head. For some, a simple bump removal is all that is required to resolve their complaints. One approach with a long history of success involves cutting (precisely) through the neck of the first metatarsal bone and shifting the head over, back into position. Different bone cuts have been performed, each with its own nuances and drawbacks. But various other components are typically involved in the constellation of procedures used to treat the deformity," says Dr. McLean.

5 Commonly Overlooked Cybersecurity Practices in Fast-Growth Specialty Groups
 
While there are more than five best practices that specialty provider groups should have in place, the following are commonly overlooked or completed haphazardly.
 
1. Comprehensive risk analysis
 
It’s common to find that a medical practice’s risk analysis is inadequate, often because they opt to perform a high-level assessment of security controls instead of a comprehensive asset-based risk analysis. A high-level assessment doesn’t include specific assessments of unique vulnerabilities, threats, and mitigating controls that may exist with individual information assets, for example, EHR systems, patient portals, telehealth apps, and imaging systems. There have been hundreds of attacks on practice groups that have exploited unprotected vulnerabilities, leading to highly preventable breaches. It’s also important to note that a risk analysis that doesn’t include all an organization’s information assets and their components puts it at risk of non-compliance with HIPAA. All organizations that have reported a breach of 500 or more records are automatically investigated by the Office for Civil Rights, which has publicly stated that compliance with its Final Guidance for risk analysis is its number one focus.

Dr. Weiss, I have been involved with speech to text and integration with EMR systems for about 25 years. I beta tested 5 AI medical charting (Ambient listening) systems. After working with the companies' programmers, I have been live in the office for over 5 months now. There are major differences between the products. I am a medical advisor for Heidi Health AI, since it was very clear to me the differences. There is a podiatry specialty. There is even a podiatry assistant and one for about 50 different specialties. They are also showing at the APMA meeting this week and are one of the sponsors of the meeting. I am at the booth. The big differences are the ability to use the product the way you want to, and be able to tailor it for your personal needs. 
 
Many products only have 1 or 2 ways to make a note. With Heidi you can make any template or any note you want to. We use it for our back to school forms, patient summaries, referral notes, etc. It listens to me and my patient. I push a button. It creates a note in 30 seconds. It automatically creates the SOAP or H&P note. I re-read, if needed, for minor edits and done. It is so much faster than using Dragon. I haven't touched it since starting AI charting. There will be more on this in the September issue of PM magazine. Please reach out anytime.

Kudos to Dr. Pressman for pointing something out which I have been preaching for my over 40+ years as a clinician. I first came aware of non-invasive vascular technology back in the late 1970s and early ‘80s during my undergraduate medical education at rotations at various VA hospitals in the Chicago area. In those days, the machines were big, bulky, and took up an entire room. Their costs and size relegated them mostly to large clinical or research facilities.
 
With computer technology, eventually the machinery got more sophisticated and totally paper free and can integrate directly into your computer and eventually into your patient's EMR. Most cost 1/3 of what digital x-rays cost, with many fitting into your briefcase. The current machinery can combine pulse volume recording and photoplethysmography, which are far more sophisticated than...

Congratulations and best of luck to Dr. Jon Hultman as he retires as Executive Director from CPMA! Jon is not just a superstar for CPMA but also APMA. Back in the ‘90s, he was a significant contributor to the Health Systems Committee. He helped the APMA members understand about capitation. His book, Here’s How Doctor, taught us how to survive in the new healthcare market of HMOs and PPOs. Jon taught me a lot when we were on the Health Systems Committee together and for many years thereafter. Thank you for all you have done to move our great profession forward.
