AZ Podiatrist Discusses Research and Diabetes Detection Dogs
Ben Pearl, DPM and David Jenkins, DPM discussed the evolution of research in podiatric sports medicine, with ongoing projects related to diabetes and exercise on Dr. Pearl’s podcast Fit Foot U. They also explored the capabilities of dogs in detecting various conditions and the importance of evidence-based medicine courses for clinicians.

How Podiatry School Helped Canadian Student in Artistic Swimming
Things have changed a lot for Jacqueline Simoneau in the last three years. After the last Olympic Games Tokyo 2020 (held in 2021), the Canadian decided she'd had enough artistic swimming. Having given a decade of her life to the sport, Simoneau decided to take some time for herself and chase another dream: completing her studies in podiatry at the University of Quebec.

In her time away from artistic swimming, the sport changed. A new scoring system was introduced, and men were given the go-ahead to compete in team events at the Olympic Games. The changes re-lit a fire within the 27-year-old, who made a successful return to the sport by winning her first World Championships gold in Doha in February after trying for nine years.

A Systematic Review of Diabetic Foot Infections
Results: The search through the databases identified 13 articles with 2,545 patients from 2021 to 2023. Overall, the predominant Gram-positive microbial species isolated were Staphylococcus aureus, Enterococcus fecalis, Streptococcus pyogenes, Streptococcus agalactiae, and Staphylococcus epidermidis. The predominant Gram-negatives included Escherichia coli, Klebsiella pneumoniae, Proteus mirabilis and Pseudomonas aeruginosa.
Conclusion: Diabetic foot infections represent a complex and multifaceted clinical entity, necessitating a holistic approach to diagnosis, management, and prevention. Limited high-quality research data on outcomes and the effectiveness of guideline recommendations pose challenges in updating and refining existing DFI management guidelines.

A patient presented with a dermatological disorder affecting multiple areas of his body including his face. His dermatologist did a biopsy and diagnosed it as lichen planus. Below is a photo of it.

He said that he absolutely has no pruritis. The cases I have seen throughout my career always presented with severe itching and that is what brought the patient into my office. Could the dermatologists be missing something?

A recently released article discussed this very interesting topic. For those who are dealing with a large diabetic patient population, it is certainly worthy of more review and discussions with the medical specialists managing the patient's glycemic control. The authors discussed their experiences in a 2-year retrospective analysis at 2 Level 1 Trauma Centers. Their conclusions were that in certain patient populations, the fructosamine levels were a better predictor of perioperative complications of infection, dehiscence, joint replacement failure, and others than HbA1C.
I am left wondering if other podiatrists, including Dr. Allen Jacobs, have had similar experiences with fructosamine levels in their patient population, including office-based and or other outpatient elective surgical patients. For more information, Please click here

RE: Replace Your Neck Stethoscope with a Hand-Held Doppler to Save Limbs

Podiatry standard of care requires us all to palpate pedal pulses. This standard alone is simply not high enough to get the job of limb preservation done. The following list contains some hard won knowledge acquired over the last 47 years of practice; hard won pearls given freely with the hope that some of the 20,000 subscribers might take notice. This note was motivated by a leg-off case where the podiatrist found +2 pulses and 9 days later the patient had an AK amputation for thrombosis. The current standard of care is pulse palpation. If a Doppler were used, perhaps this could have been avoided.
1. Your cardiologist does not palpate your heart; she uses auscultation with a stethoscope to hear cardiovascular flow abnormalities.

I was saddened to learn of Larrie Rockmacher’s passing. Larrie was an excellent podiatrist who provided my first job opportunity following my residency at Kern Hospital, our mutual alma mater. He was a generous and progressive practitioner who led the way for many who followed in northern Westchester County, NY. It was truly a privilege and joy to have been associated with him early in my practice career.

I am deeply saddened to hear of the passing of Dr. Lyman Wilson. Somewhere in the back of my mind, I thought that our wonderful mentors would live forever. Dr. Wilson was a superb example of integrity, professionalism, compassion, and mentorship. He helped to advance our profession tremendously, especially at a time when orthopedic surgeons were taking bold steps to limit our scope of practice and minimize our ability to educate our students, residents, and fellows.
Through his leadership, Dr. Wilson took a strong stance to advance our profession at our hospital, and on both state and national levels. He dedicated his time to educate residents in this program alongside other attending staff like Nicholas Grumbine, Richard Viehe, Greg Tilley, Tim Byron, Dale Rosenblum, and many others. Together, they constituted a strong fabric of education and excellence in our community.
I will be eternally grateful to Dr. Wilson for his guidance and mentorship. May he rest in peace with grace.
